- Solicitation Package (54 pages) This is a combined synopsis/solicitation for commercial services/items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals/quotations are being requested. This solicitation is being issued as a Request for Quotation (RFQ). Submit only written quotations for RFQ AG-84N8-S-16-0062. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-85-1. This acquisition is 100% set-aside for Total Small Business. The associated NAICS code is 238330 (Floor Contractors) with a size standard of $15.0. The line item description is as follows: The Ashley National Forest intends to remove and replace carpet and sheet vinyl flooring in Forest Services offices located in both Vernal, UT and Manila, UT. The Schedule of items includes Base Work (with an estimated 461 SY of carpeting and 49 SQ of vinyl), Option 1 (with an estimated 76 SY of carpeting) and Option 2 (with an estimated 97 SY of carpeting) Work. Bid shall also include 60 "spare" tiles for future patches to be left on site with the COR. The estimates provided are estimates only (reference Section 000050-Project Description, Part 1, paragraph 1.1, section A). Vendors must bid on all items, including removal of existing carpet and sheet vinyl and floor preparation and installation of new products. The Government reserves the right to award any and all items. The anticipated project completion date is June 17, 2016. Offerors should reference the attached solicitation package and attachments in order to obtain a complete understanding of the project objectives and requirements. The provision at 52.212-1, Instruction to Offerors-Commercial, applies to this acquisition. The following addenda to this provision also apply to this acquisition: 52.204-13 System for Award Management Maintenance. (JUN 2013) 52.232-40 Providing Accelerated Payments to Small Business Subcontractors. (DEC 2013) Instructions for the Preparation of Quotations Offerors shall submit with their quote a narrative describing its experience and technical capability as it relates to this requirement. Evaluation of offers will be conducted using Lowest Price Technically Acceptable. The evaluation criteria, fully listed in the solicitation package, are as follows: A contract will be awarded to the lowest-priced technically acceptable offeror. Quotes will be evaluated for acceptability but will not be ranked using the non-cost/price factors. In order to be eligible for award, offerors must receive a rating of "acceptable" in every non-price factor/subfactor. In the interest of economy and efficiency, the Government may start by evaluating the technical proposal of the lowest-priced offeror and will award to that offeror if the technical proposal is rated "technically acceptable". If the lowest offeror receives a rating of "unacceptable" in any of the non-cost factors, the Government will then move on to evaluating the technical proposal of the next lowest offeror and so on until a proposal is rated "technically acceptable". Therefore, the technical proposals of the non-low price offerors may or may not be evaluated. The Factors and Subfactors below will be used to determine technical acceptability. Offerors must receive a rating of "acceptable" on all Factors considered for award. Factor#1: Technical Approach In order to receive a rating of "acceptable" for this factor, offeror's must achieve a rating of "acceptable" for all of the following subfactors: Subfactor #1: Project Work Plan In order to receive a rating of "acceptable" for this subfactor, offeror must, at a minimum, provide a logical general project work plan indicating how you intend to accomplish the work. The project work plan must indicate how the work will be performed in accordance with the solicitation and drawings/specifications. Subfactor #2: Progress Schedule In order to receive a rating of "acceptable" for this subfactor, offeror must, at a minimum, provide a proposed progress schedule outlining time frames and sequence of work for project completion. The delivery date should meet the project completion time requirements set forth in this solicitation but must also be realistic and achievable. Subfactor #3: Quality Control Plan In order to receive a rating of "acceptable" for this subfactor, offeror must, at a minimum, provide an acceptable Quality Control Plan that demonstrates how your company will assure quality meets or exceeds the requirements of the drawings/specifications and solicitation terms and conditions. Factor #2 Experience In order to receive a rating of "acceptable" for this factor, offeror must, at a minimum, provide at least three references for relative related projects completed within the past three years. References should include a Point of Contact Name, Phone Number and project description. Factor #3 Past Performance In order to receive a rating of "acceptable" for this factor, offeror must, at a minimum, provide information on customer satisfaction and problem resolution. The offeror must have an adequate performance record in order to be determined "responsible" for award, in accordance with FAR 9.104-3. --- The provision at 52.212-3, Offeror Representations and Certifications-Commercial Items, applies to this acquisition.
