This is a SOURCES SOUGHT ANNOUNCEMENT ONLY. It is neither a solicitation announcement nor a request for proposals (RFP) or request for quotes (RFQ) and does not obligate the Government to award a contract. Requests for a solicitation will not receive a response. The purpose of this sources sought announcement is for market research to make appropriate acquisition decisions and to gain knowledge of potential qualified sources and their Small Business Administration (SBA) size classification in relation to North American Industry Classification System (NAICS) 339112. Documentation of technical expertise must be presented in sufficient detail for the Government to determine that your company possesses the necessary functional area expertise and experience to compete for this acquisition. Responses to this notice shall be in writing and include the following: a)Company name b)Address c)Point of contact d)Phone, fax, and email e)DUNS number f)Cage Code g)Tax ID Number h)Type of small business, e.g. Services Disabled Veteran Owned Small Business (SDVOSB), Veteran Owned Small Business (VOSB), 8(a), HUBZone, Women Owned Small Business (WOSB), Small Disadvantaged Business, or Small Business i)Provide a copy of your company's current Representations and Certifications from www.sam.gov. j)Must provide a capability statement that addresses the organizations qualifications and ability to perform as a contractor in accordance with the statement of work k)Provide any certifications and/or licenses, test/calibration equipment, and evidence of the ability of obtaining original equipment manufacturer parts, i.e. D-2 Operator License. l)Provide previous contracts that are similar in nature and complexity. m)Any other pertinent information that may assist the Government in relation to this requirement, commercial standards, industry best practices, etc. n)Estimated cost After review of the responses to this sources sought synopsis, a solicitation announcement may be published to www.FBO.gov. Responses to this sources sought synopsis are not considered adequate responses to the solicitation announcement. All interested offeror(s) will have to respond to the solicitation announcement in addition to responding to this sources sought announcement. The Department of Veterans Affairs, Network Contracting Office (NCO) 16, is seeking sources to provide a radiographic biopsy collection system for the Michael E. DeBakey VA Medical Center (MEDVAMC). Please see the attachment requirement details and scope of work. Comments and suggestions on the attached SOW is highly encouraged and recommended as this will assist the Government in conforming to best practices and commercial industry standards. The intended contract will be a Firm Fixed Price (FFP) award. The delivery address is as follows: Department of Veterans Affairs Michael E. DeBakey VA Medical Center 2002 Holcombe Blvd Houston, TX 77030-4298 Responses to this notice shall be emailed to the attention of Paul Filhiol at paul.filhiol@va.gov. Telephone responses will not be accepted. Responses must be received no later than December 15, 2015, NOON Central Time (CT). Ensure the following title is within the subject line of the email: "VA256-16-Q-0120: MEDVAMC Breast Biopsy System". Important information: The Government is not obligated to nor will it pay for or reimburse any costs associated with responding to this sources sought synopsis request. This notice shall not be construed as a commitment by the Government to issue a solicitation or ultimately award a contract, nor does it restrict the Government to a particular acquisition approach. The Government will in no way be bound to this information if any solicitation is issued. Introduction: oThe Mammtome system is the only system with a dual vacuum system, touch free breast biopsy collection system which automatically sorts tissue samples for radiographing, variable aperture setting, fluid management, and 200mg core samples. oFaxitron/Corevision Radiography system is in room Radiography device that allows us to confirm a successful biopsy without leaving the procedure room which helps to eliminate delays waiting for verification of core samples. oMammotest Table- which brings optimized access through: angled targeting via polar coordinates, all-angels 360 degree access, target on Scout option, lateral arm option, arm-through-hole technique. *******Any equal item quoted must conform to the capabilities listed above.****** STATEMENT OF WORK/DESCRIPTION OF WORK GENERAL INTENTION Contractor shall furnish all labor, materials, parts, equipment, tools, transportation, and calibration to provide Mammotome Biopsy System to the Michael E DeBakey VA Medical Center, 2002 Holcombe Blvd. Houston TX 77024. The Contractor shall provide all necessary training to the designated VA employees on how to use the product after the delivery and setup are complete. The contract price shall be firm-fixed, all-inclusive not limited to specified products/equipment, installation, setup/configuration, and training on how to use the installed products/equipment. All equipment shall be brand new; no refurbished or used equipment is acceptable. CONTRACT REQUIREMENTS 1.MEDVAMC would like to acquire a Mammotome Biopsy system for the new Breast Care Program with the following specifications: Mammotome Biopsy system oA dual vacuum system oTouch free breast biopsy collection system o360 degree needle rotation oNeeds to be compatible with multiple imaging modalities oFaxitron/corevision radiography system oMammotest table 2.End User Instruction and Training. Contractor shall provide on-site training to designated VA employees on how to use all the aforementioned system. Contractor shall coordinate with the Contracting Officer's Technical Representative for the schedule of this training. Training shall consist of proper use of the system, management of the system and maintenance of equipment. 3.Service. Contractor will provide one year or service for the equipment. Service contract will also include option years for this service to be extended for up to four (4) additional years. Service will meet the following minimum requirements: a.24 hour phone support Monday through Friday excluding government holidays. 4.User's Operating and Maintenance Manual. Contractor shall provide a complete set of User's Operating and Maintenance Manual at no charge to the Government. 5.Delivery Schedule. Contractor shall finish all works of installation, configuration, and setup within 30 days after receiving Notice to proceed from the Contracting Officer.
