Description
STATEMENT OF WORK (SOW) 2. Contract Title. Digisonics software maintenance and repair service Prima Support for multiple software applications. 3. Background. The New Mexico VA Healthcare System (NMVAHCS) is requesting software maintenance and repair service from Digisonics for Prima Support for multiple software applications. This service contract is required to ensure that the NMVAHCS, Vascular Lab, receives timely and cost efficient service support from Digisonics. The Digisonics service contract is for October 1, 2015 to September 30, 2016 plus 4 option year renewals. 4. Scope. The contract for renewal of Prima Support for the following software applications: "One DigiView Digital floating w/ DigiNet Pro upgrade (1 concurrent user/ 2 installs) "One DigiView Digital "One Hardware for DigiView Digital "Two Vascular reading stations with three hardware "One DICOM Gateway "Three Datalinks (2 DSR) "Two Links for Parks Flo-Lab "One Search "Two HL7 (OrIn, RsOt) "One Report-to-PACS "One DICOM Server Q/R Serial Number: DV-493 Remote Connectivity: Remote Connectivity is required for warranty and extended support. Efficient support requires that Digisonics be able to collaborate with end users on the same screen. Therefore Digisonics requires remote console access such as VNC, PC Anywhere, Radmin, or Remote Assistance. Tools such as RDP require considerable time from both Digisonics and site personnel and therefore are not acceptable. 5. Specific Tasks. The contract for renewal of Prima Support for the following software applications: "One DigiView Digital floating w/ DigiNet Pro upgrade (1 concurrent user/ 2 installs) "One DigiView Digital "Two Vascular reading stations "One DICOM Gateway "Five Datalinks (2 DSR, 2 Parks Flo-Lab) "One Search "Two HL7 (OrIn, RsOt) "One Report-to-PACS "One DICOM Server Q/R Serial Number: DV-493 Prima Support includes the following: 1.Support for covered items (software only) 2.Software upgrades 3.System-level consulting for system problems associated with the licensed software 4.Custom table changes for modifying clinical comments and reports 5.Auto-escalation of all unresolved issues after 5 business days 6.Participation in software development surveys 7.Support access through a toll-free number, M-F 7:00am-7:00pm CST Details of Prima Support for DV-493 include: Supported Hardware Prima 1 DigiView Digital Workstation Supported Software Prima 1 DigiView digital float w/ DigiNet Pro upgrade (1 CU/2 installs) 1 DigiView digital 2 Vascular Reading Stations 1 DICOM Gateway 5 DataLinks (2 DSR, 2 Parks Flo-Lab) 1 Search 2 HL7 (OrIn, RsOt) 1 Report-to-PACS 1 DICOM Query/Retrieve Software Upgrades Digisonics software upgrades are included with Prima Support contracts. Area Digisonics responsibility Site responsibility Software Support of Digisonics software as listed in the support contract. This includes answering questions on the software, bug fixes, and software upgrades as included in the warranty or support contract. System software Anti-virus software, anti-adware software. Database support & corruption/issues Database administration and support. Database corruption or damage caused by customer action or intervention not approved by Digisonics in writing. Network Digisonics consults on system /network problems, but it is a site responsibility to correct network problems. Network & other components supplied by the site. It is a site responsibility to supply a network with adequate bandwidth & stability. Hardware & Database upgrades Installation services for replacement hardware and database upgrades/conversions are not included in support contracts - Services are available for purchase. Hardware Hardware is not supported by Digisonics unless explicitly listed in the support contract. In those cases, support is limited to return to factory exchange except for on-site support provided by the hardware vendor. Hardware is normally a site responsibility. The site is also responsible for installation of replacement parts sent by Digisonics (VCR, MO drive, etc.) and communication with the hardware vendor to run requested tests and/or schedule on site visits. OS patches & upgrades OS changes affect many applications. Because of this, virtually all sites want to control when these upgrades are done. Site must consult with Digisonics. Link upgrades Upgrade to a different make/model ultrasound not included in support contracts requires purchase. Troubleshooting Remote tests to determine the cause of a failure or whether a problem is due to software, a workstation component, or a network component. Local tests as directed by Digisonics or the hardware manufacturer to determine the cause of a problem. Examples are: timing a copy outside the Digisonics application to determine network performance, switching cables between systems, switching a mouse or monitor. Installation & Training Initial installation, application configuration & training, provided these are included in the original purchase order. Reconfiguration (for new projects). Additional training. Moving software to new systems after the initial installation. Installing software upgrades. Optional service for these can be purchased from Digisonics. Hours Support within the hours specified in the support contract. Support outside the hours in the support contract. Site administration User accounts & user rights. Network paths. Site configuration options. Managing storage space including changing paths and/or copying images to a different location when a site runs out of space. Managing back-ups and disaster recovery. HOURS OF COVERAGE: A.Digisonic Support hours of coverage are Monday through Friday 7am to 7pm CST. Service will be performed during normal hours of coverage unless requested or approved by COR. B.Federal Holidays observed by the VAMC are: New Years' DayLabor Day Martin Luther King DayColumbus Day Presidents' DayVeterans' Day Memorial DayThanksgiving Day Independence DayChristmas Day 6. Performance Monitoring Vendor performance will be monitored by Biomedical Engineering, the COR, and using service to ensure conformance to stated contract requirements and expectations. Work completed will be reported to the COR to ensure that contractual conditions are met. 7. Place of Performance. Service and maintenance work will be completed on site at the New Mexico VA Health Care System 8. Period of Performance. Vendor will provide service for the period December 1, 2015 through November 30, 2016, on Digisonic equipment at NMVAHCS. Additionally there will be four (4) one-year option periods.
