This announcement constitutes the only solicitation; QUOTES ARE BEING REQUESTED AND A WRITTEN SOLICITATION WILL NOT BE ISSUED. The Combined Synopsis/Solicitation number is W912D0-15-T-0828 and incorporates provisions and clauses for commercial items in effect through Federal Acquisition Circular 2005-83. This acquisition is set aside 100% for Small Business. The NAICS Code is 561730 with a size standard of $7.5M. NOTE: The quantities are ESTIMATES and may increase or decrease depending on weather conditions. Vendors shall include all associated costs for lawn maintenance in the weekly rate for lawn maintenance and shall include all costs associated with snow removal in the price per "job". Description of Requirement: ITEM NO SUPPLIES/SERVICES QUANTITY UNIT LINE ITEM COST 0001 Lawn Maintenance 18 Week The Contractor shall provide all personnel, supplies, facilities, transportation, tools, materials, supervision, and other items and non-personal services necessary to perform lawn mowing maintenance services as defined in the Performance Work Statement (PWS) section 5 except for those items specified as government furnished services. Lawn mowing and maintenance services to turf areas of Child/Youth Service Facilities will be required after facility hours of operation throughout the duration of the service contract. The contractor shall collect and dispose of trash and debris from lawn prior to each mowing. Lawn shall be mowed to a turf height of 2 ½ to 3 inches without removing more than 1/3 of the total leaf blade height. Lawn clipping mulch shall remain on turf to recycle into the lawn. The contractor shall trim around areas which are not feasible by other moving equipment, edge sidewalks, driveways, curbs, and sweep clippings and debris off of sidewalks, driveways or other applicable hard surfaces. The contractor shall be responsible for any damage to Government property. Lawn mowing maintenance services shall be provided weekly between the periods of 15 May - 15 September. Period of Performance: 15 November 2015 to 14 November 2016 (Base year) ITEM NO SUPPLIES/SERVICES QUANTITY UNIT LINE ITEM COST 0002 Snow/Ice Removal 33 Job The Contractor shall provide all personnel, supplies, facilities, transportation, tools, materials, supervision, and other items and non-personal services necessary to perform snow/ice removal from sidewalks as defined in the Performance Work Statement (PWS) section 5 except for those items specified as government furnished services. Snow removal and ice removal from the sidewalks of Child/Youth Service Facilities will be required before and during facility hours of operation throughout the duration of the service contract. This removal will be required anytime snow/ice accumulations on the sidewalks exceeds 1-inch, or upon notification by the Contract Officer Representative (COR) that slippery or slushy conditions exist which present a potential safety concern regardless of snow depth. The Contractor is required to enter data into the Contractor Manpower Reporting Application (CMRA) system as outlined in local information titled Contractor Manpower Reporting located with the clauses incorporated into the contract. Reporting period will be the period of performance not to exceed 12 months ending 30 September of each Government fiscal year and must be reported by 31 October of each calendar year. Evaluation Criteria: Award will be made to the lowest priced vendor meeting the specifications of the requirement. Quotes shall be evaluated for award purposes, based upon the total price proposed for the base and each option year. As part of the price evaluation the Government will evaluate its option to extend services under FAR Clause 52.217-8 by adding one-half of the offeror's final option period prices to the offeror's total price. Thus, the offeror's total price for the purpose of evaluation will include the base period, 1st option, 2nd option, 3rd option, 4th option, and ½ of the 4th option price. Offerors SHALL NOT enter a price for the 6-month period. Offerors are required to upload provision FAR 52.212-3 Offeror Representations and Certifications Commercial Items into the System for Award Management (SAM), https://www.sam.gov, before quote is submitted. The Quote must include Solicitation Number, Business size, and discount/payment terms. Offeror must include Taxpayer Identification Number (TIN), Cage Code, and address. Submission of Invoices In accordance with FAR clause 52.212-4(g) the contractor shall submit all invoices electronically utilizing Wide Area Workflow (WAWF). Procedures for using WAWF are found in DFARS Clause 252.232-7003 Electronic Submission of Payment Requests and Receiving Reports. Site Visit: A site visit will be conducted on 23 September 2015 at 10:30 AM. Participants shall meet at Building 4166, Fort Wainwright AK 99703. All vendors participating in the site visit are required to notify the primary POC and alternate POC, listed below prior to the date of the site visit. The Contracting Officer may exercise the option by written notice to the Contractor within 10 days of the contract end date. (c) The total duration of this contract, including the exercise of any options under this clause, shall not exceed 5 Years and 6 Months. The required information includes: (1) Contracting Office, Contracting Officers, Contracting Officer's Technical Representative; (2) Contract number, including task and delivery order number; (3) Beginning and ending dates covered by reporting period; (4) Contractor name, address, phone number, e-mail address, identity of contractor employee entering data; (5) Estimated direct labor hours (including subcontractors); (6) Estimated direct labor dollars paid this reporting period (including subcontractors); (7) Total payments (including subcontractors); (8) Predominant Federal Service Code (FSC) reflecting services provided by contractor (and separate predominant FSC for each subcontractor if different); (9) Estimated data collection costs; (10) Organizational title associated with the Unit Identification Code (UIC) for the Army Requiring Activity (the Army Requiring Activity is responsible for providing the contractor with its UIC for the purposes of reporting this information); (11) Locations where contractor and subcontractors perform work (specified by zip code in the United States and nearest city, country, when in an overseas location, using the standardized nomenclature provided on the website); (12) Presence of deployment or contingency contract language; and (13) Number of contractor and subcontractor employees deployed in theater this reporting period (per country. As part of this submission, the contractor will also provide the estimated total cost (if any) incurred to comply with this reporting requirement. Reporting period will be the period of performance not to exceed 12 months ending September 30 of each government fiscal year and must be reported by 31 October of each calendar year. Contractors may use a direct XML data transfer to the database server or fill in the filed on the website. The XML direct transfer is a format for transferring filed from a contractor's systems to the secure website without the needfor separate data entries for each required data element at the website. The specific formats for the XML direct transfer may be downloaded from the website. Note: Information from the secure website is considered to be proprietary in nature when the contract number and contractor identity are associated with direct labor hours and direct labor dollars. AT no time will any data be released to the public with the contractor name and contract number associated with the data. Instructions to Vendors Vendors must electronically submit a signed copy of DFAR 252.209-7992 (Attach 5) as well as a fully completed bid sheet (Attach 4) NLT the date and time specified above. In addition vendors must be registered in SAM as a small business under NAICS code 561730 to be eligible for award.
