- This modification extends the closing date of the solicitation due at NOON EST Monday, November 16, 2015. REQUEST FOR QUOTES (RFQ) Mat Rental, Cleaning, and Replacement Services This is a combined synopsis/solicitation (RFQ) for rental, cleaning and replacement of mats located at Defense Finance and Accounting Services (DFAS) site in Rome, NY in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. This will be a firm fixed-price contract type. NAICS code for this announcement is 532299 with small business size standard $7.5 M. SOLICITATION CLOSING DATE: All quotes must be received by Noon (EST), Tuesday, November 10, 2015. Preferred delivery method for submission of your quote is via email: diane.m.hulett.civ@mail.mil. Quotes shall include at a minimum, in addition to the items listed in the section Basis for Award below, the following: • Unit price per CLIN as described on pricing table separately attached. • Vendor Contact Name, Phone Number and Email Address • Company Cage Code or DUNS Number • Tax Identification Number • Business Size: Small/Large/Other: _______________ Note: A pricing table as a separate attachment has been provided to assist with supplying the required pricing and vendor contact information. IMPORTANT REMINDER: Prior to submission of quote, please verify your registration is current and active via the System for Award Management (SAM): https://www.sam.gov/portal/public/SAM/. DESCRIPTION OF REQUIREMENT: (DFAS) Rome, NY requires rental of walk-off mats in various sizes that will be delivered, picked up and replaced with clean mats every two weeks from April 1 through November 14 and once per week from November 15 through March 31 as described in the Statement of Work (SOW) (attached separately). Period of Performance: One-year base period (anticipated 11/15/15 through 11/14/16) and four one-year option periods. Conformance to SOW: You must provide the services in accordance with the SOW. Unless an exception is specifically noted within your quote, your quote will be considered to be in complete conformance to the solicited requirements. BASIS FOR AWARD: The Government will award a contract resulting from this announcement to the responsible quoter whose quote conforming to the solicitation will be most advantageous to the Government, price and other factors considered. Vendors should address the following in their quote: PAST PERFORMANCE: The provided information should clearly demonstrate or identify the following: relevant past performance within the last three (3) years for at least three (3) references which demonstrate the ability to provide mat rental, cleaning, replacement and services. (Services provided at similar Government facilities are preferred.) Please include applicable point of contact information such as name, phone number and email address for relevant past performance. PRICE: A table has been included as a separate attachment titled Pricing Table to assist with submission of pricing for your convenience. The provided information should clearly identify the price per month for the base year and four options years. All quotes will be evaluated to determine that the quoted price(s) reflect a technical understanding of the requirement and those considered unrealistic may be rejected. Option(s), inclusive of any potential extensions in accordance with FAR 52.217-8, will be evaluated at time of award. (Ref FAR 52.217-5) The Government will evaluate quotes for award purposes by adding the total price for all options to the total price for the basic requirement. Evaluation of options will not obligate the Government to exercise the option(s). DFAS reserves the right to issue an order to a vendor for reasons other than the lowest price. An order may be issued based upon initial responses and awarded without discussions or negotiations. Therefore, each response should be submitted in the most favorable terms satisfying the RFQ requirement. Failure to submit quotes as requested under this RFQ or attached SOW may be subject to be non-responsive. Failure to meet a requirement will not necessarily subject a quote to disqualification. To permit a through and effective evaluation, the quote should be as concise, complete and clear as possible so as to permit the Government to make evaluation of the quote without further information being required from the quoter. The Government reserves the right to make a determination whether a quote is acceptable or unacceptable solely on the basis of the information submitted and proceed without requesting additional information. (End of Clause) FAR 52.217-8 OPTION TO EXTEND SERVICES (NOV 1999) The Government may require continued performance of any services within the limits and at the rates specified in the contract. These rates may be adjusted only as a result of revisions to prevailing labor rates provided by the Secretary of Labor. The option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed 6 months. The Contracting Officer may exercise the option by written notice to the Contractor within the period up to 14 days prior to the end of the contract.. (End of Clause) FAR 52.217-9 OPTION TO EXTEND THE TERM OF THE CONTRACT (MAR 2000) (a) The Government may extend the term of this contract by written notice to the Contractor within the period up to 14 days prior to the end of the contract; provided that the Government gives the Contractor a preliminary written notice of its intent to extend at least 15 days before the contract expires. The preliminary notice does not commit the Government to an extension. (b) If the Government exercises this option, the extended contract shall be considered to include this option clause. (c) The total duration of this contract, including the exercise of any options under this clauses, shall not exceed 5 years. (End of Clause) STATEMENT OF WORK Defense Finance and Accounting Service DFAS Rome, NY 325 Brooks Road Rome, NY 13441 November 4, 2015, 2015 The purpose of this contract is to establish a contract for delivery, placement, and rotation of walk off carpets at the facility entry points. General • Walk off carpets shall be exchanged every two (2) weeks from 1 April thru 14 November, and once a week 15 November thru 31 March. Total number of carpets is 27. • A spreadsheet is provided for type, number, schedule and location of walk off carpets. • The pick-up and exchange can be scheduled with the COR. • Collection shall be performed as scheduled during periods of inclement weather unless specific approval to delay collections is requested by the Contractor and approved by the GPOC/COR. Such approval will normally be granted only in cases of particularly severe weather. When approval to reschedule collections is granted, the contractor shall accomplish all missed collections the next Business Day after the severe weather has terminated, at no additional cost to DFAS. • If a normal delivery falls on a Federal Holiday the delivery will be made on the Next Business Day following the Holiday at no additional cost to the Government. • Pick-up and delivery will be between the core hours of 9:00 AM and 3:00 PM Monday thru Friday. Performance Standards • Replacement free of dirt, dust, spots, streaks, bugs, graffiti, and all foreign residue • All carpets of the correct size, and placed correctly Specifications of Mats • Rubberized Backing, short pile, black with edge banding. Non-Personal Services: The Government shall neither supervise contractor employees nor control the method by which the contractor performs the required tasks. It shall be the responsibility of the contractor to manage its employees. Escort will be required if employee is not able to obtain clearance. Employee/Technician will be required to submit paperwork for clearance approval. Business Relations The contractor shall successfully integrate and coordinate all activity needed to execute the requirement. The contractor shall manage the timeliness, completeness, and quality of problem identification. The contractor shall seek to ensure customer satisfaction and professional and ethical behavior of all contractor personnel. Contract Administration and Management The contractor shall establish processes and assign appropriate resources to effectively administer the requirement. The contractor shall respond to Government requests for contractual actions in a timely fashion. The contractor shall have a single point of contact between the Government and Contractor personnel assigned to support the contract. Security and Safety • Security The contractor will be required to successfully complete the DFAS security Vetting process and wear, in plain view, a DFAS pass/badge at all time within the facility. The contractor shall provide the COR with the names for all personnel working within DFAS-RO during the contract period upon award. • Safety The contractor shall comply with all applicable OSHA requirements for permit required confined spaces and Lockout/Tag-out procedures (29 CFR 1920.146) and safety requirements during all maintenance operations of the elevator(s) and associated systems. Contractor Manpower Reporting The contractor shall report ALL contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract for the Defense Finance and Accounting Service (DFAS) via a secure data collection site. The contractor is required to completely fill in all required data fields using the following web address: http://www.ecmra.mil. Reporting inputs will be for the labor executed during the period of performance during each Government fiscal year (FY), which runs October 1 through September 30. While inputs may be reported any time during the FY, all data shall be reported no later than October 31 of each calendar year, beginning with 2013. Contractors may direct questions to the help desk at http://www.ecmra/mil. Contracting Officers Representative (COR) The COR will act as the on-site technical point of the contract for the Government. He will initiate repair and replacement request and monitor performance of goods. The COR’s authority is limited to technical issues and he is not authorized to make contractual decisions. The authority to resolve monetary issues and contractual interpretation is the responsibility of the Contracting Officer Representative.
