- Patient Scales RFI Word Document Patient Scales RFI Vendor Response Workbook 1.1 General. The purpose of this request is to obtain information from industry to support the government's intent to establish a Qualified Supplier List (QSL) for the Standardization of Patient Scales. Examples of the variety of scales considered for the QSL could include but are not limited to: Patient Scales for adult and infant/pediatric application. This may include both stand-on and wheelchair scales, in portable and stationary configurations, as well as digital weight indicators or mechanical weigh-beams. This Request for Information (RFI) is an invitation to submit to Department of Defense (DoD) and DHA MEDLOG the following: Technical information and all related supporting documentation for Patient Scales and associated components and accessories. This request is a market research effort undertaken by the Government which will be used for information gathering purposes. The information received will be used for Acquisition Planning purposes and will help define future requirements for the subject item. 1.2 RFI Submission Package. Vendors must provide responses to this RFI via the RFI Vendor Response Package Worksheet. Complete answers and all supporting documentation must be provided with the vendor's submittal. These responses will be directly related to the characteristics as specified in section 4 which are subject to change. Responses for each salient characteristic shall be in the form of yes, no, or alternate proposal, unless quantitative details, such as equipment dimensions, are requested. Although explanatory comments may be provided for all responses, they are required for "no" responses. The Medical/Surgical Prime Vendor (MSPV) Program requires that all items be manufactured in a Trade Agreements Act (TAA) Compliant Country (see DFARS 252.225-7020 AND 252.225-7021), unless it is determined that no functionally equivalent item is available from a compliant source.
