- RFQ Document with CLIN descriptions (i) This is a combined synopsis/solicitation for commercial items, prepared in accordance with the format in subpart 12.6 of the FAR and as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a separate written solicitation will not be issued. (ii) Solicitation number 15-236-SOL-00044 applies, and is issued as a Request for Quote (RFQ). (iii) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-83. (iv) The North American Industry Classification System (NAICS) code is 423440 with a size standard of 100 Employees. The Indian Health Service (IHS), Division of Acquisition Policy intends to solicit a firm fixed priced purchase order to Small Businesses only. (v) See the attached RFQ Document for Contract Line Items (CLINs) and pricing structure. (vi) The IHS is in need of miscellaneous items for a Data Center. **See the attached RFQ document for details. (vii) The delivery and acceptance term for this order is FOB Destination. (viii) The provision at 52.212-1, Instructions to Offerors - Commercial Items (Apr 2014), applies to this acquisition and a statement regarding any addenda to the provision (ix) 52.212-2, Evaluation - Commercial Items (Oct 2014) (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The Government intends to evaluate offerors and award a contract without discussions with offerors. Therefore, the initial offer should contain the offerors best terms from a technical and price standpoint. However, the Government reserves the right to conduct discussions if later it is determined by the Contracting Officer to be necessary. The Government may reject any or all offers, waive informalities and minor irregularities in offers received. The following factors will be used to evaluate offers: Lowest Price Technically Acceptable. THIS IS A BRAND NAME OR EQUAL REQUIREMENT. See the attached RFQ document for manufacturer, part #s and salient characteristics. (x) FAR 52.212-3, Offeror Representations and Certifications - Commercial Items (Mar 2015) - The offeror shall complete only paragraphs (b) of this provision if the Offeror has completed the annual representations and certification electronically via the System for Award Management (SAM) Web site accessed through http://www.acquisition.gov. If the Offeror has not completed the annual representations and certifications electronically, the Offeror shall complete only paragraphs (c) through (p) of this provision. (xi) FAR 52.212-4 Contract Terms and Conditions - Commercial Items (May 2015) applies to this acquisition.
