Description
This is an INTENT TO SOLE SOURCE NOTICE only. The Department of Veteran Affairs, Network Contracting Office (NCO) 16, intends to issue a sole-source contract under the authority of 41 U.S.C 253(c) (1) - FAR 6.302-1 (only one responsible source and no other supplies or services will satisfy agency requirements), to Elekta Inc., located at 4775 Peachtree Industrial Blvd, Norcross, GA 30092. Services will be from 01/28/2016 to 09/30/2016 plus four (4) option periods under NAICS 811219. The Vendor shall provide: Statement of Work for Full Service Support for Linear Accelerator Contractor shall furnish all necessary labor, parts, transportation and equipment to provide Oncology Advanced coverage, per agreement 2015-96919-TT, for government-owned linear accelerator, all glassware and subassemblies, as listed below, located at Overton Brooks VA Medical Center, 510 E. Stoner Avenue, Shreveport, LA 71101 in accordance with all terms, conditions, provisions, schedules and specifications for prorated base year from 1/28/2016 through 9/30/2016, and four option years through 9/30/2020. Items covered: Elekta Linear Accelerator Model: Infinity Serial No: 153535 EE# 42955 Elekta Site: 10902003 Other covered items: Magnetron Thyratron, Electron Gun MLC, IView Camera, IViewGT Amorphous Silicon Panel, XVI Imaging Panel, X-ray Tube Image intensifier, All associated hardware, computers and servers. Other terms: -Contractor shall guarantees 98% uptime with this Oncology Advanced Agreement -Contractor shall provide two preventive maintenance inspections per year IAW manufacturer's requirements. -Contract price shall include a price discount for initial repair attempt (first-look) by in-house biomedical engineering technicians -The contractor shall schedule preventive maintenance visits at least 48 hours in advance. All work shall be accomplished during medical center's administrative working hours, 8:00 am to 9:00 pm, Monday through Friday, excluding federal holidays. -The following are federal holidays: New Year's Day Labor Day Martin Luther King Jr.'s Birthday Columbus Day President's Day Veteran's Day Memorial Day Independence Day Thanksgiving Day Christmas Day -Only new standard parts (manufactured by the maker of the equipment or equal thereto) shall be furnished by the Contractor. All parts shall be of current manufacture and shall have full versatility with presently installed equipment. -The contractor will coordinate all maintenance visits through the COR or other Biomedical Engineering representative. Only the COR or other -Only the COR or other Biomedical Engineering representative can authorize billable overtime. Upon completion of work, a copy of the service report will be provided to Biomedical Engineering. Failure to do so may delay payment. -Overton Brooks VA Medical Center is a smoke free facility. Smoking is prohibited inside any building. Smokers are allowed to smoke outside in designated smoking areas only. Violators shall be removed from the premises permanently. -The Contracting Officer's Representative: Jim Slepicka 318-990-5543 The linear Accelerator is used to deliver high-dose radiation treatments for cancer patients, and is crucial to achieving favorable patient outcomes (life or death). It must be working optimally to the greatest extent to for safe and effective treatments for cancer patients, as scheduled. Missed treatments may result in unfavorable treatment outcomes. Elekta is the manufacturer of this equipment. They have specialized, proprietary knowledge of this equipment and are the official repair source for this system. This notice of intent is not a request for quotations; interested parties may express their interest by providing a capabilities statement not later than 01/20/2016 at 12pm CST, to Tina.Harris3@va.gov. When responding to this announcement, respondents should refer to Announcement VA256-16-Q-0071-1 Notice of Intent in the subject line. The capabilities statement must provide clear and unambiguous evidence to substantiate the capability of the party to provide the required services. Responding as an interested party under the www.fbo.gov website doesn't constitute convincing evidence of capabilities. A determination not to compete this proposed contract upon responses to this notice is solely within the discretion of the Government. Verbal and facsimile responses are not acceptable and will not be considered. All interested parties are reminded to be registered with SAM at https://www.sam.gov/portal/public/SAM/ in order to be eligible for award of Government contracts. No telephone calls will be accepted. If after 01/20/2016, no viable responses have been received in response to this announcement, NCO 16 shall negotiate solely with Elekta, Inc. The Government anticipates the award of a firm-fixed price contract. A determination by the Government whether to compete the proposed requirement shall be based upon responses received and is solely within the discretion of the government. Information received shall be considered solely for the purpose of determining whether to conduct a competition.
