- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The Kayenta Health Center is currently under construction. The Health Center is requesting for submittal of offers for a contractor to provide the following list of medical equipment/supplies: 1. CLIN #2, Cart, Modular, Supply, Quantity 50: The item shall be a Supply/Procedure Cart with Modular design to allow drawers to be removed and exchanged, offering flexibility in the configuration. Unit shall be made of durable plastic, capable of withstanding cleaning with both hot water (pressure wash) and cleaning solvents typical to the healthcare setting. Casters shall allow for quiet and smooth transport. Drawers shall be provided in multiple sizes, configured for maximum flexibility, but offering a minimum of 1 drawer of not less than 6 inches and 2 drawers not less than 4 inches. Additional drawers shall be provided to offer a complete set of drawers for a combined total of vertical drawer storage not less than 18 inches. Cart shall have a height between 30 and 36 inches. The top shall be a durable and cleanable material, with no sharp edges. The cart must be easy to clean and maneuver. 2. CLIN #12, Cart, Case with Fold-Up Shelf, Caster Base, Quantity 3: The functional design of Case Carts maximizes usable inside space for handling storage and transport tasks efficiently. Carts can be used wherever contents must be protected from an "unclean" environment, including free-standing ambulatory care centers, and hospital based or out-patient surgeries. Material is stainless steel which provides excellent corrosion resistance and its tendency to conduct heat accelerates the drying process. Adjustable shelves allow you to configure a cart to your specific needs and easily reposition them as needs change. Features of Case Carts offer: Stainless steel cabinet and shelves have smooth or protected edges to eliminate ripping of packs that can cause costly waste or reprocessing. Approximate Dimensions: 36" wide x 29" deep x 39" Tall Fold-up side shelf shall add a minimum of 12 in. extra width when extended. 3. CLIN #13, Cart, Fetal Monitor, Mobile, Caster Base, Quantity 3: Fetal Monitor Carts are aesthetically attractive carts that conceal medical monitoring systems in Labor and Delivery environments. These units can be fully customized to suit clinicians' needs, and are made to match the existing casework in the room. Approximate Dimensions: Height: 40.75", Base Depth: 26", Base Width: 22" 4. CLIN #14, Cart, Emergency, Crash, Adult, Quantity 6: A crash cart is a set of trays/drawers/shelves on wheels used in hospitals for transportation and dispensing of emergency medication/equipment at site of medical/surgical emergency for life support protocols (ACLS/ACL) to potentially save someone's life. The contents of a crash cart vary from hospital to hospital, but typically contain the tools and drugs needed to treat a person in or near cardiac arrest. Overall cart dimensions approximately: 44.5" H x 38" W x 22" D (113 cm H x 96.5 cm W x 55.9 cm D), Unit shall include an IV pole: 85" (215.9 cm) H • Color: Red 5. CLIN #15, Cart, Emergency, Crash, Pediatric, Quantity 5: Many emergency departments, use a pediatric resuscitation cart in which each drawer is color coded and organized by patient length and weight ranges; each drawer contains all necessary equipment for resuscitation of a patient in that specific length/weight range. The color-coded resuscitation cart has intuitive advantages over the historically used, intervention-based standard resuscitation cart. In the field of pediatrics in which medication dosing and equipment size is determined by patient size, it seems logical that medications and equipment in a pediatric resuscitation cart also be grouped by patient size. Standard pediatric resuscitation equipment carts with drawers organized by intervention are inherently susceptible to delayed or erroneous equipment identification. For example, standard carts require the knowledge of how to estimate patient size and select appropriately sized equipment. Also, opening multiple drawers during a resuscitation event can be physically difficult, because equipment in 1 drawer can obstruct the opening or closing of the drawers immediately above or below it. Finally, once a drawer is open, health care providers still need to sort through multiple sizes of equipment to retrieve the appropriate item. Organizing a pediatric resuscitation cart by patient size, on the other hand, alleviates the need to estimate appropriate sizes of equipment, requires the opening of only 1 drawer, and leads to less sorting through equipment because fewer sizes are in each drawer. These features of the color-coded system used in the pediatric resuscitation cart allow health care providers to find appropriate equipment quickly and easily. • Overall Dimensions: Overall: 46.05"H x 25.03"D x 34.05"W (includes bumper, handles, and casters • Drawer Configuration: 3" (8), 6" (1) An example of this product is the Broselow System for children 3kg. to 36kg. 6. CLIN #16, Cart, ENT Procedure, Mobile, Quantity 1: ENT procedures combines ear, nose and throat and related structures of the head and neck surgical procedures, from which many surgeries are relatively straight forward and can be performed on out-patient basis. ENT carts offer thousands of standard option combinations to satisfy every need and the willingness to design and produce special configurations to then satisfy almost every need. Almost every component is of "plug-n-play" architecture and can be replaced without the use of tools. • The unit provided should offer a high volume vacuum system. Features include: Industrial rated pressure and vacuum system, 21" (53.3 cm) deep, Stainless steel 3" x 6" (7.6 cm x 15.2 cm) canisters, Two application bottles (one atomizer and one powder blower) with plastic inserts, Dual rechargeable instrument wells, Two rechargeable handles, Control panel with easy to use single switch vacuum/pressure operation and auxiliary switch control, foot control, Total component modularity with plug and play components aid in easy component replacement/repair, Latex safe (latex-free), 15" (38.1 cm) back wall with stainless steel shelf, and Four hard rubber casters. • Unit shall operate on 120V and be UL-listed. 7. CLIN #17, Cart, Procedure with Drawers, Quantity 4: Tough, durable carts should be designed to meet the demands we have today and make change easy as our needs change. A modular cart system means universal sizes and easily interchangeable drawers and accessories. All interior components and accessories can be removed from the cart body, making every surface-inside and out-accessible for cleaning. Ergonomic design, rounded corners, and a surface material that is softer than metal add up to a user-friendly design. The carts' lighter weight and sturdy casters ease mobility and movement over door and elevator thresholds and when quick response is essential. Outer Dimensions: 31 x 48 x 20 inches (width height depth) Small Drawer: 24 1/2 x 4 1/2 x 15 1/2 inches Color: Blue 8. CLIN #18 Cart, Supply/Dressing, Quantity 1: Dressing cart is designed to provide unique flexibility and contemporary styling that withstands the rigors of a healthcare environment. Corrosion-proof, smooth, stain-resistant polymer surfaces with rounded corners make cleaning quick and thorough. Antimicrobial product protection, inhibiting the growth of mold, mildew and bacteria should be included in the product finish. Technical Capability: Warranty Terms Service Coverage 2. Logistics: Delivery Terms Socioeconomic Status (e.g., HUBZone Certified, SDVOSB, etc.) 3. Price: technical factors are significantly more important than price, price will be used as a form of tie breaker if should two or more responses be equal of points. General Conditions: ASSEMBLE, TEST, & INSERVICE Wherever applicable, quoted price is to include the assembly, testing and in-service training. Acceptance of the equipment will take place upon availability for first use as intended. Seller to provide name, address, and phone number of representative(s) responsible for local service calls. All electro-mechanical equipment will be subject to acceptance inspection by buyer's Biomedical Engineer. LEAD TIME & DELIVERY Normal lead-time and delivery method is to be indicated on each bid response. Actual purchase documents will indicate desired delivery period and owner's requirements for delivery. Delivery address may be to an off-site location managed by buyer's representative. ELECTRICAL All items requiring electricity must be UL certified and have UL labels affixed. Bid response must indicate if emergency/UPS power will be required. SERVICE & OPERATING MANUALS Successful bidder shall be required to provide 3 copies each of service manuals of a type enabling basic on-site servicing and calibration and 3 copies of an instruction manual of a type providing instruction on the operation of the unit. These will be listed on the buyer's purchase order as a no charge item unless indicated otherwise on your bid response. OPTIONS & ACCESSORIES Please provide separate detailed unit pricing on each option and/or accessory being offered. List price and discounted net price is required for each line item. F.O.B. DESTINATION Provide price(s) on an F.O.B. destination basis. In quoting price(s) other than F.O.B. delivery, include the dimensions and weight of each crated item. Shipping damages to item(s) shall be the responsibility of Vendor including the filing of damage claims. A designated representative shall arrange and process all returns and replacements and will assist in damage claims by providing the condition of incoming shipment and annotated freight bills to substantiate claims. WARRANTY State type and duration of warranty on each major item/system being offered. Detail any extended warranty package available to buyer. NOTE: Warranty period shall not commence until product is placed into clinical use at the facility. SERVICE Provide name, title, ZIP code, and telephone for local authorized service technician covering Kayenta, AZ. The performance shall conform in all respects with the requirements of the Contract and with Navajo Nation Law, Policies, and Procedures regarding taxes. The Government reserves the right to issue a single award or multiple awards to the contractor(s) whose quote represents the best value as defined by FAR 2.101. In determining best value, the following Evaluation Factors will be considered: Past Performance, Technical Capabilities, Delivery and Installation, and Warranty. The Evaluation Factors and significant sub factors when combined are significantly more important than cost or price. The socio-economic status of an offeror may also be considered, should one or more quotes represent the best value. To be considered for award, your offer shall include the following documents: 1.
