W9 Vendor Questionnaire Wellness Building Cleaning This is a Procurement Opportunity with Adams & Associates Inc. at the Woodland Job Corps Center and not with the Federal Government. Mailing Address: Woodland Job Corps Center ATTN: StaceyAnn Treasure 3300 Fort Meade Road Laurel, MD 20724 Site Visit: N/A Proposal Due Date: February 22, 2016 3:00pm EST Submission of Proposals: Woodland Job Corps Center Attn: StaceyAnn Treasure Treasure.stacey@jobcorps.org Project Summary: Adams and Associates, Inc., contracting for the U.S. Department of Labor for the operation of the Woodland Job Corps Center - a 300 student residential training facility located at Woodland Job Corps Center, 3300 Fort Meade Road, Laurel, MD 20724, is currently soliciting written bids for a Wellness Building Cleaning for the period of performance is March 1, 2016 to October 31, 2016. Please see the complete Statement of Work in Section IV Extend of Work 1. Each bidder shall submit a bid based upon Part IV - Extent of Work that includes reference to the project listed in this scope of work. Any variations from the base scope of work are to be noted as distinctly separate from the base bid. •2. Bids shall be submitted for the work as a basis for the contract. The prospective bidder shall submit a bid for a Single Lump Sum Contract. •3. The bid shall include a breakdown of the hourly cost to provide the service. •4. A minimum 90-day bid guarantee is required. •5. This project is tax-exempt. •6. Prior to starting any work, the contractor shall show proof of required insurance, in amounts to cover risk as stated below: •· Comprehensive Automobile Liability Insurance ( $1,000,000 combined single limit) •· Workers Compensation / Liability Insurance ($1,000,000 per occurrence) •· Fire Legal Liability of at least $500,000 per occurrence. •· Fire Legal Liability of at least $500,000 per occurrence. •7. Final Bid Proposal shall be typed and submitted on official Company letterhead and must include the following : •· Scope of Work ID#, Project Description, Project Specific Requirements, Lump Sum Cost •· References •· Copies of all Insurance Certificates •· Copy of all licenses and certifications relevant to this proposal •· Completed Vendor Questionnaire (with DUNS# if available) and W9 •· Small Business Certification, if any •8. Net 30 payment terms Award will be made to the offeror who provides Best Value to the Center when all Evaluation Criteria are considered. Lowest Price does not guarantee award of Sub Contract. Adams and Associates retains the right to reject any and all bids. Services. All quotes must be submitted in writing to the Purchasing Department by February 22, 2016. Quotes received after the deadline and/or without all requested documentation will not be considered. Vendor is to return written formal bid on company letterhead, completed vendor paperwork, and all licenses for doing business in the State of Maryland, all employee licenses required for performing work in the state of Maryland, Certificate of Liability insurance and list of references if no past experience working with the Center. The period of performance is March 1, 2016 to October 31, 2016. Submitted bid will show at minimum breakdown as follows: •· Cost per service visit •· Printed name and Signature of authorized person submitting bid Bids, vendor questionnaire, W9, Authority to Commit if necessary, and all licensures needed to do business may be faxed to 301-362-4493, Attn: StaceyAnn Treasure, Purchasing Department; emailed to: treasure.stacey@jobcorps.org ; or mailed to: Woodland Job Corps Center Attn: StaceyAnn Treasure 3300 Fort Meade Road Laurel, Maryland 20724 SCOPE OF SERVICES Specific Requirements: •· All bidders must make an appointment to see both Wellness and Dental areas in order to submit an accurate bid. •· Price to include stripping, waxing, burnishing of tile floors and cleaning of carpet as needed for both Wellness and Dental areas. •· Must provide all equipment and supplies to effectively and efficiently clean the Wellness and Dental Areas (Cleaners, trash bags, brooms, mops, vacuums, etc.) •· Weekly cleaning/sanitizing of patient areas and doctor offices to follow: Monday and Thursday, 6:00PM - 8:00PM for Wellness & Dentist Area
