This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation VA250-16-Q-0055 is issued as a request for quote (RFQ). The North American Industry Classification System (NAICS) Code 339113 Office Supplies. Business Size Standard of 500 employees is applicable. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-78. No telephone requests will be accepted. Quote is requested for: Line 1: 400100078 - Rapid SW License Kit, v8.0 Quantity: 1ea Line 2: 80138 - Rapid 8.0 License Quantity: 1ea Line 3: FGS-0540 - Rapid SW kit, v8.0 US Quantity: 1ea Line 4: 409990031 - PillCam recorder kit, DR3 Quantity: 4 ea Line 5: FGS-0347 PillCam recorder, DR3 Quantity: 4ea Line 6: FGS-0355 - PillCam recorder cradle, DR3 Quantity: 4ea Line 7: FGS-0362 PillCam recorder accessories, DR3 Quantity: 4ea Line 8: FGS-0427 - PillCam sensor array DR3, 8-lead Quantity: 4ea Items to be delivered to the Columbus VA Medical Center, Chalmers P. Evaluations - Award will be based on Best Value. Invoices shall be submitted in accordance with Federal Regulations 52.212-4(g) to VA Financial Services Center, P.O. Box 149971, Austin, Texas 78714-8971 through the OB10 electronic invoicing system. The provisions at FAR 52.212-1, Instructions to Offerors-Commercial Items, applies to this acquisition. Offerors shall submit with their offer, a completed copy of the provisions at FAR 52.212-3, Offeror Representations and Certifications-Commercial Items or confirm this has already been completed in their SAM registration. The clause at 52.212.4 Contract Terms and Conditions-Commercial Items, applies to this acquisition. The clause at 52.212-5, Contract Terms and Conditions Required to Implement Statues or Executive Orders-Commercial Items, applies to this acquisition which shall include the following clauses: 52.222-21, 52.222-26, 52.222-35, 52.222-36, 52.222-37, 52.232-34. Offerors shall submit their quotation by e-mail to Starla.Peach@va.gov. Offers are due no later than 4:00 pm. EST, December 17, 2015.
