AMENDMENT TO IDENTIFY CHANGE IN RESPONSE SCHEDULE AS FOLLOWS: Inclusion of Questions raised and responses by vendors having attended walk-through's of the facility. This solicitation is a request for proposal (RFP) in accordance with FAR part 12 in conjunction with FAR part 13.5. The solicitation document and incorporated provisions and clauses are those identified in the original document. The associated North American Industrial Classification System (NAICS) code for this procurement is 562211, Size Standard 38.5 Million dollars. All offerors shall be sent to the Contracting Officer: Steven S. Tompkins @ steve.tompkins@va.gov. Confirmation of receipt is required. All questions or concerns regarding this solicitation were received not later than 3:00 PM EST on Dec 11, 2015 and should be forwarded in writing via e-mail to the contracting officer, Steven S. Tompkins steve.tompkins@va.gov. Offeror's requesting a site visit must coordinate with the facility representative Alexia Briwa at Alexia.Briwa@va.gov, confirm visit with the contracting officer and complete the visit NLT Dec 7, 2015. Questions for Albany VA Medical Center RMW Solicitation "Does vendor have any responsibility for transporting or processing RMW from or changing sharps containers in the CBOC or ARF Buildings? If so, please list. "Vendor has responsibility for the ARF Building, not the CBOCs. "Is vendor responsible for any solid waste other than "treated and destroyed" RMW? "Vendor is responsible for RMW, not regular trash. "Annual weight of RMW is listed at 130,000 pounds; does this include the net weight of the sharps containers and their contents? "It does not include the sharps and their contents. "Please confirm that Albany VA Medical Center will provide all red bags and rattan containers. "Yes, Albany VA will provide all red bags and rattan containers. "The Albany VA Medical Center utilizes foot pedal operated containers and plastic barrels for RMW storage in the Soiled Utility Rooms and other locations. Are these containers the property of the VA and will they be available for use by the Vendor? "The containers are the property of the VA. "Albany VA Medical Center owns the carts currently used to transport the RMW from the soiled utility rooms to the processing area; will those carts be available, as government provided equipment, for vendor use? "Yes "Facility has certain RMW that it deems unacceptable for onsite processing (e. g. rattan containers of lab waste); will these items continue to be sent to offsite processing facility? "Yes "Will the large white jugs used in surgery and currently sent offsite continue to be sent for offsite processing? "The large white jugs are considered RMW. "Will vendor have access to a Geiger counter in the processing room as government provided equipment? "Yes "Will Albany VA Medical Center's Radiation Safety Officer provide necessary training to insure that vendor is qualified to operate the Geiger counter and is in compliance with the facility's regulations? "Vendor should provide trained personnel including Geiger Counter. "The Albany VA Medical Center provided a list of all sharps container locations by size and type. Can the Albany VA Medical Center provide annual use of sharps containers by size and type? "Data not available. "Will the Albany VA Medical Center provide storage space for the inventory of disposable sharps containers? "Yes "Can the Albany VA Medical Center provide a facility floorplan overview, by floor, that is commonly used for patients to find their way in the hospital? "Albany VA has a list of the soiled utility rooms. "Would Albany VA Medical Center like separate pricing for collection, processing and sharps container exchange on weekends, holidays or emergency situations? "Yes "The solicitation states that either a reusable or disposable sharps container can be offered. The NY DES does not allow for the manual (by hand) opening of sharps containers. Does the VA have the necessary robotic equipment to open reusable containers onsite? "No the VA does not. Are we just going to stipulate use of disposable containers? Yes we are. "If a disposable sharps container is provided by Stericycle, what are the hospitals container requirements (other than being compliant with the FDA) besides the gallon capacity? oThey need to meet the requirements that we already have in place. oWhat departments are involved if any in container selection, if making a change to disposable sharps containers (ex, nursing, housekeeping/environmental services, infection control). The sizes were provided as an attachment. "Is offsite destruction of sharps waste in conjunction with the use of a reusable sharps container in accordance with requirements of this solicitation? oIf the contractor can accomplish it that way, then it is acceptable. "The solicitation states " Contractor will be responsible for operation of the (IAW attached SOP), as well as all maintenance and repairs including parts and labor for the SSM-150." Please provide annual maintenance costs for the operation of a SSM-150 which processes both regulated medical waste and sharps waste at a same sized facility which processes at least 300,000 pounds of waste per year. oAlbany did not have to do maintenance in the past because it was part of the purchase contract. "Please provide annual maintenance costs for the operation of a SSM-150 which processes 300,000 pounds of only regulated medical waste annually and sends sharps waste offsite for alternative disposal. oAlbany did not have to do maintenance in the past because it was part of the purchase contract. oSharp removal and disposal was approximately $50,000 in FY15. "Please provide the average number of times a similar sized facility has been inoperable due equipment failure. oUnknown "Please provide Red Bag Solutions pricing list for services (emergency and standard), labor hours, equipment costs, maintenance plans, and any additional optional services that may be required. oNot applicable - Contract covered it. The vendor be estimating this and/or discovering the costs as the machine has not been used to date. "In the solicitation it states that the "Contractor will supply carts, in sufficient quantity, for use in VAMC Albany to satisfy these collection requirements (carts need to be emptied and available for use by 6:00AM, 11:00AM and 2:00PM Monday thru Friday)." If Stericycle is purchasing carts for its use at the hospital, why do they have to be available during these times? I don't know why this is in there. Is the hospital planning on using them for other things unrelated to our service? No. Please clarify the intent of this requirement. This is for Sharps, correct? Yes "CARTS will not be provided as Government Furnished Equipment. "The solicitation states, "Contractor will be responsible for all testing on treated Medical Waste to insure all bio-burdens are rendered inert (in accordance with attached Standard Operating Procedure)." Please provided a detailed outline of the costs expected under this requirement, which should include, testing, ampules, other items required for validation and compliance with the hospital's SOP (one ampule per load run, etc.). oCosts are responsibility of respondant. "The solicitation states, "Parametric Controls: these include real time monitoring devices that record process feed rate or flow, cycle time and temperature/pressure. The devices that measure and record these processes must be calibrated annually by the manufacturer and recorded." Please provide the manufacture cost for annual calibration including all tests, onsite services, travel costs, etc. to be expected. oIt was part of the contract ¦ ¦we don't have this information. "The solicitation states, "Upon conclusion of the batch run, the ampule will labeled with the batch number and brought to the Microbiology Laboratory for incubation and analysis." Please confirm that the Microbiology lab will be providing this service at no charge to Stericycle. o Microbiology will still perform this service. "Will someone who has used the SSM-150 be available to provide onsite training? Is a video provided by the manufacturer or a detailed training manual? oAlbany VA will not be providing training. "The solicitation states, "Each lot of bio-indicators must be independently tested when received and after six (6) months and 200 hours of SSM-150 operation." Please provide the annual cost to utilize independent testing services as outlined in this agreement. oNo, Albany will not provide. Not previously documented. " Is the VA interested in receiving a quote for offsite incineration services? oNo "How often was the SSM-150 run in the last year. During this timeframe what percentage of time was it inoperable due to it being broken, etc.? oSSM-150 was used approximately ten times and was not inoperable during 2015, it was not used much in 2015. "What is the hospitals process for ensuring staff does not dispose of waste like cloth and metal into the containers destined for process in the SSM-150. oStandard procedures. "What are the total hours expected for Stericycle to be onsite providing technician services Monday through Friday? oThis is a contractor requirement to determine. "What department is responsible for swapping out sharps containers and moving regulated medical waste containers to the waste room when Stericycle is not onsite? oEmergencies only - EMS "Soiled Utility rooms- the RFP states "Contractor will be responsible for cleaning of carts as well as the soiled utility rooms daily. During our site visit we identified other vendors sharing/accessing this space. Does that mean we are responsible for cleaning up after other vendors? oNo, there are no other vendors. Stericycle are responsible for cleaning up their own spills.
