Draft Performance Work Statement dated 5 February 2016 THIS IS A SOURCES SOUGHT ANNOUNCEMENT ONLY. This is NOT a solicitation, but market research to determine potential sources for teleradiology reading services and/or furnishing equipment, as necessary, to provide professional interpretations of diagnostic imaging studies for the 81 st Medical Group (81 MDG), Medical Treatment Center (MTF), Keesler AFB, MS, in accordance with the draft Performance Work Statement (PWS) dated 5 February 2016. This notice does not constitute a promise to issue a solicitation in the future. Further, we are not at this time seeking proposals; therefore, DO NOT request a copy of a solicitation or submit a proposal as a result of this notice. Responders are advised the Government will not pay for any information or administrative costs incurred in response to this sources sought and that all submissions become Government property and will not be returned. All costs associated with responding to this sources sought will be solely at the responding party's expense. Not responding to this sources sought does not preclude participation in any future solicitation, should one be issued. Responses to this notice are not offers and cannot be accepted by the Government to form a binding contract. No basis for claim against the Government shall arise from a response to the notice. It is the responsibility of the interested parties to monitor the FedBizOpps website for additional information pertaining to this sources sought. It is anticipated the performance period for this requirement will be one (1) base period of 12 months and two (2) 12-month option periods. A firm-fixed price contract is contemplated. Keesler AFB does not currently have this service. The applicable NAICS Code is 621512 (Diagnostic Imaging Centers) with a size standard of $15.0M. RESPONSE INFORMATION: Responses to this Sources Sought Notice must be received no later than 3:00 PM CST, on Wednesday, 17 February 2016. Responses should be emailed to: michelle.brown.1@us.af.mil AND kimberley.alvarez.1@us.af.mil. It is the responsibility of the respondent to ensure their email has been received by contacting either the Contract Specialist, Michelle Brown, at 228-377-1801 or Contracting Officer, Kimberley Alvarez, at 228-377-1837 or via email PRIOR to the deadline. Responses and/or inquiries WILL NOT be accepted past the deadline. Note that our office will be closed Monday, 15 February 2016 for Presidents Day. Interested parties should include their company name, address, phone number, point of contact, CAGE code, company size status and whether or not they are a U.S. or foreign-owned firm. In accordance with the PWS dated 5 February 2016, all interested parties must provide a response containing a written statement, confirming they can comply with the government needs, as specified in the PWS and as listed below. All respondees to this Sources Sought Notice understand, by submitting this written statement, they are certifying they can meet ALL criteria. The written statement must also address the following items: The ability to provide non-personal professional Diagnostic Radiologist via teleradiology, connecting to the U.S. Air Force Information Network (AFIN) to provide image interpretation, consulting and technical assistance services for the following studies: Basic Film, Computed Tomography (CT), Magnetic Resonance Imaging (MRI), Ultrasound Studies, and Nuclear Medicine. The contractor must CURRENTLY be authorized to connect to the Air Force network; have, maintain and provide evidence of an active Department of Defense Information Assurance Certification and Accreditation Process (DIACAP) accreditation, Authorization to Operate (ATO) and Authorization to Connect (ATC) for the Air Force, a Defense Information Systems Agency (DISA) Business-to-Business (B2B) connection and HL-7 Healthcare System (CHCS) for automatic upload of report interpretations into a patient’s Electronic Medical Record (EMR). The contractor must possess a valid unrestricted license from any U.S. State to practice Diagnostic Radiology and be certified by the American Board of Radiology (ABR) or the American Osteopathic Board of Radiology (AOBR) in same. The contractor must be able to provide on-call services seven (7) days a week, twenty-four (24) hours per day, three hundred sixty five (365) days per year. This includes evenings, weekends and holidays. The contractor must be able to submit accurate credentialing packages for its personnel no later 30 calendar days after the initial date of performance.
