- This Sources Sought Notice IS 100% BUY INDIAN SET ASIDE per 25 U.S.C. 47, and is for the Chinle Comprehensive Health Care Facility, Navajo Area Indian Health Service, issued in accordance with Federal Acquisition Regulation (FAR) Subpart 5.101 & 5.207 to identify potential sources of Buy Indian Owned Contractors that can provide high quality hospital grade furniture, which include 21 office chairs, 19 physician seating stools, and 19 patient 3-seater waiting room chairs. Chinle Comprehensive Health Care Facility (CCHCF) is based in Chinle, Arizona (Northeast Arizona near Canyon De Chelly National Monument) on the Navajo Indian Reservation. CCHCF provides medical care for approximately 35,000 Navajo. Serving a rural area, many of our patients travel 100 miles round trip to receive care and do not have electricity or running water in their homes. And, most of our elderly patients speak only Navajo, and live according to traditional Navajo cultural practices. The Chinle Comprehensive Health Care Facility and Ambulatory Care Center is a 60 bed inpatient hospital and outpatient facility. Services offered are: Adult inpatient and Pediatric Inpatient Care, Outpatient Primary Care, Adult Intensive Care, Emergency Medicine, General Surgery, Podiatry, OBGYN, Labor & Delivery, Women's Health, Midwifery, Mental Health, Pharmacy, Optometry, Dental, PT, OT, Speech Pathology, Audiology, Lab, Public Health and School Health. THE ARIZONA DEPARMENT OF HEALTH SERVICES DESIGNATES CCHCF AS A LEVEL IV TRAUMA CENTER, SEPTEMBER 9, 2013 TO SEPTEMBER 9, 2016. General requirements: 1. Provide evidence of Past Performance/Experience of comparable scope and complexity in providing services to Federal Government or other large Government entities within the last three (3) years of operation. 2. Provide your Business capability statement. 3. Provide your DUNS# and Business Size. 4. Provide certificate issued by the Navajo Nation Economic Development, or your local Native American/American Indian tribal office to confirm that your company is Buy-Indian Owned. Submit your documents to Contracting Officer for this action: Tanya Begay, Supervisory Contract Specialist, 928-74-7635, tanya.begay2@ihs.gov. Your requests shall cite RFQ # and RFQ Title, and is due the by response date. There will be no discussions or exchanges regarding this notice. The Government does not intend to make an award on the basis of the Sources Sought Notice or otherwise pay for the information provided herein. This requirement's anticipated award date is before 08/01/2016.
