- Solicitation - W912J3-16-T-5002 Occupational Health Medical Exams in New Mexico Wage Determination - 15-2517 (Rev.-2) Dated 8 March 2016 Wage Determination - 15-2511 (Rev.-1) Dated 23 Feb. 2016 Wage Determination - 05-2361 (Rev-18) Dated 5 January 2016 The New Mexico Army National Guard (NMARNG), Santa Fe, New Mexico is soliciting on a 100% small business set-aside for services to provide Occupational Health Surveillance Physical Examinations in accordance with the Occupational Safety and Health Administration (OSHA) in various locations in New Mexico during an estimated period of performance 23-31 May 2016 in accordance with the Performance Work Statement (PWS). The estimated number of personnel to be examined is a maximum of 140. The Government intends to award a onetime Firm Fixed Price (FFP) contract for the services defined in this requriement. A solicitation will be issued along with this synopsis in accordance with FAR 12.6 - Streamlined Procedures for Evaluation and Solicitation for Commercial Items. A solicitation for this requirement will be available immediately on Federal Business Opportunities website (https://www.fbo.gov). All prospective offerors are responsible for retrieving the solicitation from this website, and to frequently monitor that site for any amendments to the solicitation. A performance work statement (PWS) detailing the specifics of this requirement is included in the solicitation. The Government estimates that an award will be made by mid May 2016 to the responsible offeror whose offer, conforming to the solicitation, will be most advantageous to the Government. To be considered for this award the contractor shall: 1.) Be able to meet all requirements defined in the synopsis, solicitation, and PWS; 2.) Submit their proposal in accordance with FAR Clause 52.212-1 and 52.212-2; 3.) Be registered and current in the System for Award Management (SAM) database per DFARS 252.204-7004 (www.sam.gov); 4.) Contractor shall be able to invoice through Wide Area Work Flow (WAWF) (https://wawf.eb.mil) for payments to be processed electronically. All questions pertaining to the details of the solicitation, or the PWS itself shall be submitted to NMARNG Contract Specialist John P. McLaughlin by email: john.p.mclaughlin74.civ@mail.mil, by 3PM MST on 21 April 2016; Questions will not be accepted after this date and an amendment ot the solicitation will be made by the next day with all vendor submitted questions and answers. All responsible sources may submit one (1) quotation in accordance with the Solicitation and shall be considered by the agency by responding to this solicitation or via email to John P. McLaughlin by 3PM MST on 4 May 2016.
