Kingsbridge Road Rm. 9b64 Bronx NY 10468 Synopsis: Please note that this is not a request for quotes or proposals, the Government is seeking information for market research purposes only. The Department of Veterans Affairs, Northport VA Medical Center, is conducting a market research to find qualified vendors to provide the following service for the Northport, NY Campus: The vendor shall provide and install new stainless steel wire located on the exterior container shell, fasteners, and lubricant grease for the water level indicator device on the 300,000 gallon elevated water storage tank. The vendor shall exclude any work on interior wet container associated with water level indictor. The North American Industry Classification Systems (NAICS) is 221310. Responses to this notice must be submitting in writing via email and must be received no later than November 16, 2015 at 10 AM EST. The purpose of this notice is to gain knowledge of potential qualified sources for this procurement relative to NAICS 221310. Qualified offerors shall have experience capable of supporting this facility. Responses to this notice will be used by the Government to make appropriate acquisition decisions. Responses to this notice should include company/individual name, a service capability statement, examples of similar facilities which you have provided services to, DUNS number, address, and point of contact. If your firm is a Service-Disabled Veteran Owned or Veteran Owned Small Business, you must be CERTIFIED in VetBiz (see internet site: http://vip.vetbiz.gov). Contractor must be registered in System for Award Management (SAM) (see internet site: http://www.sam.gov). Responses to this notice must be submitted in writing (email or fax) and received no later than November 16, 2015 at 10:00 AM EST. If a solicitation is issued, it will be announced on Federal Business opportunities website http://www.fbo.gov at a later date, and all interested parties must respond to that solicitation announcement separately from the responses to this announcement. Email: Marshand.boone@va.gov STATEMENT OF WORK A.GENERAL INFORMATION 1.Title of Project: Maintenance of water level indicator located on the outside of the 300,000 gallon elevated water storage tank. 2.Scope of Work: The vendor shall provide and install new stainless steel wire located on the exterior container shell, fasteners, and lubricant grease for the water level indicator device on the 300,000 gallon elevated water storage tank. The vendor shall exclude any work on interior wet container associated with water level indictor. 3.Background: The Northport VAMC's existing water level indicator has worn steel wire, fasteners, and grease and it requires maintenance. 4.Performance Period: The vendor shall provide the service for the Plumbing Shop located at the Department of Veterans Affairs Medical Center, 79 Middleville Road, Northport NY 11768 by 11/30/2015. 5.Type of Contract: The anticipated contract type is firm fixed price. B.GENERAL REQUIREMENTS 1.The maintenance to the water level indicator shall occur between Monday thru Friday (8am-4:30pm). 2.Upon completion of the maintenance service, the maintenance shall be inspected by the Plumbing Shop Supervisor, Charlie Solicito. 3.Report of acceptance of delivery. Copy of report shall be submitted to the Chief, Engineering Service. 4.Inspections shall be coordinated with the Plumbing Shop Supervisor, Charlie Solicito at 631-261-4400 x 2925. 5.Vendor shall show valid ID at police check point for access to the facility's repair site. D. CHANGES TO STATEMENT OF WORK 1.Any changes to this SOW shall be authorized and approved only through written correspondence from the Contracting Officer. A copy of each change will be kept in a COR folder along with all other required documents. Costs incurred by the vendor through the actions of parties other than the Contracting Officer shall be borne by the vendor. E. GOVERNMENT RESPONSIBILITIES 1.The government agrees to pay fair market price plus applicable shipping fee as well as a federal excise tax. F. CONTRACTOR PERSONNEL SECURITY REQUIREMENTS 1.The vendor shall not have access to the Department of Veterans Affairs' computer systems. The certification and accreditation (C&A) requirements do not apply and a Security Accreditation Package is not required.
