II. BID REQUIREMENTS 1. Each bidder shall submit a bid based upon Part IV - Extent of Work that includes reference to the project listed in this scope of work. Any variations from the base scope of work are to be noted as distinctly separate from the base bid. 2. Bids shall be submitted for the work as a basis for the contract. The prospective bidder shall submit a bid per hour work for all hours required. Also must submit a bid for all option periods if options are included in the Scope of Work. 3. The contractor must include with his/her proposal, certification that he/she will conform to the latest Wage Rate (N/A for this subcontract) 4. A minimum 90-day bid guarantee is required. 5. This project is tax-exempt. 6. Prior to starting any work, the contractor shall show proof of required insurance, in amounts to cover risk as stated below: • Comprehensive Automobile Liability Insurance ( $1,000,000 combined single limit) • Workers Compensation / Liability Insurance ($1,000,000 per occurrence) • Fire Legal Liability of at least $500,000 per occurrence. • Fire Legal Liability of at least $500,000 per occurrence. 7. Final Bid Proposal shall be typed and submitted on official Company letterhead and must include the following: • Scope of Work ID#, Project Description, Project Specific Requirements, Hourly Cost • References • Copies of all Insurance Certificates • Copy of all licenses and certifications relevant to this proposal • Completed Vendor Questionnaire (with DUNS# if available) and W9 • Small Business Certification, if any 8. Net 30 payment terms III. EVALUATION CRITERIA Evaluation Criteria Point Value Price: Responsible Contractor who can perform effective services at the lowest reasonable price. 60 Quality: Demonstrated experience and knowledge capabilities; Technical Excellence; Management Capabilities; Personnel qualification; Quality of the Tender packages; Technical excellence including where necessary, technical drawings, blue prints. 20 Past Performance: Past services to center or list of references for verification purposes. 10 Schedule: Explanation on how services will be carried out and the delivery timeframe. 10 TOTAL POINTS 100 Award will be made to the offeror who provides Best Value to the Center when all Evaluation Criteria are considered. Lowest Price does not guarantee award of Sub Contract. Adams and Associates retains the right to reject any and all bids. IV. EXTENT OF WORK SCOPE OF SERVICES The Center TEAP Specialist agrees to provide students of the Woodstock Job Corp Center with the services stated herein. Such services will be provided as authorized by the Center representative. Services to be provided will be in the amount of 40 hours per week. The Center TEAP Specialist agrees to adhere to all state required standards of practice and Center Mental Health oversight when applicable. The Center and the TEAP Specialist hereby agree that the services to be provided will include, but will not be limited to those health and wellness services specified in the Job Corps Federal Regulations (The Job Corps Policy and Requirements Handbook, Chapter 6, Section 6.11, R1; and applicable Techinical Assistance Guides.). These services provided in conjunction with the Center, include but are not limited to: Vendor must include the Scope of Work Identification number on submitted bid. The period of performance is from April 18, 2016 to July 31, 2016 with an option period of August 1, 2016 to January 31, 2017. Substance use prevention and education, to include: • Minimum of a 1-hour interactive presentation on substance use prevention for all new students during the Career Preparation Period. • Presentation (s) on managing substance misuse, abuse and dependency conditions in the workplace to students during the Career Development and Transition Periods. • At least three annual center-wide substance use prevention and education activities. • Clinical consultation with the Center Director, management staff, center mental health consultant, and health and wellness manager regarding substance use related prevention and education efforts for students and staff. • Coordination with other departments/programs on center to develop integrated prevention and education services. B. Assessment for identification of students at risk for substance use problems to include: • Review of Social Intake Form (SIF) or intake assessment of all students performed by counseling staff within 1 week of arrival. • Formalized assessment measures (e.g., SASSI3 or SASSIA2) and clinical judgment to determine students' level of risk for substance use. • Collaboration with the center mental health consultant to determine when a MSWR or medical separation is appropriate and should be recommended for a student with substance use conditions. C. Intervention services for students identified at an elevated risk for substance use, to include: • Individual and group intervention services with a focus on behaviors that represent employability barriers. • Collaboration with the center mental health consultant for students with co-occurring conditions of mental health and substance use. • Referral to off-center substance abuse professionals or agencies for ongoing treatment and/or specialized services. D. Drug and alcohol testing, to include: • Drug and alcohol testing procedures • Policies related to positive drug or alcohol tests • Notification of drug or alcohol test results E. Maintaining the accuracy and confidentiality of all TEAP documentation in the student health record. F. Enforcing appropriate student workplace behavior at all times while on center. G. Collecting data and preparing reports as required by the Center Director and/or the Department of Labor. H. Cooperating with corporate and Regional Office Center assessments. The TEAP specialist agrees to adhere to the policies, procedures, and guidelines established by Job Corps. They also agree to work the 40 hour weekly schedule listed below: Week 1: Monday 8:00am - 4:30pm Tuesday 10:00am - 6:30pm Wednesday 8:00am - 4:30pm Thursday 10:00am - 6:30pm Friday 8:00am - 4:30pm Week 2: Monday 8:00am - 4:30pm Tuesday 10:00am - 6:30pm Wednesday 8:00am - 4:30pm Thursday 10:00am - 6:30pm Friday 8:00am - 12:00pm Saturday 10:00am - 2:00pm It is understood and agreed that the services provided by the TEAP specialist are subject to monitoring and review by the Department of Labor.
