Description

Please note this is NOT a request for quotes or proposals. The Government is seeking information strictly for market research purposes only. The Government may or may not issue a solicitation document. The VA Northern Indiana Health Care System (VANIHCS) in Fort Wayne, Indiana is conducting a market survey for qualified firms capable of providing urology physician services. Applicable NAICS Code 621111 applies with a business size standard of $11.0 million and prospective offerors must be registered in the Systems for Award Management System (SAM). Responses to this request for information (RFI) must be submitted in writing (electronically preferred) and received no later than 5pm EST, Friday, February 19, 2016. No telephone inquiries will be accepted or returned. A brief description of work to be performed includes: The VANIHCS has a need for 1.0 full-time equivalent (FTE) or 2,080 annual hours board-certified/board-eligible Urology Physician Services. Contractor shall provide clinic and surgical care for Veterans, as well as provide professional oversight and training under a collaborative agreement for two (2) VA staff Nurse Practitioners. Contractor shall also provide Urological on-call coverage services, including weeknights, weekends and holidays, and after hours/emergency call back coverage services. Contractor shall provide 28 hours of clinic, 8 hours of surgery, and 4 hours of mandatory training, peer review, and/or meetings weekly. Tentative schedule in the urology clinic is 8 hours a day on Monday, Tuesday, and Thursday. Surgery will be 8 hours weekly on Wednesdays. Friday's schedule will be 4 hours clinic with 4 hours of administrative time. The purpose of this synopsis is to gain knowledge of potential qualified sources and their size classification (large business, service-disabled veteran-owned small business, veteran-owned small business, HubZone, 8(a), small disadvantaged business, or small business) relative to NAICS Code 621111 (business size of $11.0 Million per annum). Responses to this synopsis will be used by the Government to make appropriate acquisition decisions. Responses should include: company name, service capability statement, examples of similar facilities to which same or similar services were provided, DUNS number, including address, point of contact, and the size of the business. A solicitation is NOT currently available. If a solicitation is issued, it will be announced on the Federal Business Opportunities website at http://www.fbo.gov at a later date, and all interested parties must respond to the solicitation announcement separately from the responses to this announcement.
