- Auxiliary Roman Catholic Priest Standard Form 18 Request for Quotation DoDI 1402.5, Background Checks on Indiviuals in DoD Child Care Services Programs THIS IS A COMBINED SYNOPSIS/SOLICITATION FOR COMMERCIAL ITEMS PREPARED IN ACCORDANCE WITH THE FORMAT IN FAR SUBPART 12.6, AS SUPPLEMENTED WITH ADDITIONAL INFORMATION INCLUDED IN THIS NOTICE. THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION; PROPOSALS ARE BEING REQUESTED AND A WRITTEN SOLICITATION WILL NOT BE ISSUED. This solicitation - FA5606-16-T-0006 is issued as a Request for Quotation (RFQ). Please submit your quotation NLT 18 December 2015 at 16:00 hrs. The requested services will augment the required Roman Catholic Priest Services at Geilenkirchen NATO Air Base Chapel. Services include the provision of an Auxiliary Roman Catholic Priest to provide weekend services and confessions. Weekend services include Roman Catholic Mass in coordination with the Group Chaplain. Confession sessions will be in conjunction with Sunday Mass as coordinated by the Group Chaplain. CONTRACT LINE ITEM NUMBER 0001 Roman Catholic Mass, in accordance with below Performance Work Statement QUANTITY: 30 UNIT OF ISSUE: Unit Period of Performance: 1 January 2016 - 31 December 2016 CONTRACT LINE ITEM NUMBER 0002 Confession, in accordance with below Performance Work Statement QUANTITY: 25 UNIT OF ISSUE: Unit Period of Performance: 1 January 2016 - 31 December 2016 CONTRACT LINE ITEM NUMBER 1001 (OPTION) Roman Catholic Mass, in accordance with below Performance Work Statement QUANTITY: 30 UNIT OF ISSUE: Unit Period of Performance: 1 January 2017 - 31 December 2017 CONTRACT LINE ITEM NUMBER 1002 (OPTION) Confession, in accordance with below Performance Work Statement QUANTITY: 25 UNIT OF ISSUE: Unit Period of Performance: 1 January 2017 - 31 December 2017 The solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular 2005-81. Please utilize attached Standard Form (SF) 18 document for quotation submission; Applicable Statement of Work, Solicitation Provisions, and Clauses are detailed within attached SF 18. Notice to Offeror(s)/Supplier(s): Funds are not presently available for this effort. No award will be made under this solicitation until funds are available. The Government reserves the right to cancel this solicitation, either before or after the closing date. In the event the Government cancels this solicitation, the Government has no obligation to reimburse an offeror for any costs. Auxiliary Roman Catholic Priest Position Statement of Work (SOW) FY16 Appropriated Funds 1. Contract Position Title. Auxiliary Roman Catholic Priest 2. Scope of Responsibilities. To provide weekend services and confessions. The Auxiliary Roman Catholic priest consults and coordinates with the assigned Group Chaplain and designated representative, who in turn monitors the contractor's work. 3. Education and Experience Requirement. The Auxiliary Roman Catholic Priest must: 3.1. In accordance with AFI 52-101, Auxiliary Roman Catholic Priest, must meet the same educational requirements as an Air Force Chaplain: possess a four year Bachelor's Degree from an accredited undergraduate college or university and 72 Semester hours or equivalent graduate level education from an accredited School of Theology/Ministry Preparation. 3.2. Meet an endorsement requirement: be approved in writing by the Archdiocese for the Military Services, USA. Documentation showing approval by the religious body must be submitted to the Geilenkirchen NATO Air Base Chapel and be maintained on file. 3.3. Be able to understand and speak English fluently. 3.4. Have at least 2 years experience as a priest and experience in military chapel setting. 3.5. Furnish a resume including academics and work related references. 4. Specific Responsibilities. The Auxiliary Roman Catholic Priest will: 4.1. Provide Roman Catholic Mass in coordination with the Group Chaplain. Each Mass will constitute one unit. Weekend/Holy Day masses will be limited to 30 units. 4.2. Conduct Confession sessions in conjuction with Sunday Mass. One Confession sessions will constitute one unit. Confession sessions will be limited to 25 units. 4.3. Units for Mass and Confession sessions will be in time increments lasting approximatley one hour (one hour for Confession and approximately one hour for Mass). To include, a 15 minute preparatory period prior to scheduled Mass or Confession. Preparatory periods will vary depending upon Mass and Confession scheduling. 4.4. Perform Roman Catholic baptism for qualified applicants that meet the Roman Catholic eligibility requirements. 4.5. Conduct no more than 30 Mass units and 25 Confession units per fiscal year at Geilenkirchen NATO AB Chapel. This contract is limited to 55 units. 4.6. Coordinate all Roman Catholic support requirements with the Geilenkirchen NATO AB Group Chaplain or designated representative. 4.7. Maintain the worship areas in a clean, orderly and safe manner. 4.8. The contractor will personally perform the services described above or coordinate performance by a substitute of equal or higher proficiency who has been approved in advance by the Geilenkirchen NATO Air Base Group Chaplain. Submit the names, qualifications, and letter of good standing of proposed subcontractors to the Geilenkirchen NATO Air Base Group Chaplain for approval at least 14 days prior to the proposed absence. Contractor is responsible for payment of substitute. 4.9. Be accountable for paying any Social Security payments, Federal or Local Income Taxes, and State Unemployment Insurance premiums for themselves, as well as any individual employed or subcontracted by them to provide services under this contract. 4.10. The contractor is accountable for security and cleanliness of all facilities utilized in conjunction with their programs. 5. The Quality Control Monitor (QCM). The Quality Control Monitor for this contract is the Group Chaplain. The QCM or his/her designee will monitor and inspect the contractor's performance. The contractor will be notified in writing of all noncompliance work, and given an opportunity to correct nonconforming work. The contractor will be given an opportunity to respond in writing to any notice of noncompliance. 6. The Group Chaplain. 6.1. Reviews the contract annually to consider exercise of option. 6.2. Ensure the unit rate of payment for services described in this SOW will be paid after the services are rendered. 6.3. Coordinate with the Base Security Office to process all necessary security checks required for the contractor to have access to the base and work with children. 7. Criminal History Background Check. Complete Child Care National Agency Check and Inquiries (CNACI), FBI Fingerprint Check and Installation Records Check (IRC) or host nation equivalent. Upon favorable completion of the IRC, contractor may perform duties pending completion and favorable adjudication of a submitted CNACI, provided they are within line-of-sight supervision of an individual with a favorably completed CNACI and IRC at all times. Contractors may not perform duties without supervision until favorable completion of a CNACI, FBI Fingerprint Check and IRC or host nation equivalent. Any negative results shall result in possible termination of the contract. A CNACI will be initiated and processed through local Security Forces Authorized Requestor office using AF Form 2583, Request for Personnel Security Action. 8. Exercise of Option: The contract may be extended for an additional 12 months by issuing written notice to the contractor within 30 days of contract expiration. The preliminary notice does not commit the contract to an extension. If the option is exercised, the extended contract is considered to include this option provision and at the applicable prices set forth in the bid schedule.
