REQUEST FOR QUOTE VA263-15-Q-0120 WHEELCHAIR TRANSPORTATION SERVICE FOR THE DES MOINES VA HEALTH CARE SYSTEM (VAHCS) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Quotes are being requested, and a written solicitation will not be issued. Quotes shall reference the solicitation number. This Request for Quote (RFQ) document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-84, 09/03/2015. The Sioux Falls Network Contract Office 23 intends to award a firm-fixed-price, Indefinite Quantity Contract for Wheelchair Transportation Service for the Des Moines VA Health Care System located in Des Moines, Iowa. The intent is to award a 12-month or one-year contract. The North American Industrial Classification System (NAICS) code for this procurement is 485991, Special Needs Transportation, with a small business size standard of $15.0 million. This requirement is set aside for 100% Small Business. All contractors must be registered in the System for Award Management (SAM) at https://www.sam.gov and be a small business registered under NAICS 485991 in order to be considered for contract award. The record must be complete and not missing elements such as representations and certifications. Questions concerning this solicitation shall be addressed to John Becker, Contracting Officer, and e-mailed to john.becker3@va.gov. All questions or inquires must be submitted in writing no later than 2 days prior to the solicitation closing day/time. No phone calls will be accepted. Any amendments to this solicitation will ONLY be available on the Federal Business Opportunities (FBO) website (www.fbo.gov). This solicitation is issued under the authority of FAR 13.5 Test Program for Certain Commercial Items and will be evaluated in accordance with FAR 13 procedures. The following factors shall be used to evaluate offers: Technical, Past Performance, and Price. The Lowest Price Technically Acceptable (LPTA) source selection process will be used to evaluate offers. The Government will award a contract resulting from this RFQ to the responsible contractor whose quote is the LPTA offer. To be Technically Acceptable, offerors shall submit the following documentation: (1)Offeror shall provide a copy of Certificate of Liability Insurance reflecting the levels of coverage outlined in the solicitation document. (2)Offeror shall provide State of Iowa Certificate of Authority to operate business in State of Iowa (3)Offeror shall provide a copy of Commercial Driver's License (CDL), chauffeur's license, Class D, endorsement 3 from the State of Iowa, for all vehicle operators that will perform service under the contract. (4)Offeror to identify all Federal, State, and local government contracts and private contracts of similar type, scope, size, and complexity that are ongoing, or have been completed within the past three (3) years. Include the company name, description of services performed, point of contact, telephone number, and email addresses of responsible individual(s) who has firsthand knowledge of performance relative to the same type of service required for this solicitation. Quotes are due the day/time identified in the FBO announcement. Quotes received after this date and time will be considered late in accordance with 52.212-1(f) and will not be evaluated. Quotes shall be submitted via e-mail to john.becker3@va.gov.
