- STATEMENT OF WORK FOR CARDIOLOGIST SERVICES FOR THE INDIAN HEALTH SERVICE PINE RIDGE, SOUTH DAKOTA BACKGROUND: This health care service contract is located in the Indian Health Service (I.H.S) Facility in Pine Ridge, South Dakota. This service unit provides medical services to American Indians in the Oglala Sioux Tribe. The cardiology clinic is staffed by one part-time contract cardiologist whose mission is to provide full-scope quality cardiology services. Patients requiring cardiology services are always seen first by a physician in outpatient clinic who then submits a referral to the cardiology clinic. PURPOSE: Purpose of the work is to participate in the development and modification of treatment programs to prevent and control cardiovascular disease, utilizing the interdisciplinary approach. To successfully train I.H.S staff in assessing the risks of heart disease in community members. To serve as a consultant cardiologist providing direct patient care to individuals in institutional settings directed toward achieving cardiovascular health. Tasks: 1. Conducts cardiovascular assessments of clients and assists with developing treatment plans to delay or prevent the progression of the disease. 2. Completes basic cardiovascular diagnostic procedures such as stress testing, Holter monitoring and echocardiograms. 3. Review suitable educational and counseling materials related to lifestyle for patients and for use by professional and paraprofessional staff providing services to clients and the community. 4. Records information in the Electronic Health Record (E.H.R) of each patient seen. CONTRACTORS PERFORMANCE CONDITIONS & RESPONSIBILITIES : The contractor shall comply with the following quality of care protocol: 1. It shall be understood that eligible patients under treatment will be rendered services that provide the best medical care within the Contractor's professional training and experience. All services rendered shall be in accordance with current standard medical practices to assure quality of care. 2. The Contractor shall be familiar and adhere to the policy and procedures of the Bylaws of Medical/Dental Staff of the I.H.S Health center. The credentials of the Contractor shall comply with the Medical/Dental Staff of the I.H.S Health center for all services provided in Public Health Service Clinics. The Contractor shall agree to abide by the HIPAA and AAAHC standards. 3. The quality of work performed by the Contractor will be subject to review by chart audits and site visits by the Project Officer or designee. The quality of work performed by the Contractor will also be subject to review by the Quality Assurance program by the contracting company. 4. A list of current formulary for use will be available to the Contractor and he/she shall prescribe medications found on it as much as possible although exceptions are allowed with proper justification. 5. The cardiologist will provide documentations of current Board Certification, Licensure, Diplomas/Specialty Certificates, DEA licensure, BLS, Recent Health Exam, 3 current professional references, liability insurance/ liability claims/ adverse actions, continuing medical education and will also support the ongoing Continuous Improvement Program and the Service Unit. The Contractor's application and clinical privileges will be reviewed in accordance with the Pine Ridge I.H.S credentialing process. 6. The Cardiologist will evaluate and treat conditions as referred to him/her as specified in his/her clinical privileges: To include direct consultation to the Pine Ridge I.H.S Medical Staff: Non-invasive procedures; stress testing, carotid Doppler exam, echocardiogram, EKG, Holter monitoring and appropriate referral out for quality management of care that cannot be provided at the Pine Ridge I.H.S Health Care Facility. 7. The contractor will provide the echocardiogram services required to complete stress testing and echocardiogram procedures. 8. The contractor will complete reading of diagnostic tests including 2 D echo, 24-48 hrs holter, exercise stress test and stress echo. 9. Individuals providing services under contract, which involve regular contact with, or control over, Indian children, are subject to a character investigation in accordance with the "Indian Child Protection and Family Violence Prevention Act" P.L. 101-630. The Indian Health Service Personnel Office will conduct these investigations following award of a contract unless in the judgment of the Contracting Officer, in consultation with the Personnel Officer, investigation has been completed and the Contracting Officer notified, such individual shall not have unsupervised contact with Indian children. 10. The Contracting Officer has the authority to temporarily waive the character investigation of a contractor. The "Agreement to a Temporary Waiver of Character Investigation" (See Attachment B) authorizes an agency to hire a contractor prior to the completion of a background check provided that at all times the contractor is within the sight and under the supervision of a government employee who has successfully completed the National Agency Check with written Inquiries (NACI). 11. The Contractor understands that the I.H.S will not pay or withhold on behalf of the Contractor any sums for federal or state income tax, any other federal or state tax, unemployment insurance, social security, worker's compensation, or any other withholding pursuant to any law or requirement. All such payments and withholdings are the sole responsibility of the Contractor. 12. "In accordance with 45 Code of Federal Regulations (CFR) 164.530 (b) (1) and Indian Health Service regulations, the Contractor shall complete an appropriate level of training in the Health Insurance Portability and Accountability Act of 1996 (HIPAA), within 30 days from the beginning of the period of performance. Training provided to the Contractor will be documented and maintained in writing or electronically for six years. " 13. The Pine Ridge I.H.S Health Care Facility will bill all third party resources (Medicare, Medicaid, Private Insurance, etc.) carried by the patient for all services performed by the Contractor for the Pine Ridge I.H.S Health Care Facility. 14. Contractor is responsible for reporting all taxes from wages earned under this contract to the appropriate State and Federal Agencies, provided to the awardee is an individual. 15. Contractor is responsible for his/her own reliable transportation in the performance of the contract. 16. Contractor is responsible for Fringe Benefits and other benefits such as health, retirement, etc. for his/her employees, if other than self. 17. Contractor is responsible for maintaining satisfactory standards of competitive, conduct, appearance and integrity. The quality of work performance of the contractor shall be subject to review. 18. Contractor is responsible for notifying the designated supervisor/director if he/she will be late, ill or unable to work for other reasons. 19. Contractor earns no leave and is compensated only for hours actually worked at the agreed compensatory rate. 20. Contractor is responsible for signing-in and signing-out of immediate work area, utilizing in-house system of accountability. 21. Contractor is responsible for being in compliance with Pine Ridge I H S HIM / Medical Records policies including E H R. 22. The contractor shall be provided with an orientation to the Indian Health Service. The initial orientation and continuing orientation shall be provided by service unit personnel. 23. The Pine Ridge Indian Health Service will not provide transcription services. QUALIFICATIONS: It is a condition of this contract that all required certifications in this Statement of Work remain current during the life of this contract. The Contractor physician(s) shall speak, understand, read, and write English fluently and communicate with sufficient clarity to be understood by Aberdeen Area Indian Health Service personnel and patients. It is required that the provider have the Influenza vaccination by October 31 of each year or have a valid medical exemption for vaccination signed by a licensed independent practitioner. Individuals with a valid medical exemption for vaccination will be required to wear a mask provided by IHS during the influenza season (November 1 through March 31) and during other periods of increased influenza activity as defined by the Centers for Disease Control and Prevention and/or State public health authorities. Pine Ridge Indian Health Service hospital policy requires all staff with direct patient contact, be able to provide proof of PPD and Tdap (Tetanus, Diphtheria, acellular Pertusis). This includes all permanent employees, volunteers, and contractors (for all contractors in this category, acceptable proof can be in the form of an immunization record or written documentation from a physician). This proof must be provided as part of the application submission process prior to being accepted for a position. Contractor personnel currently serving in a contract position can receive these immunizations from the facility. PERIOD OF PERFORMANCE: This contract shall begin the date of award thru one year. The contractor shall provide two full days per month with possibility of expanding to 3 scheduled days. GOVERNMENT PROVIDED EQUIPMENT: The Service Unit shall provide office space including a desk, chair, computers and office supplies. The contractor will be authorized to use all areas of the medical treatment facility available to civil service and commissioned corps personnel in similar positions. The same restrictions apply to the contractor as any other Indian Health Service personnel to use these areas for official business activities only. The contractor will be authorized to use all administrative and professional support available to Indian Health Service personnel. This will include, but not be limited to copy machines, telefax machines and telephone service. The service unit shall provide all medical and nonmedical equipment and supplies used by the Pine Ridge I.H.S for the care and management of patients. The equipment necessary for clinic evaluation at this facility will be provided (exercise stress treadmill, Stress test console, EKG etc.). The contractor will provide the ECHO cardiogram machine, Holter monitoring assessments. CONTRACTOR PROVIDED EQUIPMENT: The government does not anticipate the need for any contractor provider equipment. SECURITY CLEARANCE: Performance of this contract will require routine access by employees of the Contractor or Subcontractor to facilities or systems controlled by the Indian Health Service (I.H.S). Before starting work requiring routine access to I.H.S facilities or systems, each person must complete a FBI National Crime History Check (Fingerprint Check) adjudicated by an I.H.S employee using the Office of Personnel Management Processing System. Contractors shall allow five business days for I.H.S processing of fingerprints taken electronically at an I.H.S site and thirty business days for non-electronic processing of fingerprints using FBI Form FD-258. A list of I.H.S site with electronic fingerprint capacity is available from the Contracting Officer. In addition, no Contractor or Subcontractor shall be permitted to perform work under this contract if listed on the HHS Office of Inspector General List of Excluded Individuals/ Entities (LEIE), http://exclusions.oig.hhs.gov. As soon as practicable, prior to the start of work, the performance of the work the Contractor shall provide to the Contracting officer the names of all individuals to be used in performance of work for screening against the LEIE. During the performance of the contract, the Contractor shall provide the Contracting Officer the names of any additional or substitute employees for screening before they begin work. The Contractor is responsible for conducting security preclearance investigations sufficient depth to ensure that each Contractor or Subcontractor employee referred to I.H.S is not on the LEIE and can obtain a favorable fingerprint clearance. Each security preclearance shall be conducted sufficiently in advance of the start of performance to avoid delays caused by denial of access. If this is a Time and Materials, Labor-Hour or Cost Reimbursement contract, the Contractor shall not charge for or be reimbursed for labor hours or other costs incurred for employees who are unable to perform due to denial of the excess time required to resolve and clear unfavorable security clearance findings. If this is a Fixed Price contract, denial of access due to security clearance findings shall not be a basis for excusable delay or an increase to the contract amount, the Government will pay for the cost to process the Contractor's suitability clearances. However, multiple investigations for the same position may, at the Contracting Officer's discretion, lead to reduction(s) in the contract price of no more than the cost of the extra investigation(s).
