Attachment A - Past Performance Questionnaire The U.S. Army Corps of Engineers Rock Island District is seeking System for Award Management (SAM) Database registered Small Businesses that can provide the following: Mississippi River, Huron Island, Des Moines County, IA, Huron Island Complex, Stage II This announcement is for information and planning purposes only and is not to be construed as a commitment by the Government, implied or otherwise, to issue a solicitation or award a contract. NO AWARD will be made from this Sources Sought Notice. This is NOT a request for proposal or quote or an invitation for bid. Particularly, the purpose of this NOTICE is to gain knowledge of potential System for Award Management (SAM) Database registered businesses interested and capable of performing this work. DESCRIPTION OF WORK: Work includes, but is not limited to: Clearing and grubbing, including mature trees. Earth work such as shaping previously dredged material to specific slopes and elevations. Earth work including excavation of existing wet soils and constructing a berm to specific slopes and elevations. Rock structure construction in water and on land. Bankline rock protection. Tree planting, including trees which will need up to a 3 year lead time. Vegetation planting, including wetland plants, grasses, and native shrubs. All work will be within the river, subject to water fluctuations. There is no land access to this project. All work will require river access. SOURCES SOUGHT: NAICS CODE: 237990 SIZE STANDARD: $36.5 Million CONTRACT ESTIMATED RANGE: $1-$5 Million POINT OF CONTACT: Responses are to be sent via email to Deputy for Small Business Programs, John Dengler, at john.e.dengler@usace.army.mil no later than 12:00 pm Central Standard Time, December, 11 2015. SUBMISSION DETAILS: All interested, responsive and responsible System for Award Management (SAM) Database registered small businesses are encouraged to participate in this market research by emailing the required information below to the point of contact listed above. Interested businesses must be registered in the System for Award Management (SAM) Database at www.sam.gov. Email submissions are required. Interested businesses shall include the following information to be considered for market research purposes. 1.Company name, address, point of contact, phone number, email address and cage code 2.Capabilities Statement indicating the contractor can meet the requirements within specified scope. 3.Company's interest in bidding/proposing on the solicitation when issued. 4.Company's business category and size. 5.Company's capability to perform a contract of this magnitude and complexity, include comparable work performed to include: Brief description of the project, customer name, timeliness of performance, customer satisfaction, and dollar value of the project- provide at least three (3) recent examples. Past Performance Questionnaire (Form PPQ-0), found as Attachment A, can be used to assist with providing a company's capabilities. However, the use of this form is not required. PLEASE SHOW CAPABILITES IN WORKING ON PROJECT SITES WHERE THERE IS NO LAND ACCESS TO THE PROJECT. SHOW PAST EXPERIENCE WORKING ON PROJECTS THAT REQUIRE RIVER ACCESS. PLEASE SHOW CAPABILITIES PERFORMING EARTHWORK INCLUDING CLEARING AND GRUBBING, SHAPING MATERIAL TO SPECIFIC SLOPES AND ELEVATIONS AND EXCAVATION OF WET SOILS AND CONSTRUCTING BERMS TO SPECIFIC SLOPES AND ELEVATIONS. PLEASE SHOW CAPABILITIES WITH PLANTING VEGETATION INCLUDING TREES, WETLAND PLANTS, GRASSES AND NATIVE SHRUBS. SHOW PAST EXPERINCE WITH ADDRESSING AND DEALING WITH PLANTINGS THAT HAVE LONG LEAD TIMES.
