Description
Corporal Michael J. Crescenz VA Medical Center (Philadelphia) Statement of Work -FY16 SOW for Specialty Bed and Mattress Rental B.2 - SERVICES AND QUANTIES /PRICES/COSTS A.Furnish specialized rental beds and support surfaces for veterans admitted in acute care and long term care settings at the Corporal Michael J. Crescenz VA Medical Center and Community Living Center located at 3900 Woodland Avenue, Philadelphia, Pennsylvania. Items need to be available 24 hours, 7 days a week including holidays within 2-6 hours of call in time to contractor. Pricing must include all services and requirements stated in the solicitation. The contract will be for a base and four (4) option years to renew. All quantities listed herein are estimated only. The Government is not obligated to any specified amount of services, but shall be obligated to provide payment for all services performed in accordance with this solicitation. B.3 - DESCRIPTION/SPECIFICATIONS/REQUIREMENTS A. SERVICES: Contractor shall deliver, repair and exchange the required Medical deliverables directly to patient care units within a reasonable time within six (6) hours (8a-5p, Monday through Friday), within 50 miles. For calls received outside this time, add one hour. If for any reason any deliverable cannot be delivered within the scheduled time, the contractor shall call and cite reasons for delay to the contact person, normally the Wound Care Nurse, Off Tour Supervisor, or the individual who ordered the product and give a time when product will be delivered. 24/7 Care Center availability to place orders or report service issues. Upon discharge of patient or no longer needing the deliverable, notification will be given to contractor to pick up used product within 36 hours of request. Only assigned nursing personnel or assigned administrative personnel are able to order the needed specialty deliverable either by telephone or online ordering. Upon discharge of patient or when no longer needed the same assigned personnel are to call contractor for pickup of used Medical deliverables. Charges begin to incur when the deliverable is delivered on-site and charges end when the contractor is called for deliverable pick-up. The full day charge may not apply if the bed was not in possession for the entire period. Charges may incur for only one bed per patient at a time regardless if the contractor leaves their equipment on-site. The services to be performed by the Contractor will be monitored by the Contracting Officer Representative (COR). The Contractor shall remove any employee from the performance of this contract within five (5) workdays of receiving notice from the Contracting Officer that the employee's performance is unsatisfactory. All Contractor employees are subject to immediate removal from performance of this contract when they are involved in a violation of the law, VA security, confidentiality requirements and/or other disciplinary reasons. Training of clinical staff will be needed if vendor product is selected - hands-on and e-learning modalities. Installation and set-up of each placement. Patient transfer assistance at time of placement. Disinfecting and cleaning between each placement and periodic cleaning as needed. Ongoing user training and in-servicing as needed. Pre-installation maintenance and electrical safety checks between each patient use. Clinical staff to: help recommend the right frames and surfaces in all areas of patient care; help create discharge and ICU transfer protocols to reduce readmission rates, participate in clinical rounds. Rental activity reports Invoices shall be certified by the Contracting Officer's Representative (COR). Payment will be made upon certification of invoice. Invoicing will be done monthly in appears. These Beds and other supplies are ordered per Veteran on an as needed basis. The following units utilize the beds and equipment with various numbers of rented per unit depending on the veteran's needs: -Medical Center: ED, OR, PACU, SPU, MICU (Medical ICU), SICU (Surgical ICU), 5E & 5W (Medical-Surgical Floors), 6E & 6W (Medical Surgical Floors), &E & 7W (inpatient behavioral health units) -Community Living Center: 1B, 1C, 2B, 2C -Residential Rehabilitation Treatment Program (RRTP) D.ORDERS: - Request for services will be made by telephone or via internet by the Would Care Nurse, Bed Coordinator, Patient Care Coordinator or nursing unit management teams. The contractor will be informed what type of commodities is needed, what room and location it is going to be used in, name of Veteran, and when it is needed. - The Contractor's employee will report to the unit nurse on duty but not before checking into the VA police department upon arrival. The supply will be placed in the Veteran's room under the direction of nursing. The contractor will leave a delivery ticket with the unit nurse and the billing will begin once the beds or other items are in the room and in use). - Whenever the bed or other supply is no longer needed for Veteran, the Contractor will be notified to pick up the bed. Billing will stop at the time the Contractor is contacted that the bed is no longer needed. - A memo designating the Contracting Officer's Representative (COR) will be provided to the contractor.
