THIS IS A SOURCES SOUGHT ONLY. NO ACTUAL QUOTES OR PROPOSALS ARE BEING SOUGHT AT THIS TIME. The purpose of this Sources Sought notice is solely for information and planning purposes and does not constitute a solicitation. This is for market research purposes only. Do not include pricing information; it will not be evaluated. The Government will not pay for any information submitted under this notice. This notice is to assist the VA in the determination of sources only. A solicitation is not currently available. Responses to this Sources Sought Notice are not a request to be added to a prospective bidders list or to receive a copy of the solicitation. The Department of Veterans Affairs, VA Central Iowa Health Care System (VACIHCS), is currently seeking information from any company regarding their capability to provide a single Community Based Outpatient Clinic (CBOC) providing Primary Care services to Veterans primarily residing in the Fort Dodge, Iowa area. In accordance with Public Law 104-262, Title 38 United States Code (USC) 8153, primary care services in a community based outpatient clinic setting are to be furnished by the contractor to eligible veterans on behalf of VA Central Iowa Health Care System. Primary Care services include longitudinal outpatient medical care the purposes of prevention and detection of disease and subsequent management of medical conditions, for veterans deemed eligible. The care shall be provided by primary care providers who are board certified in Internal Medicine or Family Practice, with experience in the care of adult patients with multiple medical problems of high complexity and Physician Assistants/Nurse Practitioners who are nationally certified and licensed to practice within a primary care environment. Additionally, the proposed CBOC is expected to comply with all relevant VA policies and procedures, including those related to quality, patient safety and performance. The care provided by the Contractor should be patient centered, continuous, accessible, coordinated, and consistent with VA standards, including the thirteen service standards detailed in VHA Directive 2006-041, "Veterans Health Care Service Standards," dated 6/27/06 (2006-041 expired on June 30, 2011 but will still be effective until a revision or rescission is published and/or subsequent revisions thereto. http://www.va.gov/vhapublications/ViewPublication.asp?pub_ID=1443 Contractor facility must be physically located in Fort Dodge in the state of Iowa and be at least 5 miles from the VA Behavioral site located at 102 North 25th Street, Fort Dodge, IA 50501. The Parent Facility for this CBOC is the VA Central Iowa Health Care System, 3600 30th Street, Des Moines, IA 50310. These services are solely dedicated to Veteran patients of the VACIHCS. The VA seeks to gain knowledge of potential qualified sources and their size classification (HUBZone, 8(a), small, small disadvantaged or service disabled veteran owned small business) relative to NAICS Code 621111, Offices of Physicians (except Mental Health Specialists); size standard of $11 Million. Responses to this Sources Sought will be used by the Government to make appropriate acquisition decisions and developing a potential procurement strategy. After review of responses to this sources sought synopsis, a solicitation announcement may be published in the FedBizOpps website. Responses to this sources sought synopsis are not considered adequate responses to the solicitation announcement. All interested offerors will have to respond directly to any corresponding solicitation by submitting a proposal. The intended contract period would be for one base year and four, one-year option periods.
