Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation VA250-16-Q-0226 is being issued as a Request for Quotation (RFQ), with the intent to award a Firm-Fixed Price contract for services. All solicitation documents and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-60. The contract duration is from award date to completion. This requirement is restricted-sufficient small business for commercial items under the North Atlantic Industrial Classification System (NAICS) 238210. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, Lowest Priced Technically Acceptable factors and Socioeconomic status are considered in the evaluation. All offerors must be registered in SAM. The following commercial service is requested: STATEMENT OF WORK The Dayton VA medical Center is seeking a Contractor to perform the following: The Contractor shall provide all necessary labor, management, supervision, technical skills, quality control, materials, tools, transportation, equipment, demolition of existing items, performance of work, and Design-Build services necessary to replace out dated Capacitor and Fans in O.I.&T.'s UPS Cabinet Equipment Tag # 1444849. Contractor to provide all Material, Labor, Equipment and Supervision necessary to: 1. Removal of existing Caps and Fans. 2. Installation of new Caps and Fans. 3. Disposal of old Caps and Fans. 4. Service to include Basic Installation. Replacement work to be performed during a scheduled preventive maintenance visit. In the event that the site requirements fall outside of the Basic Installation Service needs the Contractor will provide the Dayton VA Medical Center with an additional quote before work begins. Work to be done Monday thru Friday 8am to 5pm.and performed in strict accordance with the terms, conditions, and specifications, contained or referred to in the request. Contractor shall provide all necessary labor, management, supervision, technical skills, quality control, materials, tools, transportation, equipment, demolition of existing items, performance of work, and Design-Build services necessary to replace out dated batteries in O.I.&T.'s UPS Battery Cabinet Equipment Tag #s 1345374 & 1345375 Battery Model-HX400 40 Jars/Batteries. BATTERIES HAVE REACHED THE END OF USEFUL SERVICE LIFE, replacement is recommended. 1. Removal of existing battery strings. 2. Installation of new battery strings. 3. Recycling of old batteries. 4. Service to include Basic Installation. In the event that the site requirements fall outside of the Basic Installation Service needs the Contractor will provide the Dayton VA Medical Center with an additional quote before work begins. All work shall be done Monday thru Friday 8am to 5pm. and performed in strict accordance with the terms, conditions, and specifications, contained or referred to in the request for quote.
