Description
Synopsis - Competitive Small Business 100% Set-Aside Solicitation # FA3010-16-R-0003 Class Code: R497 NAICS Code: 622110 Description: Special Needs Coordinator (SNC) - Exceptional Family Member Program Nurse (EFMP Nurse) This notice serves to announce the intent of the 81ST Contracting Squadron to issue a solicitation for One (1) FTE Special Needs Coordinator (SNC) Exceptional Family Member Program Nurse (EFMP Nurse) - on a personal services basis at the Keesler Medical Center located at Keesler Air Force Base, MS. Contractor personnel shall work in the Exceptional Family Member Office at the Keesler Medical Center, Keesler Air Force Base (KAFB), Mississippi (MS), also referred to as the "medical treatment facility" (MTF) herein. Performance shall be in accordance with (IAW) the requirements contained in this PWS and the professional standards of the Joint Commission (JC). The SNC -EFMP Nurse shall oversee and manage the Exceptional Family Member Program - Medical (EFMP-M) IAW Department of Defense (DoD) and Air Force (AF) policy, and any subsequent implementing guidance. The SNC establishes and maintains procedures to identify family members with special medical and educational needs in a timely manner in order to coordinate accompanied travel orders and family relocations. The projected period of performance will consist of a base year (28 Nov15- 27 Nov 16) and four (4) option years, not to exceed 66 months. This competitive acquisition is set aside 100% for Small Business. The associated North American Industry Classification System (NAICS) code for this acquisition is 622110 and the small business size is $38.5 Million. The requirement is issued as a commercial service using Simplified Acquisition Procedures under the authority of FAR 13.5 The government anticipates award of a firm- fixed price contract as a result of the solicitation. Contractors eligible for award must be registered in the System for Award Management (SAM) prior to award with any DOD agency. Registration may be accomplished at website, http://www.sam.gov. Contractors must also submit their representations and certifications. After award all invoices shall be electronically submitted via Wide Area Work Flow. Registration and invoicing may be accomplished at https://wawf.eb.mil/. The Performance Work Statement (PWS) will be posted with the Solicitation. The solicitation will be available to download via http://www.fedbizopps.gov on or about 09 November 2015. The solicitation may be posted earlier or later. It is the potential offerors responsibility to monitor the website for solicitation release, postings, changes and amendments and closing date. No paper copies will be available and no electronic or verbal responses will be accepted. Interested parties may identify their interest and capability by responding to the solicitation with a delivered hard copy signed proposal (original signature) with signed amendments (if any) by the posted close date and time.
