PAGE 1 WITH BLOCKS 17A COMPLETED WITH THE CAGE CODE ANNOTATED AND BLOCK 30 A-C (COMPLETED/SIGNED) 2. PAGE 4-20 WITH THE LABOR RATE PER HOUR, TRAVEL LABOR RATE PER HOUR, AND TOTAL ESTIMATED AMOUNT FOR RM ANNOTATED ON EACH CLIN (0001, 0002, 1001, 1002, 2001, AND 2002). 3. PAGE 68 WITH DFARS CLAUSE 252.209-7991 (b)(1) and (b)(2) ANNOTATED. NO LATE PROPOSALS CAN BE ACCEPTED.
