Response date (Deadline): January 7, 2016 at 1:00pm Statement of Work (SOW) Intent to sole source A.GENERAL INFORMATION 1.Title of Project Scentair Aromatherapy for CLC 2. Scope of Work: The contractor shall provide all resources necessary to accomplish the deliverables described in this statement of work (SOW), except as may otherwise be specified for the above project. STATEMENT OF WORK Comprehensive one year program including all equipment and supplies necessary to accomplish the following results. The introduction of strategically selected scents (Scentair brand) to the air that can have a therapeutic effect on visitors and Residents. Additionally, due to the geriatric population of extended care and the prevalence of incontinence, we are aiming to reduce malodorous incidences in the common areas of the CLC. The following Equipment is being requested to accomplish this task. 3 units of scent wave + new cartridge replacement service (every 3-4 months) scent service for 1 year on CLC 1 1 Unit of Scent Direct + new cartridge replacement ( every 6 months ) scent service for dining room 1 Unit of Scent wave + new cartridge replacement ( every 3-4 months ) scent service for palliative care lounge area. Type of Procurement: The responses to this Request for Information (RFI) will help determine what, if any, type of set-aside. Contract Vehicle: The resulting contract vehicles from this process will be a firm fixed price. Responses to this notice will be used by the Government to make appropriate acquisition decisions. Responses to this notice should include company/individual name, a service capability statement, examples of similar facilities which you have provided services to, DUNS number, address, point of contact and social-economic category (ex: SBVOSB, VOSB, 8(a), HUBZone, WOSB, EDWOSB, Small Business). If your firm is a Service-Disabled Veteran Owned or Veteran Owned Small Business, you must be CERTIFIED in VetBiz (see internet site: http://vip.vetbiz.gov). Contractor must be registered in Central Contractor Registry (CCR) (see internet site: http://www.ccr.gov). Responses to this notice must be submitted in writing (email) and received not later than January 7, 2016 at 1:00PM.
