COMBINED SYNOPSIS/SOLICIATION VA246-16-Q-0187 Cardiac Loop Monitoring Service (I)This is a combined synopsis/solicitation for commercial service prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation for this action. Quotes are being requested, and a written solicitation will not be issued. (II)This synopsis/solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-84. (III)Any amendments issued to this solicitation will ONLY be available on the FBO website (www.fbo.gov). (IV)The Network Contracting Office 6-SAO EAST intends to award a firm-fixed price service contract for one year of monthly monitoring service at the Durham Department of Veterans Affairs Medical Center (VAMC), 508 Fulton Street, Durham, NC 27705. See Statement of work below for details of the service requested. (V)The North American Industrial Classification System (NAICS) code for this procurement is 334510 with a small business size standard of 500. This procurement is set-aside to small businesses and all eligible offerors may submit quotes. All offerors must be registered in System for Award Management (SAM) at http://www.sam.gov to be considered for an award of a federal contract. The record must be complete and not missing elements such as representations and certifications. (VI)Questions concerning this solicitation shall be addressed to Melisha Evans, Contracting Officer, and emailed to melisha.evans@va.gov. All questions or inquires must be submitted no later than 9:00AM Eastern Standard time on November 27, 2015. No phone calls will be accepted. (VII)The Government will award a contract resulting from this RFQ to the responsible offeror whose quote is the lowest price and meets the requirements. Quotes are due by 8:00 AM Eastern Standard time on an November 30, 2015. Quotes received after this date and time will be considered late in accordance with 52.212-1(f) and will not be evaluated. Quotes shall be submitted via email to Melisha.Evans@va.gov. (VIII)This combined solicitation/synopsis is for commercial services. Statement of Work (SOW): Looping monitors continuously records the heart rhythm and automatically saves the pre-event portion of the ECG while continuing to record its post-event portion. Its unique algorithms detect and automatically record asymptomatic events. The Durham VAMC is requesting monthly service for 20 monitors with memory loop and mobile outpatient (continuous) telemetry monitoring through an external vendor. The actual request is for the monitoring service which includes supplying the monitor, monitoring the data as it is received, and generating a report when the test is completed. Salient Characteristics Loop Auto-Trigger Monitor features would include: "Simple one-button recording for symptomatic events "Programmable triggers "10 minutes of programmable pre and post event memory "Date and time stamped "Auto-trigger and manual send "Auto-trigger of AF, bradycardia and tachycardia "Event monitoring for up to 30days Symptomatic/asymptomatic "AF "Tachycardia "Bradycardia "Patient-activated symptomatic events List of Programming Functions "Number of seconds of pre-event memory captured "Number of seconds of post-event memory to be captured "Maximum number of events that can be stored "Speaker emits an audible tone/heartbeat during recording "Heart rate is displayed in the status window "Controls atrial fibrillation trigger "Controls automatic rate trigger "Set minimum heart rate for tachycardia "Set maximum heart rate for bradycardia "Auto trigger recorded event alert "Number of events that may be recorded by the automatic rate trigger Specification requirements: "Vendor must be a Certified Diagnostic Testing Facility. (CMS requirement) "Joint commission accreditation preferred. "Vendor must provide 24 hour a day service to receive patient transmitted data, perform a preliminary interpretation, and notify the physician of potentially life threatening arrhythmias. (CMS requirement and current standard of care). "Vendor must provide 24 hour customer support. "Vendor must demonstrate an internal Quality Assurance program. "Vendor will participate in a collaborative Quality Assurance program. "Vendor will supply the monitors to the facility which will be tested by the vendor prior to each patient application. "Loop monitors will have configurable memory that can be reprogrammed to meet patient specific needs. "Monitors will transmit data by cellular technology as well as by land line. "Data will be available to the facility VIA an internet application that complies with VA information security policies. Training: Vendor will provide end user staff training on proper use. User and Service Manuals: Vendor will provide 2 complete printed sets as well as electronic version of operator manuals.
