Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 15.203, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (i) The solicitation number for this RFQ (Request for Quote) is VA741-16-Q-0075. (ii) Solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular dated 2 Jul 2015. (iii) This solicitation is published as full and open to all prospective offerors. The NAICS code associated with this is 641618. (iv) Attached Solicitation has line items listed per deliverable of project, to include one option year. (v) The Healthcare Performance Benchmark project is to determine, enable, and facilitate appropriate comparisons of VHA healthcare services with private-sector healthcare providers located in the United States. (vi) Performance period for base 1 Jun 2016 - 31 May 2017 and is scheduled to begin 1 June 2016 at location state on page 1, block 15 of solicitation. (vii) Please see provision at 52.212-1, Instructions to Offerors -- Commercial, for instructions to follow when submitting your quote. (viii) Provision at 52.212-2, Evaluation -- Commercial Items, is incorporated. Please review for the specific evaluation criteria to be used when evaluating quotes. (ix) Prospective offerors must include the a completed copy of the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items, with its offer. (x) The incorporated clause at 52.212-4, Contract Terms and Conditions -- Commercial Items, applies to this acquisition. (xi) Successful offeror must be registered with SAM (System Award Management), have an active account without any Security holds or IRS block in order to be awarded contract. ?
