Description

This amendment is issued to remove one of the pickup locations from the Performance Work Statement. The Sierra Foothills Outpatient Clinic will not be served under this contract because that location is already covered under a separate contract. A revised copy of the Performance Work Statement has been attached to this amendment. The price schedule in Attach #4 did not include the Sierra Foothills location, so no changes are needed for the price schedule. There are no other changes to the terms and conditions of the solicitation. Quotes are still due by 4:00 p.m. on February 12, 2016.
