Description: This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6 - Streamlined Procedures for Evaluation and Solicitation for Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation is being issued using Simplified Acquisition Procedures. The combined synopsis/solicitation number is VA119-16-Q-0027 and is issued as a Request for Quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005--84 (Effective October 5, 2015). This is an Unrestricted, Full and Open procurement. The associated North American Industrial Classification System (NAICS) code for this procurement is 339112. Type of Contract: The Department of Veterans Affairs (VA) intends to award a Firm Fixed-Price (FFP) Blanket Purchase Agreement (BPA) in accordance with FAR Part 13.303 with an ordering period consisting of a one year base period and four, 12-month option periods for the VA Office of Procurement and Logistics, Surgical Program Management Office. Each award will be per CLIN. One vendor may be awarded multiple CLINS. Description of requirements for the items to be acquired: The purpose of this BPA is to provide Endoscopic Snares throughout the Department of Veterans Affairs, Veterans Health Administration (VHA) facilities and secure volume based pricing on the items required. The intent is to standardize ENDOSCOPIC SNARES for all VHA medical centers and facilities. Awardee(s) shall deliver to all U.S. facilities and U.S. territories, which includes Alaska, Hawaiian Islands, Philippines Islands, Guam, American Samoa, Puerto Rico, and U.S. Virgin Islands. Calls will be placed against this BPA in writing, by phone, or via facsimile. Each Contract Line Item Number (CLIN) will be priced to meet the need for all VHA medical centers and facilities throughout the VA enterprise. The list of CLIN(s) and items, quantities and units of measure, including ordering periods and a detailed description of this requirement is provided in the Instruction to Quoters within the Minimum Technical Requirements tables provided herein. The quantities provided below are projections provided per-year, per ENDOSCOPIC SNARE. The following quantities of ENDOSCOPIC SNARES are estimates only and do not obligate the Government to purchase stated quantities. The Government estimates, but does not guarantee, that the volume of purchases through this BPA will be as set forth in the below Schedule. The Government is seeking per unit discounts based upon the estimated annual volume / usage. This BPA does not obligate any funds. Award will be made on the basis of the lowest evaluated price of quotes meeting or exceeding the acceptability standards for non-cost factors. Quotes will be evaluated for technical acceptability. To receive consideration for award, the proposed item(s) must meet all of the minimum technical requirements (MTRs) as indicated via a review of technical product literature review and product samples, including a technical evaluation test. Quoters may quote on one CLIN (with options), multiple CLINS (with options), or all CLINS (with options). The Government intends to issue a single BPA per CLIN to one contractor that will serve as the single source of supply to provide that ENDOSCOPIC SNARE type enterprise-wide, considering standardization is a top priority of this acquisition. Successful award of this acquisition will allow VHA to maintain uniformity amongst the types of ENDOSCOPIC SNARES being utilized by its field activities.
