Description
This Sources Sought/Synopsis is for the Navajo Area Indian Health Service, Chinle Comprehensive Health Care Facility issued in accordance with Federal Acquisition Regulation (FAR) Subpart 5.101 & 5.207 for Nonpersonal Service Contract for Emergency Medicine Physician services. Market research indicates there is no reasonable expectation of fulfilling this need with small business concerns alone. The purpose of this notice is to identify potential sources for providing Emergency Medicine Physician services in support of the Emergency Room Department's mission. THIS NOTICE IS OPEN ONLY TO THOSE OFFERORS WHO DO NOT HAVE A CURRENT CONTRACT WITH CCHCF FOR THIS TYPE OF SERVICE. If interested, then you may request the solicitation via email from the Contracting Officer for this action: Tanya Begay, Supervisory Contract Specialist, 928-74-7635, tanya.begay2@ihs.gov. Your request for solicitation shall cite RFQ # and RFQ Title, and is due the response date. Anticipated Award Date: Notice is Open Continuous through 09/01/2016. Period of Performance: Date of Award through 09/30/2016, with the option to extend the end date to 10/31/2016. The assignment is through the stated period of performance or until the position is filled with a permanent hire. In such instances, the provider will be released from contract assignment. Government residential quarters are currently not available, but Chinle, AZ has (3) hotels in the immediate area. The Chinle Comprehensive Health Care Facility (CCHCF) is based in Chinle, Arizona (Northeast Arizona near Canyon De Chelly National Monument). The CCHCF is a 60 bed hospital which serves as the health care hub for the region. The medical staff includes Family Physicians, Internists, Pediatricians, General Surgeons, OB/GYN's, Anesthesiologists, and a Psychiatrist. In addition to routine outpatient and inpatient primary care, services available to our patients include: Adult Intensive Care, General Surgery (including laparoscopic surgery), routine and operative Obstetrics, and 24-Hour Emergency Room Services. Health care services are provided to approximately 37,000 active users covering an area 3600 square miles CCHCF is the only medical services in the immediate area. THE ARIZONA DEPARMENT OF HEALTH SERVICES DESIGNATES CCHCF AS A LEVEL IV TRAUMA CENTER, SEPTEMBER 9, 2013 TO SEPTEMBER 9, 2016. As an Emergency Medicine Physician you can expect to perform duties as an emergency department physician. The Emergency Department and the Urgent Care department care for approximately 30,000 patients per year. The Emergency Department consists of nine (9) beds and emergency care services are provided on a 24-hour basis for adults, adolescent children and infants. The Urgent Care consists of 6 beds and is staffed by mid-level providers (physician's assistants or nurse practitioners) who see patients that present for acute care and who have been triaged as lower acuity. The Urgent Care operates form 10am -10 pm. Strong Native American Navajo cultural traditions exist within the community, offering an opportunity to learn the Navajo language, or to learn about traditional Navajo medicine. The service unit is located on the Colorado Plateau with excellent opportunities for photography, hiking, running, road biking, cross country skiing, and mountain biking. Canyon de Chelly National Monument is within 5 miles of the hospital and is a wonderful place for exploring and sightseeing, running and mountain biking. General requirements: 1. The Contract providers' shall have a total of three (3) years equivalent residency training and experience in Emergency Medicine or Family Practice, and must be board certified in the respective disciplines. 2. The contractor's provider shall maintain appropriate licensure (a valid, current, full and unrestricted license to practice Emergency Medicine in any state within the United States, District of Columbia or the Common-Wealth of Puerto Rico) and certification throughout the term of this contract. The contractor shall provide evidence of Past Performance and Experience of comparable scope and complexity in providing services within the past three (3) years. 3. The Contractor shall be able to provide evidence of Past Performance and Experience of comparable scope and complexity in providing services within the past three (3) years. 4. The Contract providers' shall have available a minimum three (3) references, including name of organization Point of Contact, Telephone Number and Email Address. The Government does not intend to make an award on the basis of the Sources Sought Notice or otherwise pay for the information provided herein.
