The purpose of this Sources Sought Announcement is for market research to make appropriate acquisition decisions and to gain knowledge of potential qualified businesses. This announcement shall not be construed as a commitment by the Government to issue a solicitation or ultimately award a contract, nor does it restrict the Government to a particular acquisition approach. The Government will in no way be bound to this information if any solicitation is issued. STATEMENT OF WORK A.GENERAL INFORMATION 1.Title of Project 526-16-2-644-0058 2.Scope of Work: The contractor shall provide all resources necessary to accomplish the deliverables described in this statement of work (SOW), except as may otherwise be specified for the above project 3.Background: Wheelchair Service and Repair Contract 4.Performance Period: The contractor shall complete the work required under this SOW in 15 calendar days or less from date of award. 5.Type of Contract: Firm-Fixed-Price B.DELIVERABLES Wheelchair and Gurney Service: Vendor will provide service at our location: 130 WEST KINGSBRIDGE ROAD BRONX NY 10468 MONDAY THROUGH FRIDAY 10:00AM - 3:30 PM Excluding Federal Holiday's. Vendor will provide all necessary labor, equipment Material Supplies and tools for onsite service and repair to all wheelchairs, manual and electric, gurneys Vendor will provide a certified wheelchair repair technician whom shall be assigned to the Spinal Cord Injury Units and the nursing Home Care Unit and will work on those wards Mondays, Wednesdays and Fridays from 10:00 am to 3:30 pm Vendor will assist with Equipment Final Fittings on the SCI ward two days a week Tuesday and Thursday from 10:00 am to 3:30pm PREVENTIVE MAINTENANCE SERVICE FOR HOSPITAL EQUIPMENT AS SPECIFIED BELOW: The preventive maintenance inspection procedures (two types, one level) represent an orderly, controlled process to be used in inspecting mechanical and electrical wheelchairs. They are to be used in conjunction with this document and the manufacturer's manuals in performing preventive maintenance Preventive Maintenance and Inspection Procedure 1. Clean the external surfaces of the wheelchair. 2. Perform internal and external visual inspection. 3. Lubricate the wheel bearings. 4. Replace the components identified as defective or worn. (Upon approval) 5. Place the wheelchair in an operational configuration. 6. Test the function of the wheelchairs brakes. 7. Test the function of the wheelchair's frame locks. 8. Test the articulation and locks on the wheelchair's leg and foot supports 9. Test the function of the arm rests. 10. Check the frame alignment Upon Approval of request equipment Parts shall be replaced only upon authorization from the designated designee personnel. Vendor will offer the following discount for parts ordered. 15% discount off list price on Motorized Wheelchair Parts. 20% discount off list price on Manual Wheelchair Parts. Preventive Maintenance Service Proposal: March 31, 2016 through March 30 2017 Please include 4 option year pricing Place of Performance: The Veterans Health Administration of NY & NJ (VISN 3): Bronx, NY 10468 All Offerors who provide goods or services to the United States Federal Government must be registered in the System Award Management (SAM) located on the web at www.sam.gov. It is desirable that any Offeror to have completed their business Online Representations and Certifications Application (ORCA). Additionally, all Service Disabled Veteran Owned Businesses or Veteran Owned Businesses who respond to this announcement must be registered with the Department of Veterans Affairs Center for Veterans Enterprise VetBiz Registry located at https://www.vip.vetbiz.gov/. All interested Offerors should submit information by e-mail (preferred) to John.Redmond2@va.gov and include the RFQ (RFI) number, title of the project (Automatic Swing Doors Operator), and company name in the email subject heading.
