- This is a SOURCES SOUGHT ANNOUNCEMENT ONLY. Responses to this Sources Sought must be in writing. The purpose of this Sources Sought Announcement is for market research to make appropriate acquisition decisions and to gain knowledge of potential qualified Large and Small Businesses interested and capable of performing the work. Responses to this notice shall include the following: (a) company name b) address (c) point of contact (d) phone, fax, and email (e) DUNS number (f) Type of small business, if applicable, (e.g. Services Disabled Veteran Owned small Business, Veteran-owned small business, 8(a), HUB-Zone, Women Owned Small Business, Small disadvantaged business, or Small Business) and (g) a capability statement that addresses the organization's qualifications and ability to perform as a contractor for the work described below. All Offerors who provide goods or services to the United States Federal Government must be registered in the System for Award Management (SAM) database found at https://www.sam.gov. Registration must include Representations and Certifications. Additionally, all Service Disabled Veteran Owned Businesses or Veteran Owned Businesses who respond to a solicitation on this project must be registered and verified with the Vet-Biz Registry located at http://vip.vetbiz.gov. The U.S. Department of Veterans Affairs (VA) Greater Los Angeles Healthcare System (VAGLAHS) is seeking a contractor to assist with the provision of housing placement and case management services through the HUD-VASH program. The program is a partnership between the U.S. Department of Housing and Urban Development (HUD) and VA Supportive Housing (VASH) to place and maintain homeless Veterans in permanent, community based housing using HUD Section 8 Housing Choice Vouchers that are paired with ongoing VA case management services. Many of these Veterans have co-morbid conditions that include serious mental illnesses, substance abuse disorders, and other serious medical conditions. VA intends to engage a Contractor to provide housing placement services along with ongoing case management services to HUD-VASH eligible Veterans and their families. The target population for the HUD-VASH Housing Placement and Case Management contract shall include chronically homeless Veterans who are the most vulnerable and may have severe mental health and/or substance abuse problems. Veterans may not be chronic and may fall into one of the priority groups such as women, women Veterans with children, OEF/OIF/OND Veterans, and/or Veterans who have physical health problems and/or Substance Use Disorders (SUD), with frequent emergency room visits, multiple treatment failures, and limited access to other social supports. Through this contract initiative, the awarded Contractor(s) will place individuals into housing while simultaneously providing case management and supportive services; and assist the Veteran in maintaining housing until the Veteran is eligible for discharge. The HUD-VASH program shall be reserved for homeless Veterans with serious mental health problems who have few resources and require long-term case management to either obtain and/or maintaining permanent supportive housing. The Contractor(s) shall provide all labor, supervision, supplies, and materials necessary to carry out the task specified in this solicitation. Performance will occur at the Contractor's location and the VA Greater Los Angeles Healthcare System (VAGLAHS) campus unless otherwise required by work requirements listed in the Performance Work Statement. The Veteran Integrated System Network (VISN) 22 geographic areas include the following VAGLAHS catchment areas. The counties/cities indicated below are based on particular geographic allocation of vouchers. The VAGLAHS consist of: San Luis Obispo, Kern County, Santa Barbara County, City of Oxnard, City of Pasadena, City of Los Angeles, and Los Angeles County. Prospective Contractor(s) shall be able to provide services to a minimum of 300 eligible beneficiaries per year for whom such care is specifically authorized by the referring VA.
