Description
This is a Combined Synopsis/Solicitation for commercial items prepared in accordance with the format in FAR 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only Solicitation; quotes are being requested and a written Solicitation will not be issued. Solicitation number VA249-16-Q-0122 is issued as a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular. This requirement is a Small Business Set-Aside. The NAICS is 238210 and the small business size standard is $15.0 Million. Introduction: The government anticipates awarding a Firm Fixed Price award, lowest price technically acceptable (LPTA). The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforms to the solicitation requirements. Description of Services: The Lexington VAMC has a requirement for a contractor to provide an "18" LED letter sign on raceway that states "AMBULANCE ENTRANCE ONLY". In addition, the contractor shall provide all wiring and mounting material. The contractor shall install the sign but not electrical power to the sign and shall obtain all necessary and required permits to carry out this work. Within five (5) days of the Contractor receiving the Notice of Award, the work shall begin. The contractor shall perform to the standards in the contract as well as all local, state, and federal regulations. See the Statement of Work (SOW) for complete details. Full text can be obtained at http://www.acquisition.gov/far/index.html Submission of Quote: The offeror shall shall submit their quote on company letterhead and shall include unit price, total, unit qty and item description as specified above, as offered discounts, proposed delivery time, name, address, and telephone number of the offeror, firm's DUNS# and ORCA document in SAM at www.sam.gov, terms of any express warranty, unit price, overall total price, applicable shipping charges, completed copy of 52.212-3 Offerors Representations and Certifications- Commercial Items, and ORCA document. All prospective bidders must include appropriate references which must include all applicable company information. The offeror should provide past performance evidence. The past performance evaluation results is an assessment of the offeror's probability of meeting the solicitation requirements. This assessment is based on the offeror's record of relevant and recent past performance information that pertain to the products and/or services outlined in the solicitation requirements. Past performance information may be obtained through other sources known to the VA and the Federal Government. In the case of an offeror without a record of relevant past performance or for whom information on past performance is not available or so sparse that no meaningful past performance rating can be reasonably assigned, the offeror may not be evaluated favorably or unfavorably on past. Therefore, the offeror shall be determined to have unknown past performance. In the context of acceptability/unacceptability "unknown" shall be considered "acceptable." STATEMENT OF WORK PART A - GENERAL INFORMATION A.1 INTRODUCTION - This requirement is for signage for the Cooper Division Ambulance Entrance A.2 BACKGROUND -The current Ambulance Entrance has no signage. This is causing traffic issues with pedestrian cars and ambulances at (VAMC, 1101 Veterans Drive, Lexington, KY 40502). A.3 SCOPE OF WORK - The Contractor shall furnish all materials and install signage at VAMC 1101 Veterans Drive Lexington, KY 40502 STATEMENT OF WORK PART B - WORK REQUIREMENTS B.1 Tasks to be accomplished: "The contractor shall create Red LED 18" lettering that states AMBULANCE ENTRANCE ONLY "The contractor shall install and mount lettering at VAMC Cooper Division at the Ambulance Bay "The contractor shall supply wiring material to VAMC Cooper Division "Contractor shall NOT supply power to signage "VAMC will supply power to signage "The contractor shall ensure that while all workers are wearing proper Personal protective equipment (PPE )at all times during the performance of any and all work performed under this contract "The contractor shall obtain all necessary and mandated permits to carry out this work "The contractor shall ensure that only FULLY QUALIFIED TECHNICIANS, WORKERS, ETC. are employed in the performance of any and all work performed under this contract "The contractor shall ensure that the highest standard of professional capability and workmanship is maintained throughout the life of this contract Additional tasks: "Contractor shall deliver all materials to VA medical center, 1101 Veterans Drive Lexington Kentucky 40502 STATEMENT OF WORK PART C - SUPPORTING INFORMATION "Work at the government site shall not take place on Federal holidays or weekends unless specifically directed by the Contracting Officer in the Contract. All work in areas involved shall be scheduled with the VA Engineering Service representative. Work shall be scheduled for proper execution to completion of contract. "HOURS OF OPERATION The contractor will perform services required during the hours of 8:00 am - 4:00 pm, Monday through Friday or as directed by the designated VAMC Lexington Point of Contact. "U.S LEGAL HOLIDAYS The following is a list of U.S. Government holidays. If the holiday falls on a Saturday, the proceeding Friday is observed as the holiday; if the holiday falls on a Sunday, the following Monday is observed as the holiday. The Contracting Officer's Representative will be identified in the task order by name, telephone number and location. Inspections made by the COR are for the sole benefit of the Government and do not relieve the Contractor of any quality control responsibilities. "The Safety Manger is responsible for safety inspections of all contract operations. The Safety Manager will be identified in the task order by name, phone number and location. "Verification of Dimensions - The Contractor shall visit the job site to thoroughly familiarize their self with all the details of the work and working conditions. The Contractor shall also verify all dimensions in the field and shall advise the Contracting Officer of any discrepancy before performing any work. The Contractor shall be specifically responsible for the coordination and proper relation of his work to the building, structure, phasing and ensure the safety of employees and workmen. "Parking/Traffic Regulations. The Contractor shall not park on grassy areas unless approved by the COR or Lexington Point of Contact and the Contractor agrees to restore areas back to VA standards. Drivers should be particularly concerned with pedestrian traffic. Yield to pedestrians in crosswalks. Posted speed limits and all other traffic controls are to be observed by operators at all times. Seat belt use is mandatory on the VA grounds. "The contractor/vendor shall at all times, ensure that all work provided complies with all local, State, and Federal rules and regulations pertaining to work place safety. The contractor shall take all proper safety and health precautions to protect the work, the workers, the public and the property. "The contractor shall furnish to the COR or Lexington Point of Contact all service documentation upon the completion of all work. "The contractor's employees shall check in and out with the VA Police obtain visitor identification each day. Employees must have a valid state or government identification. All contractor/vendor personnel shall display a VAMC Memphis issued identification badge while performing work on any VAMC property. "Smoking is not prohibited on property except in designated areas. Possession of weapons is prohibited (in vehicle or on person). Enclosed containers, including tool kits, shall be subject to search. Violations of the VA regulations may result in citation answerable in the United States (Federal) District Court. "Contractor employees (crew) when on VAMC Lexington grounds; shall not bring visitors into the facility; shall not bring into the facility, any form of weapons or contraband; shall not bring in any alcohol or drugs or be under the influence of alcohol/drugs; shall conduct themselves in a professional and polite manner at all times; shall not cause any disturbances in the facility, are subject to all the rules and regulations of the VAMC Lexington. Contractor personnel shall present a neat appearance and be easily recognized. Contractor shall furnish and require every on-site company employee to wear a company shirt and safety shoes. All company workers while on duty shall be identified by uniform shirt or blouse indicating the company logo. The print should be large enough to be easily read. In addition all company staff assigned to work at the VAMC Lexington. " All work shall be performed in accordance with VAMC Lexington rules & regulations. CHANGES TO STATEMENT OF WORK "Any changes to this SOW shall be authorized and approved only through written correspondence from the Contracting Officer. A copy of each change will be kept in a project folder along with all other products of the project. Costs incurred by the contractor through the actions of parties other than the Contracting Officer shall be borne by the contractor. CONTRACTOR PERSONNEL SECURITY REQUIREMENTS Contractor will not have access to VA computer system. MATERIAL AND WORKMANSHIP (a) All equipment, material, and articles incorporated into the work covered by this contract shall be new and of the most suitable grade for the purpose intended, unless otherwise specifically provided in this contract. References in the specifications to equipment, material, articles, or patented processes by trade name, make, or catalog number, shall be regarded as establishing a standard of quality and shall not be construed as limiting competition. The Contractor may, at its option, use any equipment, material, article, or process that, in the judgment of the Contracting Officer, is equal to that named in the specifications, unless otherwise specifically provided in this contract. (b) The Contractor shall obtain the Contracting Officer's approval of the machinery and mechanical and other equipment to be incorporated into the work. When requesting approval, the Contractor shall furnish to the Contracting Officer the name of the manufacturer, the model number, and other information concerning the performance, capacity, nature, and rating of the machinery and mechanical and other equipment. When required by this contract or by the Contracting Officer, the Contractor shall also obtain the Contracting Officer's approval of the material or articles which the Contractor contemplates incorporating into the work. When requesting approval, the Contractor shall provide full information concerning the material or articles. When directed to do so, the Contractor shall submit samples for approval at the Contractor's expense, with all shipping charges prepaid. Machinery, equipment, material, and articles that do not have the required approval shall be installed or used at the risk of subsequent rejection. (c) All work under this contract shall be performed in a skillful and workmanlike manner. The Contracting Officer may require, in writing, that the Contractor remove from the work any employee the Contracting Officer deems incompetent, careless, or otherwise objectionable. PERMITS AND RESPONSIBILITIES THE CONTRACTOR SHALL, WITHOUT ADDITIONAL EXPENSE TO THE GOVERNMENT, BE RESPONSIBLE FOR OBTAINING ANY NECESSARY LICENSES AND PERMITS, AND FOR COMPLYING WITH ANY FEDERAL, STATE, AND MUNICIPAL LAWS, CODES, AND REGULATIONS APPLICABLE TO THE PERFORMANCE OF THE WORK. THE CONTRACTOR SHALL ALSO BE RESPONSIBLE FOR ALL DAMAGES TO PERSONS OR PROPERTY THAT OCCURS AS A RESULT OF THE CONTRACTOR'S FAULT OR NEGLIGENCE. THE CONTRACTOR SHALL ALSO BE RESPONSIBLE FOR ALL MATERIALS DELIVERED AND WORK PERFORMED UNTIL COMPLETION AND ACCEPTANCE OF THE ENTIRE WORK, EXCEPT FOR ANY COMPLETED UNIT OF WORK WHICH MAY HAVE BEEN ACCEPTED UNDER THE CONTRACT. PROTECTION OF EXISTING VEGETATION, STRUCTURES, EQUIPMENT, UTILITIES, AND IMPROVEMENTS (a) The Contractor shall preserve and protect all structures, equipment, and vegetation (such as trees, shrubs, and grass) on or adjacent to the work sites, which are not to be removed and which do not unreasonably interfere with the work required under this contract. The Contractor shall only remove trees when specifically authorized to do so, and shall avoid damaging vegetation that will remain in place. If any limbs or branches of trees are broken during contract performance, or by the careless operation of equipment, or by workmen, the Contractor shall trim those limbs or branches with a clean cut and paint the cut with a tree-pruning compound as directed by the Contracting Officer. (b) The Contractor shall protect from damage all existing improvements and utilities (1) at or near the work site, and (2) on adjacent property of a third party, the locations of which are made known to or should be known by the Contractor. The Contractor shall repair any damage to those facilities, including those that are the property of a third party, resulting from failure to comply with the requirements of this contract or failure to exercise reasonable care in performing the work. If the Contractor fails or refuses to repair the damage promptly, the Contracting Officer may have the necessary work performed and charge the cost to the Contractor. CLEANING UP The Contractor shall at all times keep the work area, including storage areas, free from accumulations of waste materials. Before completing the work, the Contractor shall remove from the work and premises any rubbish, tools, scaffolding, equipment, and materials that are not the property of the Government. Upon completing the work, the Contractor shall leave the work area in a clean, neat, and orderly condition satisfactory to the Contracting Officer or the COR or Lexington Point of Contact.
