Description
This is a SOURCES SOUGHT ANNOUNCEMENT ONLY The Department of Veterans Affairs, Network Contracting Office (NCO 21), has a requirement for a contractor to provide specialized pathology, microbiology and 24 hour STAT clinical laboratory testing for the VA Palo Alto Health Care System. Services sought must be provided by a clinical laboratory that is College of American Pathologist (CAP) and Clinical Laboratory Improvement Amendments (CLIA) accredited and located within a 20 mile radius of the VA Palo Alto laboratory located at 3801 Miranda Avenue, Palo Alto, CA, 94304. The laboratory must provide 24 hour a day, 365 days a year courier service. Specimens must be picked up by a courier from the VA Palo Alto laboratory at the address above within 30 minutes of the call/order and returned to the testing laboratory within 30 minutes of leaving the VA Palo Alto laboratory. This is neither a solicitation announcement nor a request for proposals or quotes and shall not obligate the Government to award a contract. Requests for a solicitation shall not receive a response. Responses to this sources sought announcement shall be in writing. Responses to this notice should be sent via email to William Tanksley, Contracting Officer, at william.tanksley2@va.gov. The purpose of this sources sought announcement is to gain knowledge of potential sources and to identify parties having an interest in and capable of provided the requested services. If your organization is interested in providing these services under contract, please respond to this announcement by 2:00 PM PST, April 26, 2016, and include the following information: 1.Name of organization, address including zip code, point of contact, telephone number, and email address. 2.Confirmation of organization's active System for Award (SAM) registration with expiration date. 3.Business size and classification to include any designations as Certified Small Business, HUBZone Certified Small Business Concern, Section 8(a) contractor, Service Disabled Veteran Owned Small Business, Small Disadvantaged Business, Women Owned Small Business, or Large Business Concern. 4.Current copies CAP and CLIA laboratory licenses 5.Provide verification that all tests listed in the table below can be completed at the contractor's laboratory located within a 20 mile radius of the VA Palo Alto laboratory and within the specified times. 6.A brief overview of how your organization would provide the services included in items 1-6 of Section A listed below along with how your organization would comply with the requirements included in Sections B through E also listed below. A. Test results must be reported to the VA Palo Alto laboratory staff via telephone verbally and with faxed test results. 3.Tests designated as Specialized must be resulted within 48 hours of receipt in the testing laboratory, unless the test takes longer than 48 hours to complete as pre-defined in the contractor's standard operating procedure for that test. Test results must be reported to the VA Palo Alto laboratory staff via telephone verbally and with faxed test results. 4.Any test result defined as critical in the contractor's standard operating procedure must be called to a VA Palo Alto laboratory Medical Technologist immediately (no longer than 30 minutes) after being obtained. 5.Ability to provide test reports with at a minimum the following information: a.Patient's name and/or identification code (Social Security Number) b.Physician's name (if supplied) c.Lab Accession number (if supplied) d.Date/time specimen received in the contractor's laboratory e.Test(s) ordered f.Date/time of specimen collection (if supplied) g.Date test completed h.Test result i.Abnormal flags (H, L * critical) j.Reference range k.Therapeutic range (if applicable) l.Name of contractor's laboratory m.Type of specimen n.Any additional comments related to test provided o.Any information the testing laboratory has that may indicate a questionable validity of test result(s) p.Unsatisfactory specimen shall be reported with regard to its unsuitability for testing 6.Provide STAT courier service 24 hours a day, 365 days a year. Specimens must be picked up by a courier within 30 minutes of receiving the call/order from the VA Palo Alto laboratory and be delivered to the testing laboratory within 30 minutes of leaving the VA Palo Alto laboratory. B. Location of where services would be provided: Services must be provided at a clinical laboratory located within a 20 mile radius of the VA Palo Alto laboratory. C. Hours of operation: Twenty four hours a day, 365 days a year. D. Contractor qualifications: 1.The laboratory must be College of American Pathologist (CAP) and Clinical Laboratory Improvement Amendments (CLIA) accredited. 2.Technologists, medical technicians, and cytotechnologists must meet personnel qualifications required by CAP and CLIA '88. 3.Pathologists must have graduated from an approved school of medicine or osteopathy and completed a residency or fellowship in pathology, be Clinical or Anatomic Pathology board certified and must possess a valid license to practice clinical reference laboratory services from a United States (U.S.) jurisdiction. 4.The laboratory Director shall be a licensed American Board of Pathology certified pathologist. Responses to this notice are strictly for market research purposes and will be used by the Government to make appropriate acquisition decisions. This notice shall not be construed as a commitment by the Government to issue a solicitation or ultimately award a contract nor does it restrict the Government to a particular acquisition approach. The Government will in no way be bound to this information if any solicitation is issued. The North American Classification System (NAICS) code for this acquisition is 621511 and the applicable size standard is $32.5M.
