- quote doc and clauses cover sheet This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Quotations are being requested. Solicitation AG-52B1-S-16-0006 is being issued as a Request for Quotation (RFQ), to provide local provenance plugs of native forbs, grasses, and sedges for the Midewin National Tall Grass Prairie, Wilmington, IL. Vendors may provide pricing for all or a portion of the Schedule of Items. Multiple contracts are anticipated. This acquisition is set-aside for small businesses. The North American Industry Classification System (NAICS) code is 111422 and the size standard is tiny_mce_marker.75mil. All plants/plugs must be received at Midewin no later than May 6, 2016. Quotes will be submitted to: Roger Manning, USDA Forest Service, 811 Constitution Ave, Bedford, IN 47421. NO FAX QUOTES WILL BE ACCEPTED. E-mail quotes are preferable at rkmanning@fs.fed.us. Quotes must include vendor's TIN (Tax Identification Number) and DUNS number. Vendors must be registered in SAMS (www.sams.gov) Quotes must include a completed copy of (FAR 52.212-3 Offeror Representations and Certifications, Commercial Items, paragraphs (c) through (m). If the Offeror has completed the annual representations and certifications electronically at http://orca.bpn. gov., submit only paragraph (b) from the provisions and a DUNS number. (FAR) 52.212-2 Evaluation, Commercial. Quotes are due January 6, 2016 at 11:00a.m. Eastern Time.
