Description

SOURCES SOUGHT This is not a solicitation announcement. This is a sources sought synopsis only. The purpose of this is to gain knowledge of potential qualified sources and their size relative to NACIS 811219. Responses will be used by the Government to make appropriate acquisition decisions. Responses are not considered responses to any potential solicitation announcement. (DRAFT) STATEMENT OF WORK Medical Equipment Maintenance and Repair I. Specific Statement of Work for Medical Equipment Maintenance and Repair Seeking Contractor capable of furnishing labor, materials, parts, equipment, tools, transportation and supervision to perform preventative maintenance and emergency repairs on the Terumo Advanced Heart Perfusion System (S/N 0193) used in cardiovascular surgery at the Michael E. DeBakey VA Medical Center, 2002 Holcombe Blvd., Houston, TX 77024. Contractor shall provide preventative maintenance performed by licensed professionals, using only original equipment and manufacturer parts. Contractor shall travel to VA Medical Center and perform periodic preventative maintenance twice per year and perform all emergency repairs on the Terumo Advanced Heart Perfusion System (S/N 0193) as needed. Contractor shall be required to verify and document that the Perfusion System is operating properly and communicate with MEDVAMC staff when the system is ready for use. Contractor shall ensure that there is no impact to patient care during preventative maintenance. II. Security Contractor shall not have electronic connectivity to the devices. Any electronic or magnetic data storage devices, such as hard drives etc. are not to be removed from the VA facility and shall be turned in to the Information Security Officer (ISO). III. Period of Performance 1.Base Period: November 1, 2015- October 31, 2016 Option year 1 - November 1, 2016 - October 31, 2017 Option year 2 - November 1, 2017 - October 31, 2018 Option year 3 - November 1, 2018 - October 31, 2019 Option year 4 - November 1, 2019 - October 31, 2020 2. Place of Performance: Michael E. DeBakey VA Medical Center 2002 Holcombe BLVD Houston, TX 77030 IV. Responses All interested firms shall provide the following information: company name, address, point of contact, phone number, email address, capability statement, Duns & Bradstreet number and business size pursuant to the NAICS code 811219. Responses to this notice shall be e-mailed to Darnell.Rias@va.gov. Telephone responses will not be accepted. Responses must be received in writing no later than: Tuesday, October 13th at 11 am CST. This notice is to assist the VA in determining sources only. A solicitation is not currently available. If a solicitation is issued it will be announced at a later date, and all interested parties must respond to that solicitation announcement separately from the responses to this announcement. Responses are not a request to be added to a prospective bidders list or to receive a copy of the solicitation.
