- Naval Station (NAVSTA) Mayport is a Large Quantity Generator of hazardous waste with a RCRA Part B Permit that allows storage of hazardous waste for up to one calendar year. Contract is to obtain services to provide hazardous waste pickup, treatment and disposal services for the waste generated at the Naval Branch Health Clinic (NBHC). NBHC routinely generates hazardous waste that has been in contact with human body fluids. In accordance with Florida Statue Chapter 64E-16 waste must be managed as a RCRA hazardous waste. NAVSTA Mayport will provide laboratory analysis, Product Data Sheets (PDSs) and/or Safety Data Sheets (SDSs) and completed waste profiles characterizing this waste. Contractor shall complete and provide hazardous manifests and Land Disposal Restriction (LDR) documents for waste for review and changes prior to scheduling shipment. Contractor shall provide transportation from NAVSTA Mayport, Hazardous Waste Storage Facility (HWSF) to the permitted Treatment, Storage and Disposal Facility (TSDF) and comply with all local, state and federal regulations. Contractor shall provide copies of current facility operating permits, licenses and insurance showing they are authorized to provide services requested under this contract. INITIAL WASTE SHIPMENT must leave NAVSTA Mayport NLT 6 June 2016: -12, 5-gallon containers of SPENT CONTACT AND NON-CONTACT MERCURY AMALGAM (772 LBS). All containers of Spent Amalgam have been immersed in bleach. -1, 2.5-gallon container of the EXPIRED FLULAVAL VACCINATION (14 LBS). As per the manufacturer ™s information provided, the flu virus for shipment is inactive. Current waste profile and analysis are provided for the SPENT CONTACT AND NON-CONTACT MERCURY AMALGAM and manufacturer information for the EXPIRED FLULAVAL VACCINATION as Attachment #1. SUBSEQUENT WASTE SHIPMENT (1-3/YEAR): WASTE DESCRIPTIONS for future waste to be managed under contract. The specific quantities of waste generated over the past five years has been provided as Attachment #2. -MERCURY AMALGAM WASTE (D002, D009, D011): Price: /lb -MISC.
