Description
This is a SOURCES SOUGHT ANNOUNCEMENT ONLY. It is neither a solicitation announcement nor a request for proposals or quotes and does not obligate the Government to award a contract. Requests for a solicitation will not receive a response. Responses to this Sources Sought must be in writing. The purpose of this Sources Sought Announcement is for market research to make appropriate acquisition decisions and to gain knowledge of potential qualified Service Disabled Veteran Owned Small Businesses, Veteran Owned Small Businesses, 8(a), Hub-Zone, other Small Businesses, or large businesses interested and capable of providing items requested. Responses to this notice shall include the following: (a) company name b) address (c) point of contact (d) phone, fax, and email (e) DUNS number (f) Cage Code (g) Tax ID Number (h) Type of business, e.g. Services Disabled Veteran Owned small Business, Veteran-owned small business, 8(a), HUB-Zone, Women Owned Small Business, Small disadvantaged business, or Small Business HUB-Zone business and (i) must provide a capability statement that addresses the organizations qualifications and ability to perform as a contractor for the work described below. Description: The Government is interested in gaining knowledge of potential businesses that would be able to provide a safe and sober living facility for eligible Veterans involved in outpatient substance abuse treatment with the Addiction Disorder Services program at the Grand Junction VA Medical Center. Potential contractors must meet the requirements listed in the following Statement of Work: Statement of Work Access to Care Requirements The temporary lodging facility must comply with the following access to care components: 1.The facility must be able to provide lodging for Veterans within three (3) days of acceptance by the facility. 2.The facility must provide be able to provide a minimum of five (5) beds which are designated for priority use by Veterans. 3.The facility must provide lodging services to male and female Veterans. 4.Beds designated for Veteran use may be utilized by non-Veterans if the number of Veterans residing at the facility is less than five at that time. Housing Requirements The facility must comply with the following housing requirements: 1.Maximum occupancy of the facility will not exceed the number allowed by the local or county Zoning Department. 2.Veterans' housing will be based on individual needs. This may be in the general housing section or in a designated Veterans' section if provided by the contracting facility. 3.The facility shall have female rooms or sections that are securely segregated or restricted from men to ensure safety and privacy. 4.Men and women shall have separate bathroom facilities. 5.Bedding, pillows, and towels will be provided to all Veterans. 6.Laundry facilities which will be available to all Veterans. Patient Placement Placement into the facility through the VA requires all of the following elements: 1.The Veteran must be eligible for care through VHA. 2.GJVHCS ADS staff must have documented that it is believed the Veteran's treatment would be aided by placement in a safe and sober living environment. 3.The Veteran must concur with placement in the temporary lodging facility. 4.The Veteran must agree to comply with the rules/guidelines of the facility in which he/she is placed. 5.Prior to acceptance, staff from the temporary lodging facility must meet with the Veteran to determine if the facility is capable of meeting the Veteran's individual needs and if the Veteran is likely to benefit from housing at the facility. Environmental and Safety Requirements The facility must comply with all of the following environment requirements: 1.The facility must successfully complete a Site Safety Code Inspection conducted by the GJVHCS Safety Officer prior to implementation of this contract and once a year thereafter prior to exercising any option. 2.Acceptable safety and sanitation practices shall be observed throughout the facility. The building shall conform to the standards of the Life Safety Code (National Fire Protection Association Standard #101) in effect on the date of contract award. 3.The facility warrants that all applicable fire laws (City, State, & Federal) are being complied with and there are no unresolved recommendations that have been made by fire officials that have not been resolved. 4.New Veterans will be oriented to Fire and Safety Policies and Procedures within 5 days of taking residence in the facility. Veterans must be instructed in evacuation procedures when the primary and/or secondary exits are blocked. 5.The facility will comply with all fire codes and pass a fire inspection conducted by the fire department where it is located at least twice per year. The contractor shall notify the VA immediately if the facility fails to pass any fire inspection. 6.The facility will conduct and document fire drills at least every two months. 7.The facility will comply with all local zoning requirements and must pass annual inspections by the local zoning department. Food Safety and Meal Requirements The facility must comply with the following meal and food requirements: 1.The facility will comply with all local food safety standards and must pass all required inspections by the local or County Health Department. The contractor shall notify the VA immediately if the facility fails to pass any food or health inspections. 2.All Veterans will be provided three nutritional meals per day. 3.Veterans with special dietary needs will be discussed with the facility prior to admission to determine if the facility is capable of meeting the Veteran's individual needs. Facility Staffing Requirements The facility must comply with the following staffing requirements: 1.There will be a minimum of one staff member present at the facility 24 hours per day, 365 days per year for Veterans to contact in case of emergency. Location and Transportation Requirements The facility must comply with the following transportation requirements: 1.The facility must be located within a ten (10) mile radius of the main GJVHCS campus. 2.The facility must either: a) be located within one-quarter mile of a public bus transit line which provides service to GJVHCS, or b) provide transportation for Veterans to and from the facility for all scheduled appointments at the GJVHCS main campus. Medical Treatment 1.Veterans' medical needs will typically be handled by the medical staff of the GJVHCS. However, in an emergency the facility staff will utilize 911 and/or the nearest medical facility to get the Veteran any necessary medical treatment. 2.The facility staff will provide non-emergent transportation to GJVHCS when necessary for medical appointments if the use of public transportation is not feasible for the Veteran's condition. Patient Confidentiality Requirements 1.The facility must comply with all state and federal laws pertaining to the confidentiality of patient communications. These laws include but are not limited to the provisions of Public Law 104-191, Health Insurance Portability and Accountability Act (HIPAA) of 1996 and the National Standards to protect the privacy and security of Protected Health Information (PHI). 2.Other than as provided by law, the facility must have a current, written, signed authorization to release information in place before releasing any Veteran information to any party. The facility will immediately notify GJVHCS ADS staff of any breach of confidentiality. Billing Procedures 1.The facility will be paid on a per Veteran per night basis. 2.The facility must receive written confirmation from GJVHCS fiscal office for every Veteran admission. 3.Each Veteran must sign in on the VA census sheet for himself/herself on a daily basis. 4.The facility must submit monthly invoices in a secure fashion as directed by VHA. Patient Discharge 1.Other than in situations where immediate discharge of a Veteran is necessary due to the safety of the Veteran or others, no Veteran will be discharged from the facility without prior notice to GJVHCS ADS staff. 2.Facility staff will notify GJVHCS ADS staff within two hours of a Veteran permanently leaving the facility prior to the anticipated date. In the event this occurs after hours or on weekends, facility staff will notify GJVHCS ADS staff by 9:00 am of the next business day. The following elements are not required but are viewed as desirable: 1.The VHA believes in providing care in the least restrictive environment possible that will meet the Veteran's treatment needs. It is preferred that the facility be non-institutional and as "home-like" as possible. 2.The availability of staff members at the facility who have qualifications and/or experience working with clients with substance use disorders will enhance the desirability of entering into a contract with the facility. 3.The availability of substance abuse treatment activities at the lodging facility, including but not limited to AA, will enhance the desirability of entering into a contract with the facility. This treatment would need to be consistent with the Veteran's expressed treatment needs and preferences, and integrated into the Veteran's ADS treatment plan. Any such treatment would be an adjunct to the outpatient SA treatment being provided by GJVHCS and must be approved in advance by ADS staff. 4.The availability of attending religious services of the Veteran's choosing would enhance the desirability of entering into a contract with the facility. However, in no case will participation in religious services be required of any Veteran. 5.The availability of facility staff to meet with the Veteran and GJVHCS ADS staff at the VA to coordinate housing needs and treatment activities will enhance the desirability of entering into a contract with the facility. 6.The availability of services to assist Veterans with individual needs upon leaving the facility will enhance the desirability of entering into a contract with the facility. These needs may include but are not limited to: finding permanent housing, finding employment, providing ongoing support for recovery, and connecting with other resources in the community. [END] The Government is not obligated to nor will it pay for or reimburse any costs associated with responding to this sources sought synopsis request. This notice shall not be construed as a commitment by the Government to issue a solicitation or ultimately award a contract, nor does it restrict the Government to a particular acquisition approach. The Government will in no way be bound to this information if any solicitation is issued. The North American Classification System (NAICS) code for this acquisition is 339112 (size standard to be considered small business is 500 employees). Notice to Potential Offerors: All Offerors who provide goods or services to the United States Federal Government must be registered in the System Award Management (SAM located on the web at www.sam.gov). It is desirable that any Offeror to have completed their business Online Representations and Certifications Application (ORCA). Additionally, all Service Disabled Veteran Owned Businesses or Veteran Owned Businesses who respond to a solicitation on this project must be registered with the Department of Veterans Affairs Center for Veterans Enterprise VetBiz Registry located at http://vip.vetbiz.gov.
