- Attached please find RFQ number RFQ-DC-15-00190 (eBuy RFQ 1063096), which is being issued to procure services to support the U.S. Environmental Protection Agency ¿s (EPA) Environmental Response Team (ERT) from GSA Schedule number 899: Environmental Consulting Services 899-1. Quotes are being requested in accordance with Federal Acquisition Regulation (FAR) Part 8.405-3, Blanket Purchase Agreements (BPAs). This BPA is being established to provide support for the Environmental Response Training Program (ERTP). The Environmental Response Team (ERT), part of the Office of Superfund Remediation and Technology Innovation (OSRTI) is responsible for developing and managing the Environmental Response Training Program. The training program is designed to train EPA personnel, as well as personnel from federal, state, and local government agencies in techniques and methods for preventing, evaluation, mitigating, and remediating chemical releases while protecting the health and safety of response personnel and the public. In addition, to increase the number of training courses available to the response community, EPA has developed the External Provider Program (EPP), which provides training and basic course material packages to qualified training providers. These providers then conduct the training and participants receive EPA certificates upon completion of the training. The training courses presented under the training program last from 1 to 5 days. They provide participants with fundamental information about the subjects covered, providing a foundation for developing knowledge and skills in a variety of response activities. All presentations emphasize the practical application of instructional material through problem-solving, case studies, lectures, workshops, demonstrations, exercises, and special training sessions. The Government anticipates establishing a single-award BPA as the mechanism to procure support for these services. The anticipated duration of the BPA is estimated to be June 1, 2016 through May 31, 2021 for a maximum total of five (5) years. In accordance with FAR 8.405-3(d)(2), the period of performance for this BPA shall consist of a one (1) year Base Period and four (4) one (1) year Option Periods. RFQ ATTACHMENTS & QUOTE PREPARATION INSTRUCTIONS: This RFQ consists of the attachments as listed below. Please review each in its entirety as you prepare your response. ¿Standard Form 18 ¿Applicable Clauses (Attachment 1) ¿Performance Work Statement & List of ERTP Courses (Attachment 2) ¿Labor Categories & Qualifications (Attachment 3) ¿Pricing Instructions & Schedule of Professional Services (Attachment 4) ¿Technical Evaluation Criteria (Attachment 5) ¿Past Performance Questionnaire (Attachment 6) ¿Client Authorization Release Letter (Sample) (Attachment 7) ¿EPA Requirements for Quality Management Plan (Attachment 8) ¿Invoice Preparation Instructions (Attachment 9) Quotation packages submitted in response to the RFQ shall include the following: 1) An executed copy of the SF-18. Please ensure blocks 12 thru 16 are completed. 2) A written technical submission that addresses the technical evaluation criteria and a pricing proposal that provides individual loaded labor rates for all positions identified, including GSA contract hourly rates and any applicable discount percentage for each year as specified in the Schedule of Professional Services. Contractors shall propose other direct costs (ODCs) as specified in the Schedule of Professional Services (Attachment 4). If additional ODCs are anticipated outside of those listed, the quote shall identify such costs as separate line items and provide a narrative explanation of the basis for including the additional ODC category. Contractors shall complete Attachment 4, Pricing Instructions and Schedules for Professional Services for the Base Period and Option years. 3) An acknowledgement that your company is registered and has an up-to-date (active) record in the System for Award Management (SAM). Please provide your DUNS number so your credentials may be retrieved from https://www.sam.gov/portal/public/SAM/ 4) A copy of the signed cover page of your GSA contract award. Length of Proposal: Responses shall be clear, concise, and grammatically correct. The responses shall be formatted to include all the components as identified above. Proposals (technical and pricing combined) are limited to 50 (double-sided) pages including tables and figures. Font size for text for the response and all attachments shall be 11-point or larger (Times New Roman) and the pages shall have 1-inch margins. Each individual evaluation criteria listed in Attachment 5 shall have its own separate tab within the package submitted. Resumes for proposed personnel and tabs are not included in the page count. NOTE: Failure to comply with the requirements of this RFQ and submission of an incomplete quote package may eliminate a contractor from being considered for this award.
