Combo- Detailed Expense fillable sheet Statement of Work This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation W912NS-16-Q-3011 is being issued as a Request for Quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-86. This procurement is a 100% Small Business set-aside IAW FAR 19.502-2 (a). The NAICS code is 721110 and the small business size standard is $32,500,000.00. The following commercial items are requested in this solicitation; CLIN 0001 - Hotel Venue Space/Lodging/Meals Strong Bonds Event 18 - 20 March 2016 •1. Event Schedule Friday, 18MAR16, 6:00p-11:30p Friday, 18MAR16, Staff set up from 1:00p-4:00p Friday, 18MAR16, Check In/Registration from 4:00p-6:00p Friday, 18MAR16, Dinner meal conducted from 6:00p-7:00p Friday, 18MAR16, Event conducted from 7:00p-11:00p Saturday, 19MAR16, 7:00a-5:00p Saturday, 19MAR16, Staff set up from 7:00a-8:00a Saturday, 19MAR16, Breakfast meal conducted from 8:00a-9:00a Saturday, 19MAR16, Event conducted from 9:00a-11:30a & 12:30p-5:30p Saturday, 19MAR16, Lunch meal conducted from 11:30a-12:30p Sunday, 20MAR16, 7:30a-1:30p Sunday, 20MAR16, Staff set up from 7:00a-8:00a Sunday, 20MAR16, Breakfast meal conducted from 8:00a-9:00a Sunday, 20MAR16, Event conducted from 9:00a-1:00p •2. Lodging Room Requirement Requesting 36 contracted king rooms -Lodging rooms on lower levels -Lodging rooms away from elevators, ice machine areas, etc. Away from any and all distractions, excessive noises, loud groups, etc. -Prefer all lodging rooms in main building where event space is located. -Lodging offered at the government rate. -Prefer Wifi in lodging rooms at no additional charge. -Prefer Parking for lodging rooms at no additional charge. *Strong Bonds will provide the names for the reservations. *Check in will be 18MAR16 *Check out will be 20MAR16 *Will give final numbers by 7MAR16. Requesting 1 blocked king room -Lodging offered at the government rate. -Prefer Wifi in lodging room at no additional charge. -Prefer Parking for lodging room at no additional charge. *Strong Bonds will provide the names for the reservation. *Check in will be 18MAR16 *Check out will be 19MAR16 •3. Meeting Room Requirement Two meeting rooms - Briefing rooms -Prefer all meeting space away from other events/busy/noisy areas. Rooms need to be roomy and not a tight fit to promote a retreat feel and less of a training feel. -Seating for 40 people, in sets of two, classroom style, with table cloths, in each briefing room. -1 6' & 2 6' tables with table cloths for registration & in back of room, 2 chairs per table, in each briefing room -Pads and Pens provided by venue in each briefing room -Water Station & Trash Can in each briefing room -Easel in each briefing room -Prefer free wi-fi in meeting space for random use in each briefing room -Prefer both Meeting Space on 24 hour hold from 18MAR16-20MAR16 Two meeting rooms - Meals room -Prefer all meeting space away from other events/busy/noisy areas. -Seating for 40 people, rounds, with table cloths in each room -Water Station & Trash Can in each room. A/V Requirement Friday-Sunday, 18-20MAR16 - For each Briefing room -AV Cart with power and power strip -Projector -Projector Screen -DVD Player -External speakers/tie in to house sound with necessary cables -Strong Bonds Staff will bring in their own laptop. Hotel must not charge additional fees for bringing in own equipment. •5. Food Service Requirement Friday, 18MAR16 - Meal Needs; Serve 66 people, each meal; buffet style -Dinner -Please provide a per person cost, per meal (Cannot exceed the per meal government meal rate) *Will give final numbers by 7MAR16. Detailed Expense Sheet (see attachment) CLIN 0002- Contractor Manpower Reporting- The contractor shall report ALL contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract for the 139 th Airlift Wing via a secure data collection site. The contractor is required to completely fill in all required data fields using the following web address: http://www.ecmra.mil/. Reporting inputs will be for the labor executed during the period of performance during each Government fiscal year (FY), which runs October 1 through September 30. While inputs may be reported any time during the FY, all data shall be reported no later than October 31 of each calendar year.
