Description

CLEMENT J. ZABLOCKI MEDICAL CENTER ENHANCED-USE LEASE DEVELOPMENT PROJECT, MILWAUKEE, WISCONSIN (VA10116R0084) Question In regards to Bldg #2 (Old Main), there appear to be plan area pieces missing from the plan set: 1st floor original building (front of bldg to dining hall/kitchen addition junction) and 2nd floor (original building back wing). Please advise if these are available to issue as well. Also, if there are exterior elevational drawings for Bldg 1 & 2., these would also be helpful but not as critical as the missing floor plan areas. Response: The elevational drawings for B 1 and B2 are attached, as well as the missing plans for B2. The Site visit is now scheduled for March 11 at 10:30 in the Building 43 Dining Room with registration scheduled to start at 9:30. This procurement is for an Enhanced Use Lease (EUL) opportunity at the Clement J. Zablocki Medical Center Enhanced-Use Lease Development Project, Milwaukee, Wisconsin. The VA objective for this RFP is to receive proposals from one or more Offerors to develop the sites to provide supportive housing to homeless and near homeless individuals, and their families with a preference for Veterans under long-term ground lease agreements with VA for up to seventy-five (75) years. In return for a long-term lease of the property, the selected Offeror(s) will provide fair consideration to VA. The United States Department of Veterans Affairs ("Department" or "VA") on behalf of the CLEMENT J. ZABLOCKI VA Medical Center is soliciting competitive proposals from private, public, and/or not-for-profit development offers in leasing and developing the subject property as described in the attached RFP. The Clement J. Zablocki VA Medical Center is located in Milwaukee, Wisconsin. The Medical Center (VAMC) has Community Based Outpatient Clinics (CBOCs) in Appleton and Green Bay, serving 11 counties in North Central Wisconsin; in Union Grove, serving 4 counties in Southeastern Wisconsin, and in Cleveland, serving 4 counties in East Central Wisconsin. The VAMC delivers primary, secondary, and tertiary medical care in 194 acute care beds (including Psychosocial Residential Rehabilitation Treatment Programs) and provides 600,000+ outpatient visits annually through an extensive outpatient program. The Community Living Center (long term care) units of 113 operating beds offer geriatric programming that preserves and enhances the patient's dignity. The 356 operating domiciliary beds are the focus of several innovative programs, including an Alcohol/Drug Rehabilitation Unit, a Rehabilitation and Treatment Unit for Veterans with chronic mental health problems, and a Transitional Program that helps Veterans successfully return to life in the community. The Medical Center also assists the Veterans Center located in the City of Milwaukee. In addition, the Milwaukee VAMC has maintained a partnership with the Center for Veterans Issues (CVI) for the provision of grant and per diem services for homeless Veterans at two sites in Milwaukee and Union Grove, Wisconsin (Vets Place Central, in Milwaukee and the Veterans Assistance Program in Union Grove). All of Milwaukee's programs serve a Veteran population of 321,421 in the southeastern and central parts of Wisconsin (Central Market Veteran population for 2005). The VAMC is a part of Veterans Integrated Service Network (VISN) 12, which includes the Iron Mountain VAMC in Michigan; Tomah VAMC in Wisconsin, William S. Middleton Memorial Veterans Hospital in Madison, Wisconsin, North Chicago VA Medical Center in Illinois (future site of the Lovell Federal Ambulatory Care Center), Edward Hines, Jr., VA Hospital in Chicago Illinois, and the Jesse Brown VAMC in Chicago. Special programs include interventional radiology, cardiac surgery, comprehensive cancer care including radiation therapy, an extensive telemedicine program with Iron Mountain, MI VAMC, a Spinal Cord Injury Unit, and, in long term care, a Geriatric Evaluation and Management Program and a palliative care program. The Medical Center is the VA Great Lakes Health Care System's northern, tier hub for both imaging, and pathology/laboratory medicine.
