Description

DBA Wage Determination MTC Terms & Conditions W-9 Self-Certification Form Construction Bid Package Management and Training Corporation (MTC), operator of the Tongue Point Job Corps Center, is requesting proposals for some asbestos abatement that is urgently needed. Details of the solicitation are in the attached Construction Bid Package (CBP-TPJCC Bldg. 100 (Res4) Asbestos Abatement 15-45.pdf.) - Please note that there is a specific time frame that this service will need to be completed. Due to a related project currently on hold, we ask that project is completed no later than December 30, 2015.. In addition to the bid sheet, all valid bids must include a W-9 and completed MTC vendor self-certification form (attached). Bidders must self-certify under the NAICS listed in the RFQ. (NAICS codes can be found at http://www.census.gov/eos/www/naics/) This solicitation is listed under NAICS code: 562910 Remediation Services. All bids are due Monday morning, December 21, 2015 NLT 8:30AM (PST)
