- The purpose of this Source Sought Notice is to search for small business concerns capable to provide LPN and NA Nursing Services for the Tennessee Valley Healthcare System (TVHS). This Sources Sought Notice is for market research purposes only and shall not be considered an Invitation for Bids, Request for Quotations, or a Request for Proposal. Do not submit a proposal or quote. This market research is issued for information and planning purposes only and does not constitute a solicitation nor does it restrict the Government as to the ultimate acquisition approach. In accordance with FAR 15.201(e), responses to this notice are not offers and cannot be accepted by the Government to form a binding contract. Additionally, there is no obligation on the part of the Government to acquire any products or services described in this sources sought. You will not be entitled to payment for direct or indirect costs that you incur in responding to this Sources Sought Notice. Any contract that might be awarded based on information received or derived from this market research will be the outcome of a competitive process. Interested parties are responsible for adequately marking proprietary, restricted or competition sensitive information contained in their response. The Government does not intend to pay for the information submitted in response to this Sources Sought Notice. Point of Contact: Sara Vickroy sara.vickroy@va.gov Contracting Officer The draft Statement of Work (SOW) follows: Description/Specification/Work Statement 1. General Information 1.1 Introduction and Overview This requirement is for licensed practical nurse and nursing assistant staff to provide direct patient care at the Nashville and Alvin C. York campuses, and the Community Based Outpatient Clinics located within the Tennessee Valley Healthcare System (TVHS). Agency staff will provide flexible and reliable staff for nursing coverage as the census, patient acuity or other staffing needs dictate. 1.2 Background Nursing Service currently has over 100 vacancies and continues to struggle with losses which cannot be replaced in a timely manner. The ability to deliver patient care in a safe manner has been compromised by the number of vacancies in the service. This has affected patient care and there is a critical shortage of nursing staff. 1.3 Scope of Work Procurement of a temporary pool of staff that can and will be used at the sole discretion of the facility based on patient care needs which can consist of as little as 32 hours a week or up to 40 hours per week. Pool should consist of licensed practical nurses and nursing assistants to provide hands on patient care on the medical surgical units, critical care units, community living centers, acute psych, outpatient clinics, Community Based Primary Care Clinics, and any other area of Nursing Service. 2. Requirements 2.1 Tasks and Skills Required 2.1.1 Licensed Practical Nurse (LPN) "Follows VA's regulations, especially pertaining to diagnostic and treatment procedures. "Follows admission and discharge procedures, orients new patients, and documents the process completely. "Cares for patients who are medically and/or emotionally seriously ill. "Handles patients with extreme symptoms, including agitation, hyperactivity, suicidal ideation/intent, catatonia, regression and confusion. "Relates therapeutically with patients on a one to one basis according to their needs and the plan of care. "Monitors patient's dietary intake and nutritional status. "Supervises meals to encourage good nutrition and safe eating habits. "Performs work within the safety and infection control policies and guidelines. "Practices infection control procedures for protection of self, patients and others. "Applies acceptable lifting of patients, provides patient care, administers medications, lifting, moving and/or transferring patients using equipment to provide patient care usually involves lifting and/or pushing the equipment (e.g. medication cart). "Use proper body mechanics, such as squatting instead of stooping, bending or twisting. "Appropriately use personal protective equipment and clothing during performance of duties whenever there is an anticipated occupational exposure to blood, body fluid and/or tissue. "Removes personal protective clothing in designated containers. "Attends required training about occupational hazards and required protective measures as required. "Reports all accidents in a timely manner (within the same work shift the incident occurs unless extenuating circumstances exist) "Assures all paperwork is completed within appropriate time limits required by TVHS policies. "Uses current knowledge of pharmacology to recognize and report therapeutic and/or untoward effects of medications. "Uses practical knowledge of human behavior, motivations, and reactions to situations in working effectively with patients, family and other staff members. "Actively participates in ward in-service programs and the orientation for Contractor employees. "Uses principles of adult education to teach patients and their families about treatment needs and procedures to be used in the medical center and at home. "Participates in change of shift by communicating all pertinent information to Team Leader, Charge Nurse and/or receiving shift. "Accurately enters and retrieves patient information via computer/written/verbal means, maintaining confidentiality and privacy of patient information obtained in order to complete required job duties. 2.1.2 Nursing Assistant (NA) "Safely transports patients, specimens, records, etc, as assigned. "Provides patient assistance with ADLs such as bathing, toileting, ambulating and other hygiene functions. "Perform veni-puncture as applicable (with validated competency only) using established technique and procedures following all safety precautions. "Initiates appropriate procedures in all emergency situations (i.e. cardiopulmonary arrest, seizures and obstructed airway, etc. "Assists co-workers and works as a team member. "Participates in training as assigned, completes all training in a timely manner. "Enters and retrieves computer patient information with guidance. "Maintains confidentiality of patient information via written, verbal and/or electronic means. "Responds to patient requests for assistance timely and effectively. "Reports and documents care provided, patient response and changes in patient's physical or behavioral status. "Encourages rapport among patients, families, volunteers, co-workers, visitors and support services. "Provides input into the development of the Nursing Care Plan and for change of shift report by communicating pertinent information. "Demonstrates compassion, respect and empathy in communicating with patients, families, volunteers, co-workers, visitors and support services. "Practice safety precautions according to written procedure in performance of duties when working with patients, patient's personal care devices and unit equipment. "Maintains a safe/clean environment. "Safe and economic use of supplies. "Proper duties in a safe manner to prevent patient/employee accident/injury. "Reports potential safety hazards to management promptly. "Assures proper patient identification using two positive identifiers before administering care. "Demonstrates customer-first, customer focused skills in interactions. "Professional and helpful manner using appropriate communication and actions. "Reports to duty as scheduled on time. "Handles conflicts and problems appropriately. "Assists in giving patients and families basic information about policies and procedures. Nurse Managers in the assigned area will be responsible for ensuring quality patient care. Agency staff will be held to the same performance and professional standards as employees of the Tennessee Valley Healthcare System. 3. Supporting Information 3.1 Place of performance Staff will work in primary care at the Community Based Outpatient Clinics or at the Nashville and Alvin C. York Campuses. 3.2 Period of Performance The period of performance is October 1, 2016 until September 30, 2017 with the option for two (2) one-year option periods. 3.3 Special Considerations The services provided by the contractor will be performed in accordance with VA policies, procedures, Tennessee Valley Healthcare System Memorandums, VHA Directives, Nursing Service Policies and Procedures, Joint Commission Accreditation Manual and Patient Safety Goals. Copies of policies will be provided to them upon request, to include: "Missing Patient Policy - 626-03-1136-20 "Employee Attire - 626-05-05-27 "Hand Hygiene - 626-05-118-03 "Absence and Leave - 626-06-05-07 "Standard Blood/Body Fluid Precautions Policy - 626-01-111-05 "Code Blue/Stat Calls: Respond to Medical Emergencies - 626-02-11-12 "Information Security Policy - 626-05-005-02 "Standards of Ethical Conduct - 626-06-118-12 "Restraints and Seclusion - 626-06-118-12 "Privacy and Release of Medical Information and FOIA Requests - 626-10-00-34 3.4 Contractor Furnished Materials - N/A 3.5 Government Furnished Materials and Services All agency staff must complete Nursing Core Orientation, including the BCMA & CPRS computer class, VHA Privacy training, and VHA Cyber Security Awareness training provided to all new nursing employees prior to receiving any work assignments. Any monies paid out for orientation/training will be reimbursed by agency to facility in the event the employee does not return to duty. Agency staff on some units may be furnished a locator badge that works with the nurse call system. It is the responsibility of the agency to ensure that locator badge is turned in by the agency employee at the end of their assignment with TVHS. Failure to do so will result in the agency paying for the cost to replace the locator badge. 3.6 Contracting Officer's Technical Representative A Contracting Officer's Technical Representative (COTR) will be designated in writing for furnishing technical guidance and advice during the term of the contract. This includes monitoring the general direction of the work to ensure full compliance with all terms and conditions of the contract. 3.7 Requests for Service / Cancellation The COTR or designee with provide to the contractor the number of hours required. In the case of short-term notices for personnel, the Government shall use its best efforts to request personnel at least four (4) hours prior to the start of the shift. In case of cancellation by the contractor, contractor shall provide replacement within two (2) hours of the scheduled shift. In case of cancellation by the Government, the Government shall notify the contractor within two (2) hours prior to start of the shift. If the notification is less than two (2) hours and contractor is not able to contact personnel, the contractor will pay agency nurse for four (4) hours and expect the agency nurse to work for four (4) hours. In case of cancellation by the contractor or agency nurse, contractor shall provide replacement within two (2) hours of the scheduled shift. If agency is unable to fill the shift, contractor will deduct the equivalent of two (2) hours salary from the Government invoicing for the role that was not filled. 3.8 Personnel Policy / Parking The contractor shall be responsible for protecting the personnel furnishing services under this contract. To carry out this responsibility, the contractor shall provide the following for these personnel: "Worker's Compensation "Professional Liability Insurance "TB Skin Testing "Income Tax Withholding "Social Security Payments "Parking Reimbursement "Fit mask test (where appropriate) The parties agree that such personnel shall not be considered VA employees for any purpose and shall be considered employees of the contractor. The contractor shall be responsible for the provision of appropriate parking passes for their personnel furnishing services under this contract. 3.9 Medical Records Patient medical records are confidential and are protected by the Privacy Act of 1974, 5 U.S.C. 552a; VA Claims Confidentiality Statute, 38 U.S.C. 5701 and Drug/Alcohol Abuse Confidentiality Statute, 38 U.S.C. 7332 [Authority: Drug/Alcohol Reg. 38 C.F.R. 1.460]. Any individual making unauthorized disclosures may be criminally liable for violations of the Act. 3.10 Contract Administration The Contracting Officer is the only person authorized to approve changes or modify any of the requirements under this contract. The Contractor shall communicate with the Contracting Officer on all matters pertaining to contract administration. Only the Contracting Officer is authorized to make commitments or issue changes, which will affect price, quantity, or quality of performance of this contract. In the event the contractor effects any such change at the direction of any person other than the Contracting Officer, the change shall be considered to have been made without authority, and no adjustment will be made in the contract price to cover any increase in costs incurred as a result thereof. 4. Qualifications of Key Personnel 4.1 Licensed Practical Nurse "Education-Graduate of a school of practical or vocational nursing approved by the appropriate state accrediting agency and/or the National League of Nursing. "Experience- Completion of at least two years of experience specific to unit assignment. He/she has the knowledge and skills necessary for concentrated effort and self-direction in carrying out the most complex practical nursing care assignments, including assignments for selected acute care patients requiring skilled or specialized nursing care. "Licensure-Nurse will maintain current, in good standing and unrestricted license while caring for VA patients 4.2 Nursing Assistants "Experience-Completion of at least one year of experience specific to assigned unit. He/she has the knowledge and skills necessary for concentrated effort and self-direction in carrying out primary bedside nursing care assignments. All agency staff will be required to have a current BLS issued as a Health Care Provider from the American Heart Association, proof of TB skin test within last 12 months and receive a qualitative fit-test for a N-95 Kimberly Clark "Duckbill" respirator per OSHA RESPIRATOR STANDARD GUIDELINES prior to arrival. The contractor will be responsible to ensure that contractor employees providing work on this contract are fully trained and completely competent to perform the required work. The contractor is required to maintain records that document competence / performance level of contractor employees 5. Security 5.1 Computer Security VA Handbook 6500.6 (https://www.voa.va.gov/DocumentView.aspx?DocumentID=228) 5.2 Contractor Personnel Security Requirements All contractor employees who require access to the Department of Veterans Affairs' computer systems shall be the subject of a background investigation and must receive a favorable adjudication from the VA Office of Security and Law Enforcement prior to contract performance. This requirement is applicable to all subcontractor personnel requiring the same access. If the investigation is not completed prior to the start of the contract, the contractor will be responsible for the actions of those individuals they provide to perform work for VA. "Position Sensitivity - The position sensitivity has been designated as low risk. "Background Investigation - The level of background investigation commensurate with the required level of access is National Agency Check with written inquiries. "The contractor shall bear the expense of obtaining background investigations. If the investigation is conducted by the Office of Personnel Management (OPM), the contractor shall reimburse VA within 30 days. "The contractor shall prescreen all personnel requiring access to the computer systems to ensure they maintain a U.S. citizenship and are able to read, write, speak and understand the English language. "The contractor shall submit or have their employees submit the following required forms to the Contracting Officer within 30 days of receipt: "Optional Form 306, Declaration for Federal Employment "Department of Veterans Affairs Electronic Fingerprinting Memorandum "Background Investigation Worksheet "The contractor, when notified of an unfavorable determination by the Government, shall withdraw the employee from consideration from working under the contract. "Failure to comply with the contractor personnel security requirements may result in termination of the contract for default. 6. Evaluation Information 6.1 Format The candidate information must be submitted in the following format: 6.1.1 Technical (Key Personnel's Experience and Qualifications) "List of education and training "Summary list showing evidence of license, certification and/or registration, and expiration dates. "Statement regarding contractor's evaluation of their candidate assuring that the individual's knowledge, experience and competence are appropriate for his/her assigned responsibilities as required by this task order. Candidate must be provided a copy of this task order and fully understand what is required in their performance. Additionally, contractor may provide copies of recent performance evaluations/appraisals. "Statement regarding proof of health status requiring candidate to be physically able to cover job responsibilities noted herein. "Statement from contractor verifying candidate to have a clean criminal background check and pre-employment verification of no convictions for abuse or neglect, as required by VHA regulations. 6.1.2 Availability "Provide the date the candidate will be available to work for the TVHS. 6.1.3 Past Performance "At least 3 written references attesting the contractor employee's abilities and competencies to perform the required procedures as described in the statement of work. 6.1.4 VET-Pro "Statement whether nursing candidate is currently VET-Pro Certified or not. 6.1.5 Cost / Price "Cost and/or price must be an all-inclusive rate. 6.2 Basis for Award Award will be made to the offeror determined to be the "best value" to the Government.
