Description
1.The purpose of this amendment is to correct a system error in procurement type posting to FBO. Original FBO posting was listed as a Small Business set aside. Contracting Officer selected Service Disabled Veteran Small Business Set Aside (SDVOSB) in eCMS but that selection was not reflected in FBO. This amendment effectively clarifies the set aside type to SDVOSB. 2.The original closing date and time was set for November 25, 2015 at 11:00am. This amendment changes closing date and time to November 27, 2015 at 11:00am. All questions and concerns must be submitted no later than that time. 3.Vendor questions and concerns are answered below. 3.1.In the Synopsis page 2 it states "Contractor shall provide a Certified Respiratory Therapists (CRT), a Registered Respiratory Therapist (RRT) or Registered Nurse (RN) during home visits when a new patient is enrolled into the VISN 8 Home Respiratory Care Program." 3.1.1There will be no education for this contract, it is for delivery only. 3.1Page 7 SOW 1.3 states equipment is to be no more than 6 months old. Can this be revised to read Equipment must be in good working order and meet the prescribing physician's criteria? 3.2.1Yes, equipment must be in good working condition. 3.3Page 7 B.1- Secures 2 portable liquid systems with canellas, carrying bag or portable transport cart. HRCT currently provides all soft goods including cannulas. Will the HRCT continue providing or will HRCT provide these items to Contractor? 3.3.1VA supplies all the soft goods to the contractor Cannula. 3.4Page 2 of 102 states: "Contractor shall perform in accordance of PWS to include: Contractor shall provide a Certified Respiratory Therapists (CRT), a Registered Respiratory Therapist (RRT) or Registered Nurse (RN) during home visits when a new patient is enrolled into the VISN 8 Home Respiratory Care Program." Question: In the Statement of Work pages 7 through 21, there is no mention of a requirement to provide Certified Respiratory Therapists (CRT), a Registered Respiratory Therapist (RRT) or Registered Nurse (RN)s. As a matter of fact, there isn't even any language discussing the contractors role when a new patient is enrolled into the VISN 8 Home Respiratory Care Program. Who is responsible for a new patient set-up, the VA or the contractor? Are contractors required to perform new patient set-ups with a Certified Respiratory Therapists (CRT), a Registered Respiratory Therapist (RRT) or Registered Nurse (RN)? 3.4.1Contractor will not have to send out CRT it is the responsibility of the VA, Contractor is for delivery only and set up, CRT will educate veteran on equipment. 3.5Page 18, Section 19 of the SOW: "19. EMPLOYEE QUALIFICATIONS AND CERTIFICAITONS: Contractor shall have the appraisals license or certificate required to perform under this contract. Copies of certificates or licenses shall be submitted to CO at time of award. Question: I have never heard of an appraisals license. What is an appraisals license? 3.5.1Omit need for appraisal license. It was added in error and should not be a consideration factor. 3.6EVALUATION FACTORS: Page 59 states: The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government. Question: Please clarify which evaluation factor for Award will be used for this solicitation. Is it Lowest Priced Technically Acceptable? Or Most Advantageous to the government? 3.6.1In this case, most advantageous to the government is lowest price technically acceptable. Thus, LPTA will be the qualifying factor in awarding to successful bidder. 3.7Sample patient education materials given to the veteran/family at set up of Home Oxygen, documentation regarding reeducation needs of veterans/families and any other information to demonstrate capabilities in this area. The sample educational materials provided must cover compressed gas, liquid 02 and ventilators. Question: According to the SOW and the price/cost schedule, there is no mention of ventilators on this contract. Please clarify if we need to provide sample educational materials covering ventilators. 3.7.1No ventilators in this contract it is the responsibility of the VA to issue and educate not contractor. 3.8Documentation detailing and demonstrating the experience and knowledge of company's management personnel in successfully providing Home Oxygen equipment, supplies and services to a minimum of 250 veterans monthly. This includes patients with gas O2, liquid O2, and ventilators. Question: According to the SOW and the price/cost schedule, there is no mention of ventilators on this contract. Please clarify if we need to provide documentation detailing and demonstrating the experience and knowledge of company's management personnel in successfully providing ventilators. 3.8.1No ventilators in this contract it is the responsibility of the VA to issue and educate not contractor. 3.9Documentation of company's organizational and corporate structure that demonstrates the company's ability to meet the requirements of the solicitation. Include documentation regarding the processes for complex patient set ups including but not limited to new ventilator patients and chronic ventilator patients. Question: According to the SOW and the price/cost schedule, there is no mention of ventilators on this contract. Please clarify if we need to provide documentation regarding the processes for complex patient set ups including but not limited to new ventilator patients and chronic ventilator patients. 3.9.1No education, it is the responsibility of the VA CRT to educate.
