Responders are solely responsible for all expenses associated with responding to this RFI. SOURCES SOUGHT ANNOUNCEMENT: The Department of Veterans Affairs, Coatesville Veterans Affairs Medical Center 1400 Blackhorse Hill Road, Coatesville, PA 19320, is seeking to obtain sources for a de-install, relocate, and install of PT/KT rehab exercise equipment. If a small business, please indicate socio-economic status. To perform this service the moving company must have the essential proper training to perform the service of: breakdown of Life Fitness PT equipment, adequately movie all pieces of equipment and finally the reinstall of Life Fitness Equipment. Proof of training to perform this service on Life Fitness equipment is essential to ensure equipment is not damaged during the de-install, moving and re-install. This ensures the safety of veteran patients using the equipment. This is not a request for competitive quotes. It is a notice to obtain possible sources that have the capability for providing this service to be performed the week of April 25 thru April 29, 2016. STATEMENT OF WORK (SOW) 1. Contract Title. Relocate PT/KT Gym Equipment 2. Background. - Currently in need of nine (9) treatments tables, 4 treadmills, 4 bikes, 1 hydralift, 2 UBE, 1 bike UBE/combo, 1 leg press, 2 sets of parallel bars, and one (1) universal weight rack to be moved from current location in Building 58, Rooms B03 & 238 to new location in Building 69. Currently in need 3 Treadmills, 2 stationary bikes, 2 recumbent bikes, 2 elliptical machines, 17 universal machines, 3 flat benches, 2 chest press benches, one dumbbell set and one heavy weight rack with large weight plates to be moved from current location Building 38, Room B21 to new location in Building 69. 3. Scope. The selected Contractor shall dissemble, move, reassemble, and ensure proper operation of fifty-eight (58) pieces of gym equipment from current location ( Bldg. 58 and 38) to new location (Bldg. 69). 4. Specific Tasks. Contractor shall dissemble gym equipment in current locations, use loading dock at Bldg. 38 to load equipment onto to delivery vehicle and drive to building 69. Contractor shall: "Provide for proper disposal of packaging materials. oAt a minimum, the following waste categories shall be diverted from landfills: cardboard; paper and packaging; plastics (e.g., ABS, PVC); clean dimensional wood and palette wood. "Provide on-site instructions and supervision of separation, handling, salvaging, recycling, reuse, and return methods to be used by all parties during waste generating stages. "Provide the COR the names and locations of mixed debris reuse and recycling facilities or sites to be used. "Provide the COR the names and locations of trash disposal landfill facilities or sites. "Provide the COR documentation that these facilities or sites are approved to receive the materials to be disposed. "Provide the COR a monthly summary of debris diversion and disposal, quantifying all materials generated at the work site and disposed of or diverted from disposal through recycling. oSpecifically, the Contractor shall maintain records to document the quantity of waste generated; the quantity of waste diverted through sale, reuse or recycling; and the quantity of waste disposed by landfill or incineration. Prior to beginning work, Contractor shall check -in with Rehabilitation Supervisor or designee. Work shall be completed during the normal business week of Monday through Friday, 8:00 am to 4:30 pm, excluding Federal holidays. Contractor shall provide COR with a proposed schedule.. 5. Performance Monitoring. Contractor shall keep COR informed of installations completed each day. COR will routinely inspect completed installations for quality. COR will also randomly inspect work in progress. 6. Security Requirements. All contractor representatives who will perform this work shall acquire the appropriate VAMC identification badges. No other special security requirements are anticipated. 7. Government-Furnished Equipment (GFE)/Government-Furnished Information (GFI). None is anticipated under this contract. 8. Other Pertinent Information or Special Considerations. a. Identification of Possible Follow-on Work. No follow-on work is anticipated. b. Identification of Potential Conflicts of Interest (COI). No COI is anticipated. c. Identification of Non-Disclosure Requirements. Contractor will be working in an area where patient care takes place with potential for incidental exposure to patient information. Contractor shall not disclose any patient information. d. Packaging, Packing and Shipping Instructions. See above regarding storage of materials. No other special requirements. e. Inspection and Acceptance Criteria. Contractor shall keep COR informed of installations completed each day. COR will routinely inspect completed installations for quality. COR will also randomly inspect work in progress. Mounting installations are expected to be performed by the agreed upon method. 9. Risk Control. Contractor shall provide all applicable training and protective equipment to protect Contractor's employees from any hazards, including blood borne pathogens, which may be encountered in the performance of this contract. 10. Place of Performance. All work is to be performed at the Coatesville VA Medical Center, 1400 Blackhorse Hill Road, Coatesville, PA, 19320, within Building 69. 11. Period of Performance. Contractor shall complete installations within 20 days of notification to begin work.
