The work is located at the Willamette National Cemetery, 11800 Southeast Mt. Scott Blvd, Portland, OR. 97086 1 Job $_________ 1.GENERAL SCOPE OF WORK: The Contractor shall furnish all materials, equipment, labor and supervision to construct Concrete Sidewalks at East and West Columbaria. The work is located at the Willamette National Cemetery, 11800 Southeast Mt. Scott Blvd, Portland, OR. 97086 All work will be done in accordance with attached drawings labeled "New Sidewalk at Columbarium", dated 07/15/2015 and specifications. Completion Time: Thirty (60) calendar days from date of issuance of Notice to Proceed (NTP). Director: George Allen TEL: (503) 273-5256 George.Allen1@va.gov Contracting Officer's Representative (COR): TEL: (503) 273-5258 Larry Hughes Larry.Hughes4@va.com Work in this project includes but is not : 1.Remove existing decomposed granite paving and necessary landscaping. 2.Verify if existing aggregate base is usable. If not, provide new 4" aggregate base. 3.Install new 4" concrete sidewalk with color and light surface retarded finish. Contractor will supply the PM and Director with color samples prior to pouring. 4.Repair/Reinstall any damaged landscaping and other cemetery areas affected by construction. 5.Provide protection for the Columbaria walls during construction. Mask Columbaria when pouring concrete around walls. GENERAL REQUIREMENTS All work will be coordinated with the Contracting Officer's Representative (COR) and/or the Cemetery Director. Work will be conducted in such a manner to minimize the impact on the Cemetery operations. All work will be accomplished after the hours of 4:30 and restore water service before 8:00 am, Monday through Friday, except Federal Holidays. Only the Contracting Officer (CO) has the authority to make changes to the contract that affects price and/or time. General Notes: 1.Prior to submission of any bid, the Contractor shall perform a thorough field survey of the existing site conditions and features. Verify at project site the exact size, clearances of all existing services being relocated, extended, connected to or removed. Any site conditions which may cause significant deviation from the site plan shall be brought to the attention of the Contracting Officer or his/her Representative (COR) for clarification prior to submission of the bid. Verify dimensions of all owners furnished and installed equipment to insure proper coordination with construction. Contractor shall bear all costs for relocation of existing utilities from failure to advise of conflict in writing prior to submission of any bid, and/or from failure to properly coordinate installations of system. 2.Submittals: Prior to installation, the contractor shall submit for approval the manufacturer's literature and specifications for ALL materials being installed. 3.Construction Coordination: a.Coordination of Construction with Cemetery Director: The burial activities at a National Cemetery shall take precedence over construction activities. The Contractor must cooperate and coordinate with the Cemetery Director, through the RE/COR, in arranging construction schedule to cause the least possible interference with Cemetery activities in actual burial areas. Construction noise during the committal services shall not disturb the service. Trucks and workmen shall not pass through the service area during this period. b.The Contractor is required to discontinue his work sufficiently in advance of Easter Sunday, Mother's Day, Father's Day, Memorial Day, Veteran's Day and/or Federal holidays, to permit him to clean up all areas of operation adjacent to existing burial plots before these dates. c.Cleaning up shall include the removal of all equipment, tools, materials and debris and leaving the areas in a clean, neat condition on a daily basis. Contractor is responsible for disposing of debris off site. 4.Contractor shall coordinate with the Cemetery Director to locate the construction fencing, temporary portable potties, material storage containers, etc. if required for the performance of this contract. Public access to the National Cemetery shall not be impaired. 5. The Contractor shall assume sole responsibility for safety of all persons on or about the construction site, in accordance with applicable laws and codes. Guard all hazards in accordance with the safety provisions of the latest manual of accident prevention published by the Associated General Contractors of America and OSHA. 6. The Contractor shall guarantee that all work done under this contract will be free from faulty materials or workmanship and hereby agrees to repair or replace without cost to the VA and to his/her satisfaction all defects or imperfection appearing in said work within a period of one year after the date of final acceptance by the VA of all work done under this contract. 7. Matters related to safety, and any actions of the Contractor, shall meet all safety requirements of Miramar National Cemetery Safety Officer, Department of Veterans Affairs, OSHA, and the State. It is incumbent upon the Contractor to be familiar with these requirements. "Safety" shall also include the Contractor having a safety representative who maintains regular and routine contact with the Safety Officer at MNC. Contractor shall provide training of the contractor's and subcontractor's workers as necessary to meet these requirements. 8. Repair and replace any existing structure to existing condition when altered or damaged by the construction. 9.Completion Time: Seven (30) calendar days from date of issuance of Notice to Proceed (NTP). 2.WARRANTY: The Contractor shall guarantee that all work done under this contract will be free from faulty materials or workmanship. The Contractor hereby agrees to repair or replace without cost to the VA and to his/her satisfaction all defects or imperfection appearing in said work within a period of one (1) year after the date of final acceptance by the VA of all work done under this contract. Contractor must respond within 48-hours to perform any warranty work. Time to have parts delivered to site that are not locally available is not included in this 48-hour response time.
