Description

RFP SP4703-16-R-5004 Part 1 Attachment 1 - Sample Task Order PWS Attachment 2 - Key Personnel Template Attachment 4 - Past Performance Questionnaire Attachment 3 Rough Order of Magnitude RFP SP4703-16-R-5004 Part 2 This acquisition is being issued to provide Continuous Process Improvement (CPI)/Lean Six Sigma (LSS) Support Services for the Defense Logistics Agency, Headquarters (DLA, HQ) to include: expert consultation, advisory services for CPI/LSS and associated methodologies to include Lean Six Sigma, Enterprise Process Management, Business Process Engineering, Information Technology (IT) systems administration, and Operations Research. All Task Orders (TOs) issued off the Indefinite Delivery Quantity (IDIQ) contract will be performance based acquisitions utilizing a Performance Work Statement (PWS). Following the award of the IDIQ, the specific scope of each effort will be based on the specific tasks to be set forth in individual task orders of the following areas: TASK AREA 1 - Continuous Process Improvement Services TASK AREA 2 - Program Management Services TASK AREA 3 - Change Management Services TASK AREA 4 - Training and Curriculum Development/Maintenance Services Attachment 1: Sample Task Order Performance Work Statement (PWS) Attachment 2: Key Personnel Template Attachment 3: Rough Order of Magnitude (ROM) Attachment 4: Past Performance Questionnaire (PPQ) While the CPI/LSS support will primarily support DLA requirements, the resultant contract will also allow for ordering by other DoD agencies.
